Septic permit approved per compliance with the engineer design and specifications with engineer stamp dated
9/9/99. Any changes must be approved by this department and the design engineer pr 2f to them being made.
System is design for a 2,000 concrete septic tank. Per Pitkin County regulations a 30% reduction is taken for the
use of "EQUALIZER -36" Infiltrator Units in a trench configuration using an equivalency area of 24.79 sq. ft. The
design shall be 6 trenches of 10 units long for a total of 60 units. There shall be 6 feet of undisturbed soil between
the trenches.
Minimum horizontal distances between components of the system and physical features shall conform to the Pitkin
County ISDS regulations.

Plans and specifications of the pro sed individual sewage disposal system have been reviewed and are considered
satisfactory. Permission is hereby granted to the owner or the agent to perform the work indicated in accordance with the Pitkin
County ISDS Regulation in effect at the time of issue. This permit becomes invalid 6 months from the date that the permit was
issued unless system construction has commenced or an extension has been approved in writing by the Department.
As -built drawings must be included with this permit before the final approval will be issued.

The fee for a ISIDS application is $330 for a permit that takes 6 hours or less for the department to approve. If approval takes
longer than 6 hours, a rate of $55 per hour will be charged. The maximum fee is $1000. The basic fee of 3330 is due at the time
of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit: second, before
final approval of the ISDS permit.
Application for an individual sewage disposal system is hereby submitted. I hereby certify that the above information is true and
accurate and that I have provided true and accurate information on locations of all existing and proposed wells, contour
intervals, buildings, property lines, ditches, slopes, waterlines, springs, suction or irrigation lines, drinking water cisterns, drain
tiles, irrigation ditches, lakes, water courses, streams, floodplains, dry gulches, and existing septic systems. I hereby certify
that any such features not shown on attached site map are not present. Issuance of the permit does not imply the approval of
any other permit required for construction pursuant to Pitkin County codes. No construction may be undertaken until all
approvals and permits have been obtained.
The Aspen/Pitkin Environmental Health Department. Pitkin County and employees of these agencies will be held harmless
should the individual sewage disposal system fail or malfunction. The permit to construct is issued on information submitted by
the applicant or his/her representatives. The owner assumes full responsibility in case of failure of the system.

Exploratory pits were excavated on July 22, 1999 with a backhoe.

Locations of exploratory pits were measured approximately by pacing from features on the site
plan provided.

Elevations of the exploratory pits were obtained by interpolation between contours on the site plan
provided. Logs are drawn to depth.

The exploratory pit locations and elevations should be considered accurate only to the degree
implied by the method used.

On April 6, 2000 Jerome Gamba & Associates, Inc performed a final inspection of the Roberts
Residence Individual Septic Disposal System (I.S.D.S.) as constructed.
The system as observed by Jerome Gamba & Associates, Inc., was found to be in substantial
conformance to the plans and specifications signed and stamped September 9, 1999 except for the
following field design revisions:
1. The septic tank is located at the approximate center of the residence, situated parallel to the
house.
2. An additional clean-out was added after the septic tank, situated to access the run of 4 inch
pipe to the distribution box.
3. Top trenches have a 48 -inch bury depth to the top of the Infiltrator® unit. The lower
trenches have a bury depth the top of the unit of 24 inches.
4. The separation between the leach trenches is approximately 20 feet.
5. Flow Equalizer units were added to pipes in Distribution box.

CONDITIONS OF APPROVAL
11 This well shall be used in such a way as to cause no material injury to existing water rights. The issuance of
the permit does not assure the applicant that no injury will occur to another vested water right or preclude
another owner of a vested water right from seeking relief in a civil court action.
2) The construction of this well shall be in compliance with the Water Well Construction Rules 2 CCR 402-2,
unless approval of a variance has been granted by the State Board of Examiners of Water Well Construction
and Pump Installation Contractors in accordance with Rule 18.
3) Approved pursuant to CRS 37-92-602(3)(c) for the relocation of an existing well, permit 165735. The old well
must be plugged and sealed according to the Water Well Construction Rules within ninety (90) days of
completion of the new well. The enclosed Well Abandonment Report form must be completed and submitted
to affirm that the old well was properly plugged and sealed.
4) Approved as the only well on a residential site of ± 15.81 acres described as that portion of Lot 15 of Sec. 14,
Twp. 8 South, Rng. 88 West, 6th P.M., Pitkin County, more particularly described on attached exhibit A. This
property is currently vacant and a physical address has not been assigned.
5) The use of ground water from this well is limited to ordinary household purposes inside one (1) single family
dwelling, and the watering of the user's noncommercial domestic animals. The water withdrawn from this well
shall not be used for irrigation or other purposes.
6) The maximum pumping rate shall not exceed 15 GPM.
7) The return flow from the use of this well must be through an individual waste water disposal system of the
non -evaporative type where the water is returned to the same stream system in which the well is located.
8) This well shall be located not more than 200 feet from the location specified on this permit.
9) Approved for the installation of a pump in, and the use of, an existing well, constructed on January 20, 1999,
to a depth of 440 feet, under monitoring hole notice MH -35455, acknowledged January 11, 1999.
NOTE: Expired permit 157082, was previously issued for this lot.
