Septic permit approved per compliance with the engineer design and specifications dated August 6, 2003. Any
changes must be approved by this department and the design engineer prior to them being made.
Minimum horizontal distances between components of the system and physical features shall conform to the Pitkin
County ISDS regulations. There must be a minimum 132 foot setback between the wells and the field. There must
also be a minimum 82 foot setback between the ditch and the field. These distances must be confirmed.
This system will be using the existing 1500 gallon tank pending the engineer's confirmation of structural integrity.
An effluent filter must be added to the outlet. The effluent will be distributed by serial distribution with 24 EQ -36
Infiltrator chambers.
This department does not endorse any brand of products. This permit must be kept on-site during installation.
The engineer must do a final inspection of the installation and submit an as -built letter to this department. This
department must also be called for an inspection with a minimum of 48 hours advanced notice.

satisfactory. Permission is hereby granted to the owner or the agent to perform the work indicated in accordance with the Pitkin
County ISDS Regulation in effect at the time of issue. This permit becomes invalid 6 months from the date that the permit was
issued unless system construction has commenced or an extension has been approved in writing by the Department.

Percolation Testing, Replacement Septic System, 638 North Thomas Road,
Pitkin County, Colorado.
Dear Mr. Kelly:
As requested, Hepworth - Pawlak Geotechnical, Inc., performed percolation testing at the
subject site. The testing was done in accordance with our agreement for professional
services to you, dated May 21, 2003.
A profile pit and three percolation holes were excavated at the locations shown on Figure
1. The test holes were dug in the bottom of shallow backhoe pits and were soaked by the
client prior to our testing. The subsoils exposed in the percolation holes are similar to
those exposed in the profile pit and consist of about'/2 foot of topsoil overlying slightly
silty sandy gravel with cobbles and boulders to the pit depth of 8 feet. No free water was
observed in the pits and the soils were slightly moist.
Percolation testing was conducted on July 25, 2003, by a representative of Hepworth -
Pawlak Geotechnical, Inc. The percolation test results are summarized on Table 1. The
percolation test results indicate infiltration rates of 6 and 7 minutes per inch. Based on
the subsurface conditions encountered and the percolation test results, the tested area
should be suitable for a conventional infiltration septic disposal system.

On June 2, 2004, a letter was sent to you and your engineer (Timberline Engineering), a copy of
which is enclosed, stating that we still had not received an "As Built" letter from the engineer
who designed the individual sewage disposal system for the fore mentioned property. .
The "As ,Built" .letter should confirm that they observed the installation of the system as designed
and any changes in the system must be noted in the letter or by a new set of plans. Without this,
we have been unable to complete the processing of this permit.
Therefore, we regret to inform you that a copy of this letter will be attached to the property deed
indicating that there is an open issue with the property unless the "As Built" letter is received by
November 16, 2004. -
When Pitkin County Environmental Health receives documentation
have beenmet, steps will be taken to reflect on your deed(s) that the
property no longer pertain.. -

This letter is to advise you that we have not been able to final the Individual Sewage Disposal System
(ISDS) permit for the above stated residence/property.
It is mandatory that this office receives an "As Built" letter from the engineer who designed the
system. The letter will confirm that they observed the installation of the system as designed. Any
changes to the system must be noted in the letter or by a new set of plans. We cannot sign the
Certificate of Occupancy for this property without the "As Built" letter.
This office must receive the "As Built" letter from the engineer no later than 30 days from the date of
