The fee for a ISDS application is $330 for a permit that takes 6 hours or less for the department to approve. If
approval takes longer than 6 hours, a rate of $55 per hour will be charged. The maximum fee is $1000. The basic
fee of $330 is due at the time of application. The remainder, if any will be due in two stages: first, at the time of
issuance of the ISDS permit; second, before final approval of the ISDS permit.
Application for an individual sewage disposal system is hereby submitted. I hereby certify that the above information
is true and accurate and that I have provided true and accurate information on locations of all existing and
proposed wells, contour intervals, buildings, property lines, ditches, slopes, waterlines, springs, suction or
irrigation lines, drinking water cisterns, drain tiles, irrigation ditches, lakes, watercourses, streams, floodplains, dry
gulches, and existing septic systems. I hereby certify that any such features not shown on attached site map are
not present. Issuance of the permit does not imply the approval of any other permit required for construction
pursuant to Pitkin County codes. No construction may be undertaken until all approvals and permits have been
obtained.
The Aspen/Pitkin Environmental Health Department, Pitkin County and employees of these agencies will be held
harmless should the individual sewage disposal system fail or malfunction. The permit to construct is issued on
information submitted by the applicant or his/her representatives. The owner assumes full responsibility in case of
failure of the system.

I apologize I didn't get this to you sooner. I field inspected and As -Built the ISDS for Terry & Jean this
summer.
The leach field was built the same as the plans except it was 11.5' from the North property line instead of
10' as shown on the plan.
Two septic tanks were used instead of one as shown on the plans. A 1250 gallon tank 5'-2" from the house
and a 1000 gallon tank 6 feet from the leach field were installed.
Attached is a copy of the site plan with As -Built information on it. If you need any additional information
or have any question please give me a call.

We recommend the surface of the field be seeded using a native seed mix available at seed stores. No landscaping or automatic
sprinkler system is allowed over the field. Livestock should not be confined in the field area.
Our design and recommendations are based upon data supplied to us by others. If subsurface or site conditions are found to be
different from those described in this report, we should be notified to evaluate the effect it may have on the proposed ISDS. If
the County Environmental Health Department requests changes or modifications to this design, we should be contacted to
evaluate the effect on the ISDS.
All construction shall be in accordance with the ISDS Regulations.

Dear Mr. and Mrs. Mobley:

As you requested, we have reviewed the report by Hepworth Pawlak Geotechnical, Inc., dated May 6, 1998 under Job No.
197 575, in which they determined the average percolation was 60 minutes per inch for the new leach bed area. We also
reviewed an improvement survey prepared by the Sexton Survey Co. dated October 10, 1997 under Job No. 97013, which
shows the site conditions.
It is our understanding the existing trailer house is to be removed and a new home with four bedrooms to be constructed. We
have designed an individual sewage disposal system (ISDS) based upon the above-mentioned information fora fhve bedroom
house. The site sloped from the east to west at a moderate slope of 10% to 15%. An existing sewage leach field is located
approximately 10 feet north of an existing garage. This design was based upon a new leach field and the old field to be
abandoned.

As requested, a representative of Hepworth-Pawlak Geotechnical, Inc. performed
percolation testing at the subject site. The results of our work are presented in this
report. We previously performed percolation testing of the northwest portion of the site
reporting our findings on October 3, 1997, Job No. 197 575.
A profile pit and three shallow backhoe pits had been excavated in an area about 100
feet northeast of the existing residence as shown on Fig. 1. The pits had been
excavated and the shallow pits had reportedly been hand dug and pre-soaked by the
client one day prior to our site visit. The subsoils observed in the profile pit consisted of
about 'h foot of topsoil overlying clayey silty sand with gravel and scattered cobbles to
the pit depth of 81h feet. No free water was encountered in the profile pit and the
subsoils were moist.
Percolation tests were performed in the, silty sand with gravels on May 4, 1998 by a
representative of Hepworth-Pawlak Geotechnical, Inc. The testing was conducted in the
bottom of the shallow backhoe pits that were about 3 feet deep. The percolation test
results are summarized on Table I. The percolation test results indicate infiltration rates
of 60 minutes per inch. The percolation rates were based on the last 3 readings of each
test.
Based on the subsurface conditions encountered and percolation test results, the tested
site appears suitable for a conventional infiltration septic disposal system.

engineering services to you dated September 23, 1997.

40 feet northwest of the existing residence as shown on Fig. 1. The pits had been
excavated and the shallow pits had reportedly been pre-soaked by the client one day
prior to our site visit. The subsoils observed in the profile pit consisted of about 1/2 foot
of topsoil ove depth
explored of 8 feet. No free water was encountered in the profile pit and the subs- o`er
were moist.
Percolation tests were performed in the silty sandy gravels on September 26, 1997 by a
representative of Hepworth-Pawlak Geotechnical, Inc. The testing was conducted in
the bottom of the shallow backhoe pits that were about 3 feet deep. The percolation test
results are summarized on Table I. The percolation test results indicate an infiltration
rate between 36 and 60 minutes per inch with an average at about 47 minutes per inch.
The percolation rates were based on the last 3 readings of the test.
Based on the subsurface conditions encountered and percolation test results, the tested
site appears suitable for a conventional infiltration septic disposal system.

The Aspen/ Pitkin Environmental Health Department has reviewed the details of the Mobley
Minor 1041 Hazard Review application under the authority of the Pitkin County Land Use Code
and has the following comments.
ADEQUATE PROVISIONS FOR WATER NEEDS: Sections 2-17 and 3-1104:
"It is the policy of the County to insure the availability of a water supply of adequate quality, quantity, pressure and dependability for fire
protection and support of a proposed land use prior to approval of the use. The County shall require land uses to hook up to existing public
systems if service is available."
The applicant has agreed to serve the project with potable water provided by the Carbondale City
Water System, but has not yet provided a "...letter of intent to service the proposal...'.' as defined in
the Pitkin County Code.
A condition of approval for this application is the receipt of written confirmation from
the Carbondale City Water Department stating they can serve the Mobley residence.
Without meeting this requirement a septic permit and building permit can not be issued.
SEWAGE TREATMENT AND COLLECTION: Sections 2-18 and 3-1105: "it is the policy of the County to
ensure that adequate sewage treatment facilities are available to serve existing and new developments. Public and private sewage disposal systems
and connections to such systems shall comply with the sewage disposal guidelines of Pitkin County's Individual Sewage Disposal System
Regulation."
Lots in the area are served by individual septic systems. All such systems installed within Pitkin
County require septic permits issued by the Environmental Health Department, and must comply
with the County's septic regulations. To receive a septic permit, the applicant will need to submit
a septic permit application through a Registered Professional Engineer who will design the
domestic wastewater system. Included in the packet must be percolation test results and profile
hole information, site plan with pertinent physical features, floor plan for the dwelling and

caretaker unit (if applicable) and a copy of the County's 1041 approval. Once the completed
application is received, this Department will analyze and review the proposed septic design.
The applicant will need to adhere to minimum horizontal setback requirements for placement of
septic system components. The leach field will need to be located a minimum of 100 feet from any
and all wells.
A condition of approval for this application is the septic system designed by Sopris
Engineering shall be installed as designed, or with agreed to modifications of the
original design. Issuance of the septic permit by the Environmental Health Department
must occur prior to a building permit being issued.
WATER OUALITY IMPACTS: Sections 2-14 and 3-705 and 3-710:
"It is the policy of the County to preserve and protect its present water resources. To this end it is the policy of the County that no land use be
initiated which would adversely affect the quantity, quality, or accessibility of the County's water resources; or which would occur at the expense.
of established water -dependent agricultural activities; or which would result in increased salinizaflon of water resources, loss of minimum stream
flows, further destruction of wildlife habitat, or major expenditures to reacquire or redistribute major water resources. It is also the policy of. the
County to maintain a natural vegetative buffer along its surface waters such that the surface and groundwaters of the area are not encroached upon
by land uses or other human activities which could cause deterioration of water quality or impair the natural treatment processes provided by
meadows and wetlands."
The Environmental Health Department will be addressing water quality impacts on down stream
water quality. This application is not expected to impact .down stream water quality if the
Ird imum horizontal distances between components of the septic system and physical features are
in accordance with the Pitkin County Sewage Regulations.
The Environmental Health Department is charged with preserving and protecting the quality of
Pitkin County's water resources. Since a large percentage of the population is dependent upon
water from wells, elimination of groundwater pollution, and protection of aquifers and their
drainage areas are of utmost importance. Roofs and asphalt areas can be nonpoint sources of water
discharge which can contaminate water supplies. This Department recommends that nonpoint
sources of discharge be retained on the property of origin which can be accomplished through
landscaping, drainage patterns, detention ponds, and dry wells for water runoff from buildings.
A condition of approval for this application is: None
AIR QUALITY: Sections 2-13 and 3-602:
"Only that development is permitted which will not contribute significantly to degradation of air quality in Pitkin County. Developments may not
constitute an indirect or direct air pollution source under Federal, State or County regulations."
This project is not expected to contribute significantly to degradation of air quality in Pitkin
County.

A condition of approval for this application is: None
FUGITIVE DUST
Activities such as road building and landscaping require a Fugitive Dust Plan. This plan would
need to include, but is not limited to; fencing, watering of haul roads and disturbed areas, daily
cleaning of adjacent paved roads to remove mud that has been carried out, speed limits, or other
measures necessary to prevent windblown dust from crossing the property line or causing a
nuisance.
A condition of approval for this application is: None, since this is a request for a single
family dwelling with a small proposed driveway.
FIREPLACE
The applicant must file a fireplace/woodstove permit with the Environmental Health Department
before the building permit will be issued.
The less densely populated parts of the county, like this site, may have two devices per building: a
maximum of one wood burning fireplace and a second device which can be either a gas log
fireplace or a certified woodstove. In addition, unlimited numbers of decorative gas appliances are
allowed. Coal may not be burned in any device. No wood burning device may be installed in
barns or agricultural buildings.
A condition of approval for this application is: None, since the applicant has submitted
the required fireplace registration permit.
ASBESTOS
Private Residential: Prior to demolition the applicant should have the building tested for asbestos,
and if any is present, should contact this Department regarding proper removal.
Condition of approval of this application is: None, since no demolition is proposed for the
existing mobile home.
CONFORMANCE WITH OTHER ENVIRONMENTAL HEALTH LAWS: Section 2-7: "It is the policy of
the County to ensure that no use or development of land is permitted which is in violation of the laws of the County, the State of Colorado, or the
United States of America."

The fee for a ISDS application is $330 for a permit that takes 6 hours or less for the department to approve. If
approval takes longer than 6 hours, a rate of $55 per hour will be charged. The maximum fee is $1000. The basic
fee of $330 is due at the time of application. The remainder, if any will be due in two stages: first, at the time of
issuance of the ISDS permit; second, before final approval of the ISDS permit.
Application for an individual sewage disposal system is hereby submitted. I hereby certify that the above information
is true and accurate and that 1 have provided true and accurate information on locations of all existing and

Dear Mr. and Mrs. Mobley:
As you requested, we have reviewed the report by Hepworth Pawlak Geotechnical, Inc., dated October 3, 1997 under Job No.
197 575, in which they determined the average percolation was 47 minutes per inch. We also reviewed an improvement survey
prepared by the Sexton Survey Co. dated October 10, 1997 under Job No. 97013, which shows the site conditions.
It is our understanding the existing trailer house is to be removed and replaced with a new home with four bedrooms. We have
designed an individual sewage disposal system (ISDS) based upon the above-mentioned information for a four bedroom house.
The site sloped from the east to west at a moderate slope of 10% to 15%. An existing sewage leach field is located
approximately 10 feet north of an existing garage. This design was based upon a new leach field and the old field to be
abandoned.
The location of the proposed leach field is shown on the attached Exhibit A. The sewage load for a four bedroom home is 800
gallons per day (gpd). A peaking factor of 1.75 for design loading equals 1400 gpd. We recommend a standard seepage bed
for the ISDS to be installed as shown on Exhibit A.
Design Criteria:

distribute effluent on both ends of the field. The dispersal gravel to be covered with a Miraft 140n geotextile fabric or approved
equal.

We recommend the surface of the field be seeded using a native seed mix available at seed stores. No landscaping or automatic
sprinkler system is allowed over the field. Livestock should not be confined in the field area.
Our design and recommendations are based upon data supplied to us by others. If subsurface or site conditions are found to be
different from those described in this report, we should be notified to evaluate the effect it may have on the proposed ISDS. If
the County Environmental Health Department requests changes or modifications to this design, we should be contacted to
evaluate the effect on the ISDS.
All construction shall be in accordance with the ISDS Regulations.
