Septic permit approved per compliance with the engineer design and specifications dated June 25, 2004. Any
changes must be approved by this department and the design engineer prior to them being made. Minimum
horizontal distances between components of the system and physical features shall conform to the Pitkin County
ISDS regulations.
The design is for one 1500 one -compartment concrete septic tank followed by one 1250 gallon 2 -compartment
concrete septic tank with an effluent pump in a Biotube pump vault in the second compartment. The drain field is a
drip irrigation design in 9 sections, each section 8 ft wide by 95 ft long, for a total area of 6849 SF.
This department does not endorse any brand of products. This permit must be kept on-site during installation.
The engineer must do a final inspection of the installation and submit an as -built letter to this department. This
department must also be called for an inspection with a minimum of 48 hours advanced notice.
'Revegetation over the area of the field is very important for the functioning of the system. Pitkin County has guidelines that must be
followed to assure the plants that are introduced are appropriate for the conditions of the area. The use of native plants is strongly

The fee for a ISDS application is $600 for a permit that takes 6 hours or less for the department to approve. If approval takes
longer than 6 hours, a rate of $100 per hour will be charged. The maximum fee is $1000. The basic fee of $600 is due at the
time of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit; second,
before final approval of the ISDS permit.
Application for an individual sewage disposal system is hereby submitted. I hereby certify that the above information is true and
accurate and that I have provided true and accurate information on locations of all existing and proposed wells, contour
intervals, buildings, property lines, ditches, slopes, waterlines, springs, suction or irrigation lines, drinking water cisterns, drain
tiles, irrigation ditches, lakes, water courses, streams, floodplains, dry gulches, and existing septic systems. I hereby certify
that any such features not shown on attached site map are not present. Issuance of the permit does not imply the approval of
any other permit required for construction pursuant to Pitkin County codes. No construction may be undertaken until all
approvals and permits have been obtained.

should the individual sewage disposal system fail or malfunction. The permit to construct is issued on information submitted by
the applicant or his/her representatives. The owner assumes full responsibility in case of failure of the system.

NOTE: The Department may require greater distances where warranted. For repair or upgrading of existing systems where the size of the lot prevents meeting these distances, the repaired
facility may not be closer to water supply components than the existing facility.
(1) Crossings from these distances may be permitted if the drinking water or wastewater pipe is encased for at least ten (10) feet on each side of the crossing or place where components are
closer than specked in the table. Pipe of Schedule 40 rating or better must be used, of sufficient diameter to easily slide over and completely encase the line. Ridged end caps of at least
Schedule 40 rating must be glued or secured In a watertight fashion to the ends of the encasement pipe. There must be a hole of sufficient size to accommodate the pipe in the lowermost
section of the ridged cap so that the conveyance pipe rests on the bottom of the encasement pipe. The area in which the pipe passes through the endcaps must be sealed with an
approved underground sealant compatible with the piping used.
(2) Add 8 feet additional distance for each 100 gallons per day of design flow over 1000 gallons per day as specified in the table, unless it can be demonstrated by an RPE or geologist that a
mechanical or natural barrier will prevent contamination.

This letter presents a summary of the installation observations of the Onsite Wastewater System
(OWS) at the subject site. The Installation Observations were performed from August 20 to
September 21, 2007. The installation was based on designs by CHURCH & Associates, Inc. under
Job No. 16447 dated June 25, 2004.
On August 20, 2007, Brian Edwards was called by Rick Pickard and visited the site to assist with the
installation of the infiltration bed area. There were questions with regards to the placement within
the building envelope and the method of construction. We also observed that the first tank had been
installed.
On August 31, 2007 Brian Edwards was called by Rick Pickard to observe the placement of the first
zone of the infiltrative area and assist with the layout. The second tank with the pump chamber had
not yet been installed.
On September 20, 2007 COWC was called to inspect the installation of the second tank and pump
chamber. The system alarms and pump operation were found to be in good working order. The air
release valve was in place and encased in an irrigation box as specified. The infiltrative area was
observed and deficiencies were noted and directed to contractor for remedy. Deficiencies noted
were two zones were not installed level in addition to the zone valve manifolds were not level.
Contractor agreed to remedy and call for inspection when corrected; Nancy Mackenzie of Pitkin
County Health attended.
On September 21, 2007 COWC was called to observe correction of prior deficiencies. Deficiencies
were corrected to specifications.
The Onsite Wastewater System appears to be installed in general conformance with COWC plans
and specifications. The installation observation does not imply a guarantee of warranty or

workmanship. An as -built drawing of site and details plans indicating the tank area and drainfield

As requested we have prepared an onsite wastewater system (OWS) design for the subject property. The
subsurface investigation was performed by CHURCH & Associates, Inc. on May 27, 2004.
SITE CONDITIONS
The property is in a rural residential area of Pitkin County, where OWS are required. The proposed drain
field area has a 20% to 25% slope to the north-northwest, and has a full cover native grasses and
sagebrush with scattered scrub oak and aspen trees. Water will be provided to the site by a well located
166+ feet from the proposed drain field. The proposed drain field site is not in a confined drainage area
and has moderate solar and wind exposure. The site is suitable for use of a drip irrigation or
evapotranspiration -absorption (ETA) OWS.
PROPOSED CONSTRUCTION
A four-bedroom residence with a living area greater than 6000 square feet (ft) is proposed at the
location indicated on Figure 2. The OWS was sized based on 4 bedrooms and 1040 gallons per day
(GPD) using Pitkin County Environmental Health Department loading of 130 gallons per day per
person. The sewage loading with a peaking factor of 1.75 is 1820 GPD. This loading includes
washing machines and garbage grinders.
If additional bedrooms in unfinished areas are possible, it is suggested that the OWS be constructed to
handle the additional loading. The installation of a properly engineered OWS to serve future build -out
can be cost effective.
SUBSURFACE CONDITIONS
The subsurface investigation was conducted by digging a test pit and five percolation holes, 25 to 54
inches deep, at the locations presented on Figure 1. Subsurface conditions encountered in the test pit

consisted of 4.5 feet of slightly silty, clayey sand, underlain by slightly sandy clay with occasional
colluvial cobbles and small boulders to the maximum depth explored of 8 feet. No ground water was
encountered in the test pit. Percolation tests indicate percolation rates ranging from 16 minutes per inch
(MPI) to 160 MPI. The average percolation rate is 81 WL An application rate (AR) of 0.27 gallons per
square foot per day (gal ft /day) is appropriate for this percolation rate. The application rate utilizes slow
soil absorption and evapotranspiration. Percolation data is presented in Table 1.
RECOMMENDATIONS
The OWS installation includes two septic tanks with an effluent pump and a dosed, shallow trench, drip
irrigation drain field, as indicated on Figures 2 through 5. The OWS is designed on an applicationifate of
0.27 gal/ft2/day and a sewage loading of 1040 GPD. The instalmust include one 15 gallon,
single -compartment, concrete septic tank followed by one 1250 -gallon, two-compartment, concrete
septic tank with an effluent pump in a M94Q be® pump vault in the second compartment. Pump
specifications are presented as Figure 5. The drain field installation must include 6840 ftZ of drip -irriga-
tion field in nine sections as presented on Figures 2 and 3. The drip irrigation field configuration can be
modified to fit the topography and preserve existing vegetation where possible. The trenches should be
backfilled to reestablish the original slope. Grading and/or retaining structures may be needed in drain
field areas to achieve even distribution and separation to ground surface. If this is required, the topsoil
should be removed prior to grading and replaced to promote revegetation.
The surface of the drip irrigation or ETA field should be seeded after installation of the system. A native
seed mix from the area available at seed stores should be used. These mixes do not require irrigation and
develop a growth 10 to 15 inches high. No automatic irrigation system should be installed over the field.
OPERATION AND MAINTENANCE
The owner must realize an OWS is different from public sewer service. The owner must assume
responsibility for maintenance of the system. The system is relatively maintenance free, but the owner
must have the septic tanks pumped and filters cleaned (hosed off). It is recommended the tank be
pumped and filter cleaned every two years. There are daily considerations, such as not putting plastic or
other non -biodegradable material into the system. Water use must be monitored so toilets are not al-
lowed to run when seals malfunction. To illustrate the point, a running toilet will consume in excess of
1000 gallons per day if allowed to flow. An excess 1000 GPD loading could flood and irreparably harm
the field.
We caution against installation of a water softener. The hydraulic loading from the backwash of a water
softener can be detrimental to OWS and a separate drywell should be constructed for the backwash
waste, if a water softener is installed. No landscaping or plastic can be used over the field, which will
reduce evapotranspiration from the field. Chemically treated water from a swimming pool or spas
should not be introduced into the OWS.
LIMITATIONS
A drip irrigation design requires installation by a contractor who is an experienced installer. Our
investigation, layout, design and recommendations are based on data submitted If subsurface conditions
different from those described are encountered, we should evaluate the effect of the changes on the
proposed OWS. If the health department makes modifications to this design, we should be contacted to
evaluate the impact on the performance of our OWS design.

requirements, septic tank construction, and other specifications, which are not depicted in our report, are
to conform to the ISDS regulations. The installer of the system is to have demonstrated knowledge of
