Tbis Permit must be retairrd alLd -ade available for inspection at the above location upon
request.
This Permit shall not be trausferred witheut approval of the issuing agency.
This Permit shall expire 120 Joys after its issuance if construction has not commenced and
a valid building permit has not been obtained. Any ch-inw'e jn plans or specifications after
the permit has been issued invalidates the perm,t, urlea� approval is secured from the
Health Officer for such changes.
This Permit is temporary until final approval is gianted by the issuing agency in writing.
Final approval. cannot be granted until constructLjr, repair and/or operation have occurred
in conformity with the condi.tionF of this: permit. This Permit is valid only for the indi-
vidual sewage disposal system nd/or equipment spec.itical.l? identified herein.
Before issuing final approval o` this Permit, the lssrtng agency reserves the right to
impose conditional terms and conditions required -o T.:^_t its applicable regulations on a
continuing basis. Monitoring and testing requirements may also be imposed. The issuing
officer shall be notified not les:, than 24 hours prior to backfill.ing or closing up the
work which would prevent in3pe-t4on of cumponents in9ta l''ed in places otherwise inaccessibly:
following such backfilling or cl.u�ure.
This Permit may be revoked or sus,^_nded by the issuing agent;
regulations of said agency or of the Stacc Board of Health, as
to meet any term or Condit Lor imposed thereon during temporary

approval or upon final approval.
Each and every condition of this Permit is a material part hereof and is not severable. Anv
challenge to, or appeal of, a condition hereof e.hall constitute a rejection of the entire

This Permit is issued and accepted with the understanding that a fee in the amount of

system for compliance with the Provisions of Article 10, Title 25, C.R.S. 1973, as amended,

of subject system with a cease and desist order issued pursuant to C.R.S. 1973, Section
25-10-107(1)(k).
The issuance of this Permit dues not imply compliance with ocher state or local regulatory
or building requirements, nor shall it act to certify that tl•e subject system will operate
in compliance with applicable tt,te and local regulations adopted pursuant to Article 10
of Title 25, C.R.S. 1973, as amcncled, except for the pirposes of establishing final approval
of an installed system for. is; ua:rce of a L,cal occnpancy permit pursuant to C.R.S. 1973,
25-10-111(2).
