If approval takes longer than 6 hours, a rate of $55 per hour will be charged. The maximum fee is $1000. The basic fee of 3330 is due at the time of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit: second, before final approval of the ISDS permit. Application for an individual sewage disposal system is hereby submitted. The system as observed by Jerome Gamba & Associates, Inc., was found to be in substantial conformance to the plans and specifications signed and stamped September 9, 1999 except for the following field design revisions: 1. The septic tank is located at the approximate center of the residence, situated parallel to the house. 2. An additional clean-out was added after the septic tank, situated to access the run of 4 inch pipe to the distribution box. 3. Top trenches have a 48 -inch bury depth to the top of the Infiltrator® unit. The lower trenches have a bury depth the top of the unit of 24 inches. 4.
