Septic permit approved per compliance with the engineer revised design and specifications stamped and dated
7/8/05 based on new perc rates. Any changes must be approved by this department and the design engineer prior
to them being made. Minimum horizontal distances between components of the system and physical features shall
conform to the Pitkin County ISDS regulations.
Design is for one 1000 gallon single compartment concrete septic tank followed by a 1000 gallon two compartment
concrete septic tank with a Biotube effluent filter. A flow splitter basin distributes the flow to two beds of standard
gravelless chambers, each bed is 6 rows of 8 chambers for a total of 96 chambers.
This department does not endorse any brand of products. This permit must be kept on-site during installation.
The engineer must do a final inspection of the installation and submit an as -built letter to this department. This
department must also be called for an inspection with a minimum of 48 hours advanced notice.

The fee for a ISDS application is $600 for a permit that takes 6 hours or less for the department to approve. If approval takes
longer than 6 hours, a rate of $100 per hour will be charged. The maximum fee is $1000. The basic fee of $600 is due at the
time of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit; second,
before final approval of the ISDS permit.
Application for an individual sewage disposal system is hereby submitted. I hereby certify that the above information is true and
accurate and that I have provided true and accurate information on locations of all existing and proposed wells, contour
intervals, buildings, property lines, ditches, slopes, waterlines, springs, suction or irrigation lines, drinking water cisterns, drain
tiles, irrigation ditches, lakes, water courses, streams, floodplains, dry gulches, and existing septic systems. I hereby certify
that any such features not shown on attached site map are not present. Issuance of the permit does not imply the approval of
any other permit required for construction pursuant to Pitkin County codes. No construction may be undertaken until all
approvals and permits have been obtained.
The Pitkin County Environmental Health Department, Pitkin County and employees of these agencies will be held harmless

Dear Nancy:
This letter represents construction certification of Sopris Mountain Ranch, Parcel 30
ISDS pursuant to the plans and specifications prepared by Pinnacle Design Consulting
Group, Inc.
The Parcel 30 ISDS was installed under the Pitkin County Environmental Health
Department ISDS Permit Number 05044, on file with your records.
The new ISDS system was designed to serve the recently completed residence located in
unincorporated Pitkin County. The drain fields were designed as a gravel -less chamber
beds and were separated into two separate fields as depicted on the "drawing of record"
plan included herewith. I personally inspected the installation of the gravel -less
chambers, septic tanks, distribution box, and effluent piping. I observed the installation
of the system including site preparation, septic tanks, gravel -less chambers, and manifold
piping as shown on our plan and detail sheets.
The enclosed "drawing of record" plan represents the proposed ISDS improvements with
respect to the subject residence.
I trust that this letter provides adequate certification of the Parcel 30, Sopris Mountain
Ranch ISDS installation pursuant to the plans and specifications prepared by my firm.

1. I did not take any pictures of the distribution box. However, it has an access manway on top of it so it is still accessible.
2. The installer was Metcalf Concrete. Jason Metcalf is the owner. He runs an excavation business as part of his concrete
business, which is based in New Castle, Colorado.
3. The system was installed in the fall of 2005. I believe sometime in October. The photos that I took were developed from a
disposable camera and therefore were not dated until March of 2006 when I needed other photos from the same camera for
another septic system asbuilt drawing.

Enclosed please fmd a revised OWS plan and calculations for the Parcel 30 of the Sopris
Mountain Ranch. As you know, we conducted percolation tests in an alternate area and
obtained better absorption results than the original percolation test conducted by HP
Geotech.
The alternate test locations seemed to contain more alluvial deposits and less clay than
the original percolation test holes.
Our percolation test results and revised system calculations are shown on Sheet 2 of 2
enclosed

Dear Nancy,
Enclosed are two copies of the revised OWS for the subject parcel. We have modified
the drain field in accordance with the standards from the Tri -County Health Department
for drip irrigation systems. We used an allowable application rate (R), in this case, of
0.06 gal/sflday which is based on an infiltration rate of 0.01 gal/sf/day and an evaporation
rate of 0.05 gaUsf/day (assuming 32" of evaporation per year). The calculations have
been shown on the new set of plans.

Attached, please find the water balance spreadsheets. Section VIII C. d.
(2) on Page 26 of the State Guidelines on ISDS has a Long Term Acceptance
Rate of 0.20 gallons/sq.ft./day for percolation rates in excess of 60
minutes per inch; however, I recommend 0.10 gallons/day/square foot for
percolation rates in excess of 120 minutes per inch.
Section VIII C. 4 g. and h. discuss requirements for sizing ET beds -Use the
formula or the water balance method.
I will also fax you the information on the design of drip irrigation
systems. Keep in mind that Pitkin County may require a different equation
and loading rates than indicated in the information I will fax you.

Subject: Preliminary Subsoil Study for Foundation Design and Percolation
Testing, Proposed Residence, Lot 30, Sopris Mountain Ranch, Pitkin
County, Colorado
Dear Mr. Mandich:
As requested, Hepworth-Pawlak Geotechnical, Inc. performed a subsoil study and
percolation testing for preliminary foundation and septic disposal designs at the subject
site. The study was conducted in accordance with our agreement for geotechnical
engineering services to you dated July 21, 2003. The data obtained and our
recommendations based on the proposed construction and subsurface conditions
encountered are presented in this report.
Proposed Construction: Design plans for development of the lot were conceptual at the
time of this study. We understand that the proposed residence will be a single family
residence with a full basement level located in the area of the exploratory boring and
pits shown on Figure 1. Ground floors could be slab -on -grade or structural above
crawlspace. Cut depths are assumed to range between about 4 to 10 feet. Foundation
loadings are assumed to be relatively light and typical of the proposed type of
construction. The septic disposal system is proposed to be located northeast and
downhill of the proposed residence. The irrigation ditch is proposed to be relocated
around the building footprint.
If building conditions or foundation loadings are significantly different from those
described above, we should be notified to re-evaluate the recommendations presented in
this report.
Site Conditions: The lot was vacant at the time of our field exploration and is located to
the south and uphill of the subdivision road which crosses the north end of the lot. The
building area is accessed via a rough graded dirt road. A live irrigation ditch flows
northeast through the proposed building footprint and continues along the east side of
the building envelope. The ground surface in the building envelope is irregular but
mostly flat with typically a moderate slope down to the northeast. Vegetation consists

of grass and weeds with sage and oak brush. Scattered cobbles and boulders were
observed on the ground surface.
Subsurface Conditions: The subsurface conditions at the site were evaluated by
drilling one exploratory boring, and excavating two exploratory pits in the building area
and one profile pit in the septic disposal area at the approximate locations shown on
Figure 1. The logs of the boring and pits are presented on Figure 2. The subsoils
encountered, below about 1 foot of organic topsoil, consist of a shallow depth of stiff to
very stiff sandy clay with scattered rock fragments up to cobbles size overlying dense
gravel, cobble and boulder size rock fragments in a sandy clay matrix down to the
maximum explored depth of 6 to 8 feet. Results of swell -consolidation testing
performed on relatively undisturbed samples of the upper sandy clay, presented on
Figures 4 and 5, indicate low compressibility under existing moisture conditions and
light loading and high expansion potential when wetted. Results of a gradation analysis
performed on a sample of the underlying gravel and cobble rock fragments (minus 6
inch fraction) obtained from Pit 2 are presented on Figure 6. The laboratory test results
are summarized in Table 1. No free water was observed in the boring or pits at the
time of drilling or excavation and the soils were slightly moist to moist.
Foundation Recommendations: Considering the subsoil conditions encountered in the
exploratory boring and pits, and the nature of the proposed construction, we
recommend spread footings placed on the natural coarse granular subsoils and designed
for an allowable soil bearing pressure of 3,000 psf for support of the proposed
residence. Footings may need to be designed for a minimum dead load if expansive
soils are encountered in footing areas. Due to the variable nature and settlementfheave
potential of the subsoils, bearing conditions in footing areas should be evaluated at the
time of construction. Footings should be a minimum width of 16 inches for continuous
walls and 2 feet for columns. The upper clay soils and loose disturbed soils
encountered at the foundation bearing level within the excavation should be removed to
expose the undisturbed natural soils. Exterior footings should be provided with
adequate cover above their bearing elevations for frost protection. Placement of
footings at least 42 inches below the exterior grade is typically used in this area.
Continuous foundation walls should be reinforced top and bottom to span local
anomalies such as by assuming an unsupported length of at least 10 feet. Foundation
walls acting as retaining structures should be designed to resist a lateral earth pressure
based on an equivalent fluid unit weight of at least 55 pcf for the on-site granular soil as
backfill. Rock larger than about 6 inches, topsoil and clay soils should not be included
in the backfill.

Floor Slabs: Due to the variable settlement/heave potential of the subsoils, the
feasibility of slab -on -grade construction should be evaluated at the time of construction.
Structural floors over a crawlspace are commonly used where expansive subgrade soils
are encountered. The following recommendations should be followed if slabs -on -grade
are proposed.
To reduce the effects of some differential movement, interior slabs should be separated
from all bearing walls and columns with expansion joints which allow unrestrained
vertical movement. Floor slab control joints should be used to reduce damage due to
shrinkage cracking. The requirements for joint spacing and slab reinforcement should
be established by the designer based on experience and the intended slab use. A
minimum 4 -inch layer of free -draining gravel should be placed beneath basement level
slabs to facilitate drainage. This material should consist of minus 2 -inch aggregate with
less than 50% passing the No. 4 sieve and less than 2% passing the No. 200 sieve.
All fill materials for support of floor slabs should be compacted to at least 95% of
maximum standard Proctor density at a moisture content near optimum. Required fill
can consist of the on-site granular soils devoid of vegetation, topsoil, and rock larger
than about 6 inches or road base.
Underdrain System: Although free water was not encountered during our exploration,
it has been our experience in the area that local perched groundwater can develop
during times of heavy precipitation or seasonal runoff. Frozen ground during spring
runoff can create a perched condition. We recommend below -grade construction, such
as retaining walls, crawlspace and basement areas, be protected from wetting and
hydrostatic pressure buildup by an underdrain system.
The drains should consist of drainpipe placed in the bottom of the wall backfill
surrounded above the invert level with free -draining granular material. The drain
should be placed at each level of excavation and at least 1 foot below lowest adjacent
finish grade and sloped at a minimum 1 % to a suitable gravity outlet. Free -draining
granular material used in the underdrain system should contain less than 2% passing the
No. 200 sieve, less than 50% passing the No. 4 sieve and have a maximum size of
2 inches. The drain gravel backfill should be at least 11/2 feet deep.
Surface Drainage: The following drainage precautions should be observed during
construction and maintained at all times after the residence has been completed:

avoided during construction. Drying could increase the expansion
potential of the clay soils.
2) Exterior backfill should be adjusted to near optimum moisture and
compacted to at least 95% of the maximum standard Proctor density in
pavement and slab areas and to at least 90% of the maximum standard
Proctor density in landscape areas. Free -draining wall backfill should be
capped with about 2 feet of the on-site, finer graded soils to reduce
surface water infiltration.
3) The ground surface surrounding the exterior of the building should be
sloped to drain away from the foundation in all directions. We
recommend a minimum slope of 12 inches in the fust 10 feet in unpaved
areas and a minimum slope of 3 inches in the first 10 feet in pavement
and walkway areas. A swale may be needed uphill to direct surface
runoff around the residence.
4) Roof downspouts and drains should discharge well beyond the limits of
all backfill.
5) Landscaping which requires regular heavy irrigation should be located at
least 10 feet from the building. Consideration should be given to the use
of xeriscape to limit potential wetting of soils below the building caused
by irrigation.
Percolation Testing: Percolation tests were conducted on July 25, 2003 to evaluate the
feasibility of an infiltration septic disposal system at the location designated by Mr.
Mandich. One profile pit and three percolation holes were dug at the locations shown
on Figure 1. The test holes (nominal 12 inch diameter by 12 inch deep) were hand dug
at the bottom of shallow backhoe pits and were soaked with water one day prior to
testing. The soils exposed in the percolation holes are similar to those exposed in the
Profile Pit shown on Figure 2 and consist of dense gravel and cobble rock fragments in
a sandy clay matrix. Percolation testing indicates infiltration rates of 120 to 240
minutes per inch. The percolation test results are presented in Table 2. A civil
engineer should design the infiltration septic disposal system.
Limitations: This study has been conducted in accordance with generally accepted
geotechnical engineering principles and practices in this area at this time. We make no
warranty either express or implied. The conclusions and recommendations submitted in
this report are based upon the data obtained from the exploratory boring and pits located
as shown on Figure 1, the proposed type of construction and our experience in the area.

identified at the exploratory boring and pits and variations in the subsurface conditions
may not become evident until excavation is performed. If conditions encountered
during construction appear different from those described in this report, we should be
notified at once so re-evaluation of the recommendations may be made.
This report has been prepared for the exclusive use by our client for preliminary design
purposes. We are not responsible for technical interpretations by others of our
information. As the project evolves, we should provide continued consultation and field
services during construction to review and monitor the implementation of our
recommendations, and to verify that the recommendations have been appropriately
interpreted. Significant design changes may require additional analysis or modifications
to the recommendations presented herein. We recommend on-site observation of
excavations and foundation bearing strata and testing of structural fill by a
representative of the geotechnical engineer.

1) This well shall be used in such a way as to cause no material injury to existing water rights. The issuance of this permit
does not assure the applicant that no injury will occur to another vested water right or preclude another owner of a vested
water right from seeking relief in a civil court action.
2) The construction of this well shall be in compliance with the Water Well Construction Rules 2 CCR 402-2, unless approval
of a variance has been granted by the State Board of Examiners of Water Well Construction and Pump Installation
Contractors in accordance with Rule 18.
3) Approved pursuant to CRS 37-92-602(3)(b)(II)(A) as the only well on a tract of land of 35.03 acres described as lot 30,
Sopris Mountain Ranch division of land, Pitkin County.
4) The use of ground water from this well is limited to fire protection, ordinary household purposes inside not more than
three (3) single family dwellings, the irrigation of not more than one (1) acre of home gardens and lawns, and the watering
of domestic animals.
5) The maximum pumping rate of this well shall not exceed 15 GPM.
6) The return flow from the use of this well must be through an individual waste water disposal system of the
non -evaporative type where the water is returned to the same stream system in which the well is located.
7) Pursuant to Rule 6.2.3 of the Water Well Construction Rules, the well construction contractor shall submit the as -built well
location on work reports required by Rule 17.3 within 60 days of completion of the well. The measured location must be
accurate to 200 feet of the actual location. The location information must include either the 1/4, 1/4, Section, Township,
Range, and distances from section lines, or a GPS location pursuant to the Division of Water Resources' guidelines.

these lots, however, the applicants should be aware that there are some lots in the
Sopris Mountain Ranch area.that have had to- have engineered systems installed. Site
specific soil data will have to be obtained, and the applicants will need to adhere to. -
minimum horizontal setback requirements for placement of sewage disposal system
components. A setback chart has been Included with these comments. They -will need .
to: locate the leach field sites.a minimum of 100 feetfrom their wells and from any-.
neighboring wells. 'Depending on the size of the homes and the number of -bedrooms _.
or rooms of similar size, there is the possibility that the .distances will have to be greater
than 100 feet between wells.and sewage disposal systems. For. example,.a home :of.
between 2,000 4 6,000 sq. ft. with 5.bedrooms or rooms of similar size will need to:be.at
least 156 feet from any wells, The proposed building envelopes are large so there
shouldn't.be a problem maintaining the required distances from _the.well and sewage
disposal system components. Placement of the wells should be considered prior to
drilling to insure that there will be no problems with maintaining these setbacks. 1
It is desirable to locate the system downhill from the house so that the systerp can
operate by gravity:.. The leaching area itself should-be.on an area that is as level as
possible. According to the information provided, the. building envelopes range...'

between 10 to 25% slopes. Since no soil absorption system shall be permitted in areas
where the ground slope is in excess of 30% unless designed by an engineer, this
Department may need sufficient information to document that the area for the sewage
disposal systems) is not in excess of 30%. If the sewage disposal systems) is installed in
separate trenches terraced down the slope, the system(s) should function properly.
According to the information provided in the application, the lot line adjustment has
created a new building envelope where the slopes are less.severe. From the standpoint
of this Department, that has allowed for a more suitable location for a sewage disposal
system to be installed.
The area for the leach fields should be located in an area of no traffic, planted with dry
land grass, and should not be watered. The applicants should refer to the Pitkin County
Sewage Regulations for detailed information, or call our office to discuss this site.
If the well permit states that the sewage disposal system must be of the non-
evaporative type where the water is returned to the same stream system in which the
well is located, a completely lined field, such as an evapo-transpiration bed, is not
allowed unless the applicant contacts the Colorado Division of water Resources and a
new well permit is in place or a plan of augmentation is created.
A condition of approval for this application is the receipt and approval of
the sewage disposal permits by the Environmental Health Department
before building permits can be issued. The sewage disposal systems must
be located within the building envelopes.
WATER QUALITY IMPACTS: Sections 2-140 and 3-70.5 and 3-70.10:
-It is the policy of the County to preserve and protect its present water resources, recognizing the County's semi -arid character and that significant
transmountain and transbasin diversions and the vested rights of senior appropriators in the basin have materially curtailed the availability of an
already scarce water resource. To this end it is the policy of the County that no land use be initiated which would adversely affect the quantity, quality.
or accessibility of the County's water resources; or which would occur at the expense of established water -dependent agricultural activities; or which
would result In increased saIinization of water resources, loss of minimum stream flows, further destruction of wildlife habitat, or major expenditures
to reacquire or redistribute major water resources. It is also the policy of the County to maintain a natural vegetative buffer along its surface waters
such that the surface and groundwaters of the area are not encroached upon by land uses or other human activities which could cause deterioration of
water quality or impair the natural treatment processes provided by meadows and wetlands.' .
The Environmental Health Department is charged with preserving and protecting the
quality of Pitkin County's water resources. Since a large percentage of the population is
dependent upon water from wells, elimination of groundwater pollution, and
protection of aquifers and their drainage areas are of utmost importance.
Surface water and groundwater contamination can also be caused by inadequate
setback distances from sewage disposal systems to proximate wells, rivers, creeks,
ponds, ditches and reservoirs. Minimum horizontal distances between components of a
sewage disposal system and physical features must be in accordance with the Pitkin
County Sewage Regulation. This Department reserves the right to require water quality
sampling at the homeowner's expense.

A condition of approval for this application is that all setback distances be
maintained between sewage disposal system components and wells, rivers,
creeks, ponds, ditches and reservoirs.
AIR QUALITY: sections 2-130 and 3-602:
'Only that development is permitted which will not contribute significantly to degradation of air quality.
This project is not expected to contribute significantly to degradation of air quality in
Pitkin County.
Activities such as road building and landscaping require a Fugitive Dust Plan. This plan
would need to include, but is not limited to, fencing, watering of haul roads and
disturbed areas, daily cleaning of adjacent paved roads to remove mud that has.been
carried out, speed limits, or other measures necessary to prevent windblown dust from
crossing the property line or causing a nuisance,
A condition should be approval by the Aspen/Pitkin Environmental Health
Department of the fugitive dust control plan, containing enough measures
to ensure that dust does not become a nuisance or blow onto adjacent
property.
The applicant must file a fireplace/woodstove permit with the Environmental Health
Department before the building permit will be issued.
The less densely populated parts of the county like Sopris Mountain Ranch may have
two devices per building: a maximum of one wood burning fireplace and a second
device which can be either a gas log fireplace or a certified.woodstove. In addition,
unlimited numbers of decorative gas appliances are allowed. Coal may not be burned in
any device. No wood burning device maybe installed in barns or agricultural buildings.
A condition of approval for this application is the receipt and approval of
the fireplace/woodstove permits by the Environmental Health Department
before the building permits can be issued.
CONFORMANCE WITH OTHER ENVIRONMENTAL HEALTH LAWS: section 2-70:
