The property is located outside of Carbondale Colorado, in an area where OWS and wells are necessary. tSITE CONDITIONS A single-family residence presently exists, although will be demolished. A.4404mm single-family ' residence having less than 6000 SF and garage with no plumbing are proposed. There is an existing drain field that will be abandoned. The proposed drain field area is relatively flat, and vegetation consists of a moderate cover of grasses. The Crystal River flows to the east of the proposed residence. ' Water is provided by a well located to the south of the proposed drain field. SUBSURFACE ' The subsurface was investigated by digging one profile pit, as indicated. The materials encountered in the profile pit consisted of 1 -foot of a sandy root zone, underlain by clayey sand with sparse cobbles to ' the maximum depth explored of 8 feet. Groundwater or bedrock was not encountered. A soil sample was collected at a depth of 3 -feet. A gradation analysis was performed, and indicated a sand to loamy sand. An LTAR of X"als/SF/day will be used to design the OWS. ' DESIGN SPECIFICATIONS The OWS design is based on 4 -bedrooms and an average sewage load of 600 GPD. The installation ' must include a 1500 -gallon, two-compartment, precast concrete septic tank, with a BiotubeTM pump vault in the second compartment to dose the drain field. Tank alternatives may be considered. One gravelless chamber bed, using `Quick -4' InfiltratorTM chambers, must be installed as indicated. The ' bed must have 3 rows of 20 chambers for a total of 60 chambers, equaling 593 SF of infiltrative area. A 1.5 -inch perforated PVC lateral must be suspended within chambers. The lateral must have 3/16- 1 000008 11 - 11 1 40 ft Onsite Wastewater Systems inch holes placed on 3.5 -foot centers. The holes must face upward with the last hole in each lateral facing down for drainage. A 1.5 -inch level manifold will be used to distribute effluent. The irrigation ditch is to be placed in a culvert. The ditch must be culverted within 50 feet of drain field or a 2 iiI Ie I as m rc ' Construction must be according to the county ISDS regulations, the septic permit provided by Pitkin County Environmental Health Department, and this design. This design for the subject property, noted above, is valid only in conjunction with the approved County Health Department permit. ' OPERATION INFORMATION AND MAINTENANCE The surface of the drain field should be seeded upon completion. Vegetation is an important factor in ' drain field performance. Erosion control should be practiced prior to and during re -vegetation. Geo - fabrics or plastics should not be used over the drain field. No heavy equipment, machinery, or materials should be placed on backfilled drain field. Livestock should not graze on the drain field. ' Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWS. For Example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a drain field. ' The homeowner should pump the septic tank every two years and clean the effluent filter as needed. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed ' into the OWS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWS. No hazardous wastes should be directed into the OWS. Mechanical room drains should not discharge into the OWS. The OWS is engineered for domestic ' waste only. ' ADDITIONAL CONSTRUCTION NOTES If design includes a pump, air release valves and weep holes should be installed to allow pump lines ' to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain field due to the potential to compact soil. Extensions should be placed on all septic tank components to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be ' used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices is optimal. 1 INSTALLATION OBSERVATIONS ' ALL SERVICE septic, LLC, and the county must view the OWS during construction. The OWS observation should be performed before backfill, after placement of chambers and distribution pipes. Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must ' also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the installation. 1 000009 I ' ft Onsite Wastewater Systems LIMITS: The design is based on information submitted. If soil conditions encountered are different from conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction must be according to the county regulations. Requirements not specified in this report must follow county regulations. The installer should have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. Please call with questions. The installation included a 1500 -gallon, two-compartment concrete septic tank with the addition of a Biotube pump vault in the second compartment, followed by a gravelless chamber bed. The bed has three rows of 20 `Quick -4' chambers, for a total of 60 chambers, equaling 593 SF of infiltrative area. Effluent will be distributed with pressurized laterals. There is an irrigation ditch located to the west of the septic tank and drain field. The ditch was piped within 50 feet of the septic tank and drain field. The pipe was then backfill with onsite soils. The pump and control panel were checked on October 6th 2009. The pump, alarm, and control panel were wired and functioning. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are made to the OWTS. The observation was based on information submitted and our site visit. If conditions are different from conditions described in this letter, ALL SERVICE septic, LLC should be notified. All additional OWTS construction must be according to the county regulations. Drainage is approximately 4% to the east. A percolation test/soils evaluation was conducted on the property of reference on 05/01 /01 by Tom A. Crank, registered professional engineer (R.P.E.). A soils observation trench (excavation A) was located approximately 125' east of the west property line and approximately 15' south of the north property line. Perc excavations were located approximately 10' south of the observation trench (excavation B); 23' south of the observation trench (excavation C); and 45' south of the observation trench (excavation D). The soils observation trench (excavation A) was extended to a depth of 10' below ground surface (BGS). There was no evidence of ground water or a high seasonal water table in the open excavation to a depth of 120" BGS. The soils evaluation indicates four distinct soil horizons underlie the site. A lithological description follows: dypt (im description 0"- 14" silty clay, dark brown to gray; blocky 14" - 27- sandy clay/clayey &and, brown; medium hard 27" - 62" sand to clayey sand, tanto brown; medium dense; friable 62" - 120" gravel to cobbles in fight brown sand to clayey sand matrix Page I of 3 00005U 05/11/2001 15:51 9702577453 CPOtAF C01)ST 0 Perc holes were constructed in excavation B at depths of approximately 36 in depths of 24 is and 52 in.; and in excavation D at depths of 24 in. and 48 in. saturated at the time of the test. The following sito-specific considerations should be observed during implementation of the ISDS: • Care should be taken to maintain required setbacks from all pertinent lot features 6.6- the well and all open water courses) on the property. A design pert rate of 30 min/in. is recommended for overall system swing. • Care should be taken during construction to avoid compaction and smearing of the exposed infiltrative surfaces and preserve the natural permeability of the native soils.
