AGENDA ITEM TITLE: 2009 Ambulance Licensing: Advanced Life
Support

DESCRIPTION OF ISSUE:
Rich Walker, Chairman of the Emergency Medical Trauma Advisory Council, is
requesting approval by the BOCC for licensure approval for Stadium Medical Inc.for
one special event, Denver Post Ride the Rockies June 17, 18, 19 and 20, 2009.
A. Advanced Life Support Ambulances
Stadium Medical: Ambulances Medic OS and 07
The applications for Stadium Medical Inc. Service and the 2 Ambulances approved by
the Stadium Medical Service Director and Physician advisor is on file at the County
Clerk's office. As well there was a unanimous vote to recommend approval for this
License by the Pitkin County Emergency Medical Trauma Advisory Council
(EMTAC).
RECOMENDED BOCC ACTION: Staff recommends approval of licenses.

PITKIN COUNTY AMBULANCE LICENSE FOR:
STADIUM MEDICAL INC.
MEDIC OS AND 07
GRANT OF LICENSE/PERMIT
The undersigned, representing the Board of County Commissioners of Pitkin County,
Colorado, does hereby grant a license to the above-named applicant to provide
ambulance service within Pitkin County. This license shall have upon it any
restrictions listed below, shall be granted any waivers listed below, and shall be valid
for the dates listed below, unless revoked by the Board pursuant to the provisions of
Title Six (Health and Safety-Ambulance Licensing) of the Pitkin County Code and any
other applicable County ordinance or law of the State of Colorado. Licensee shall
comply with all applicable laws and regulations, including but not limited to Article
3.5 of Title 25, C.R.S.
RESTRICTIONS/CONDITIONS: License is only for the one special event June 17,
18, 19 and 20, 2009 for the Denver Post Ride the Rockies.
WAIVERS GRANTED: Not Applicable
THIS LICENSE IS VALID FOR: June 17, 18, 19 and 20, 2009.
