Septic permit approved per compliance with the engineer design and specifications dated August 11, 1999. Any
changes must be approved by this department and the design engineer prior to them being made.
Minimum horizontal distances between components of the system and physical features shall conform to the Pitkin
County ISDS regulations.
This system is sized for a 3 bedroom home. A 1000 gal, 2 -compartment septic tank will be installed. The
absorption field will be 427 sq ft with 3 rows of 10 Infiltrator Units in a bed configuration. Well setback will be
greater than 100 ft and setback from the irrigation ditch to the bed will be greater than 50 ft.

satisfactory. Permission is hereby granted to the owner or the agent to perform the work indicated in accordance with the Pitkin
County ISDS Regulation in effect at the time of issue. This permit becomes invalid 6 months from the date that the permit was
issued unless system construction has commenced or an extension has been approved in writing by the Department.
As -built drawings must be included with this permit before the final approval will be issued.

In January, we'd sent you and your engineer (Timberline Engineering) a letter stating that we still
needed to receive a final letter and as -built drawing for the individual sewage disposal system for your
property in Seven Oaks Subdivision. To date, we have not received the requested information (we'd
asked you to get the information to us no later than 1/25/2000).
As was mentioned in the previous letter, we cannot "final" your sewage disposal system. Without the
system being "finnled", we will not be able to sign any Certificates of Occupancy when they come up for
approval. A building cannot be occupied without a Certificate of Occupancy.
It is imperative that we be able to final your system as soon as possible. Our mission is to protect public
health as well as to protect the groundwater. Both of these issues may be compromised by an
improperly working sewage disposal system. Seeing a system installation through to completion is our
(and the public's) assurance that the system has been installed in accordance with the design and should
work properly. If we do not receive the requested information by June 15'h, 2000, we will attach a copy
of this letter to the property deed indicating that there is an open issue with the property.
On 10/16/99 a final inspection was conducted by Pitkin County Environmental Health and we have been
waiting ever since for this information.

920-5070. If you have sent the information to us and it has crossed in the mail with this letter, please
accept our apologies and disregard this letter.

RESIDENTIAL APPLICATIONS - GENERAL INSTRUCTIONS
There are a variety of uses for ground water in Colorado. This form (GWS -44) can be used when applying for a permit for
a new well or replacement of an existing well for the following types of uses:
ORDINARY HOUSEHOLD USE inside one single family residence (NO outside water use allowed)
OTHER RESIDENTIAL USE (sometimes referred to as "domestic" use) which may include use in up to three single-
family residences, watering of up to one acre of home gardens and lawns, and watering of domestic animals
LIVESTOCK WATERING on a farm, ranch, range, or pasture
If you are applying for a NEW household use only well, or for a NEW 35+ acre residential well outside the Denver Basin or
Designated Basins of eastern Colorado, please use simplified fortes GWS -49 or GWS -50. DO NOT use this form for
registration of an existing unpermitted well (Use Forth GWS -12), monitoring/observation wells (Use Form GWS -46), gravel
pit wells (Use Form GWS -27), or for other uses not listed above, including - commercial, industrial, crop irrigation, municipal,
etcetera (Use GENERAL PURPOSE Forth GWS -45).
FEES Applications must be submitted with the appropriate required non-refundable filing fees. The required filing fee for
most well permit applications is $60. The filing fees for replacement or deepening well permit applications for most
previously permitted residential and livestock water wells is $20.
Checks should be payable to the COLORADO DIVISION OF WATER RESOURCES.
Applications are evaluated in chronological order. Please allow approximately six weeks for processing.
APPLICATIONS must be completed clearly, and legibly, in BLACK INK or typed. ALL ITEMS in the application must'
completed. Incomplete applications may be returned to the applicant for more information. Do not change or atter ti.
application in any way.
THE LOCATION of the well in item 4 must be correctly and accurately described. The county, quarter/quarter, section,
township, range, principal meridian, and distance from section lines must be provided.
NOTE: Distances are not necessarily the same distances as the distances from (your) property lines.
For additional assistance in describing the location of your well, review the publication entitled "How to Determine
Well Location" which was provided with your packet, or can be requested from any Colorado Division of Water
Resource office.
A LEGAL DESCRIPTION of your lot or parcel of land is required in item 5. For tracts of less than 35 acres approval may

Subject: Subsoil Study for Foundation Design and Percolation Test, Proposed

Dear Mr. Batto:
As requested, Hepworth-Pawlak Geotechnical, Inc. performed a subsoil study and
percolation test for foundation and septic disposal designs at the subject site. The study
was conducted in accordance with our agreement for geotechnical engineering services
to you dated October 22, 1998. The data obtained and our recommendations based on
the proposed construction and subsurface conditions encountered are presented in this
report. Evaluation of potential geologic hazard impacts on the site are beyond the scope
of this study.
Proposed Construction: The proposed residence will be a two story log structure with
an attached garage located in the area as shown on Fig. 1. Ground floor of the
residence will be structural over a crawlspace. The garage floor will be slab -on -grade.
Cut depths are expected to be up to about 4 to 6 feet. Foundation loadings for this type
of construction are assumed to be relatively light and typical of the proposed type of
construction. The septic disposal system is proposed to be located about 20 to 30 feet
southwest of the residence.
If building conditions or foundation loadings are significantly different from those
described above, we should be notified to re-evaluate the recommendations presented in
this report.
Site Conditions: The lot was vacant at the time of our field work. The site is located
near the bottom of a northwest facing hillside adjacent to the Crystal River. The river is
about 280 feet to the northwest of the building site and about 28 feet lower in elevation.
The ground surface in the building area slopes moderately steep down to the northwest
at grades up to about 25 %. There is about 6 feet elevation difference in the building
area and 35 feet across the lot. The terrain becomes steeper to the southeast and
northeast of the building area. Vegetation consist of grass and weeds with scruboak on
the steep hillside to the southeast. An existing irrigation ditch is located uphill and to

the east of the building site along the easterly lot line. Sandstone bedrock is exposed
along each side of the valley.
Subsurface Conditions: The subsurface conditions at the site were evaluated by
excavating two exploratory pits in the building area at the approximate locations shown
on Fig. 1. The logs of the pits are presented on Fig. 2. The subsoils encountered,
below about one foot of topsoil, consist of silty sandy gravel with cobbles and boulders.
Results of a gradation analysis performed on a sample of the gravel (minus 3 inch
fraction) obtained from the site are presented on Fig. 3. The laboratory testing is
summarized on Table I. No free water was observed in the pits at the time of
excavation and the soils were slightly moist to moist.
Foundation Recommendations: Considering the subsoil conditions encountered in the
exploratory pits and the nature of the proposed construction, we recommend spread
footings placed on the undisturbed natural gravels designed for an allowable soil bearing
Opressure of 2,000 psf for support of the proposed residence. Footings should be a
minimum width of 16 inches for continuous walls and 2 feet for columns. Loose and
disturbed soils encountered at the foundation bearing level within the excavation should
be removed and the footing bearing level extended down to the undisturbed natural
gravels. Voids created by the removal of large rocks can be backfilled with compacted
sand and gravel or concrete. Exterior footings should be provided with adequate cover
above their bearing elevations for frost protection. Placement of footings at least 42
inches below the exterior grade is typically used in this area. Continuous foundation
walls should be reinforced top and bottom to span local anomalies such as by assuming
an unsupported length of at least 11 feet. Foundation walls acting as retaining
structures should be designed to resist a lateral earth pressure based on an equivalent
fluid unit weight of at least 45 pcf for the on-site gravel, excluding oversized rock, as
backfill.
Floor Slabs: The natural on-site soils, exclusive of topsoil, are suitable to support
lightly loaded slab -on -grade construction. To reduce the effects of some differential
movement, floor slabs should be separated from all bearing walls and columns with
expansion joints which allow unrestrained vertical movement. Floor slab control joints
should be used to reduce damage due to shrinkage cracking. The requirements for joint
spacing and slab reinforcement should be established by the designer based on

experience and the intended slab use. A minimum 4 inch layer of free -draining gravel
should be placed beneath slabs to facilitate drainage and act as a leveling course. This
material should consist of minus 2 inch aggregate with less than 50% passing the No. 4
sieve and less than 2% passing the No. 200 sieve.
All fill materials for support of floor slabs should be compacted to at least 95% of
maximum standard Proctor density at a moisture content near optimum. Required fill
can consist of the on-site gravels devoid of vegetation, topsoil and oversized rock.
Underdrain System: Although free water was not encountered during our exploration,
it has been our experience in mountainous areas that local perched groundwater can
develop during times of heavy precipitation or seasonal runoff. Frozen ground during
spring runoff can create a perched condition. We recommend below -grade
construction, such as retaining walls and crawispace areas, be protected from wetting
and hydrostatic pressure buildup by an underdrain system. The existing irrigation ditch
® above the building site could be a potential source of perched groundwater.

The drains should consist of drainpipe placed in the bottom of the wall backfill
surrounded above the invert level with free -draining granular material. The drain
should be placed at each level of excavation and at least 1 foot below lowest adjacent
finish grade and sloped at a minimum 1 % to a suitable gravity outlet. Free -draining
granular material used in the underdrain system should contain less than 2% passing the
No. 200 sieve, less than 50% passing the No. 4 sieve and have a maximum size of
2 inches. The drain gravel backfill should be at least 11/2 feet deep.
Surface Drainage: The following drainage precautions should be observed during
construction and maintained at all times after the residence has been completed:
1) Inundation of the foundation excavations and underslab areas should be
avoided during construction.
2) Exterior backfill should be adjusted to near optimum moisture and
compacted to at least 95 % of the maximum standard Proctor density in
pavement and slab areas and to at least 90% of the maximum standard

Proctor density in landscape areas. Free -draining wall backfill should be
capped with about 2 feet of the on-site, finer graded soils to reduce
surface water infiltration.
3) The ground surface surrounding the exterior of the building should be
sloped to drain away from the foundation in all directions. We
recommend a minimum slope of 12 inches in the first 10 feet in unpaved
areas and a minimum slope of 3 inches in the first 10 feet in pavement
and walkway areas. A swale will be needed uphill to direct surface
runoff around the residence.
4) Roof downspouts and drains should discharge well beyond the limits of
all backfill.
Percolation Testing: Percolation tests were conducted on November 12, 1998 to
evaluate the feasibility of an infiltration septic disposal system at the site. Three
® percolation holes were dug at the locations shown on Fig. 1. Exploratory Pit 2 was
used as the profile pit due to its proximity to the septic disposal area. The test holes
(nominal 12 inch diameter by 12 inch deep) were hand dug at the bottom of shallow
backhoe pits and were soaked with water one day prior to testing. The test holes were
protected from freezing overnight with rigid foam insulation. The soils exposed in the
percolation holes are similar to those exposed in Pit 21 shown on Fig. 2 and consist of
silty sandy gravel with cobbles. The percolation test (results are presented in Table H.
The percolation test results indicate an infiltration rate between 10 and 23 minutes per
inch with an average of 15 minutes per inch. Based on the subsurface conditions
encountered and the percolation test results, the tested area should be suitable for a
conventional infiltration septic disposal system.
Limitations: This study has been conducted in accordance with generally accepted
geotechnical engineering principles and practices in this area at this time. We make no
warranty either expressed or implied. The conclusions and recommendations submitted
in this report are based upon the data obtained from the exploratory pits excavated at the
locations indicated on Fig. 1, the proposed type of construction and our experience in
the area. Our findings include interpolation and extrapolation of the subsurface
• conditions identified at the exploratory pits and variations in the subsurface conditions
may not become evident until excavation is performed. If conditions encountered
during construction appear different from those described in this report, we should be

notified at once so re-evaluation of the recommendations may be made.
This report has been prepared for the exclusive use by our client for design purposes.
We are not responsible for technical interpretations by others of our information. As
the project evolves, we should provide continued consultation and field services during
construction to review and monitor the implementation of our recommendations, and to
verify that the recommendations have been appropriately interpreted. Significant design
changes may require additional analysis or modifications to the recommendations
presented herein. We recommend on-site observation of excavations and foundation
bearing strata and testing of structural fill by a representative of the geotechnical
engineer.
