Any changes must be approved by this Department and the design engineer prior to construction. This Permit must be kept on-site during installation. The engineer'must do a final inspection of the installation and submit an "as -built" letter to this Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection with a minimum of 48 hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation. The absorption area must beat least 90 feet from Lewis Lake and any other watercourse. This permit is conditioned upon the property owner(s) providing for the regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturers recommendations and the requirements of the Pitkin County OWTS Regulation. Revegetation over the area of the field must be maintained in accordance with Section 6.28.110 of the OWTS Regulation This design will consist of one 2000 gallon single compartment tank followed by one 1000 gallon recirculation tank (with 2 Advantex AX -20 units), and one 1000 gallon dosing tank. Automatic distributing valves will distribute effluent to 2 sand filters. Each sand filter will consist of 2 feet clean, coarse sand meeting specifications outlined in the regulation. A gradation of the filter material shall be submitted to the Department prior to final approval of the system. There must be at least 6" of aggregate below the distribution laterals. Dimensions of the sand beds will be 16' by 50' with a slope of 3:1. THIS PERMIT IS EXPRESSLY CONDITIONED UPON COMPLIANCE WITH ALL REQUIREMENTS OF THE PITKIN COUNTY OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE. Correa control settings will be verified. Operations data from panel will be recorded. Processing Tank: During each visit, the scum and sludge layers in the processing tank (septic mak) will be measured. A recommendation for pumping will be made when the scum thickness exceeds four inches, or the sludge thickness exceeds 24 itches. Pumping System: During each visit all pumps shall be observed to verify properperforroaucc. The pumps will be operated manually_ The Biombe set= will be removed and cleaned. Recirculating Filter The filter will be observed for signs of excess organic build-up or improper performance. The media will be cleaned as necessary. The system will be observed to verify proper distribution across the media and distribution laterals will be flushed_ Drain FSeldr The surface area arvuad the dram field will be observed for signs of failure such as lush vegetation growth or effluent ponding. Liquid levels within the field will be observed through the observation pipe and levels recorded. Items than are not included in Lhis contract include: 1. Corrective and/or repair maintenance if necessary, other than covered under warranty. 2. Effluent sampling and analysis if applicable. 3. The cost of tank pumping. Alarms: VPI will respond to ak= conditions at no cost during the agreement period. Service Calla Service calls will be billed at a rate of 570.00 per hour and .545.00 per hour for travel time. A visit to the site other than the routine visits provided by this agreement or a system warranty issue will be deemed a service call. Examples would include (but ara not limited to) damage caused by acts of vandalism, failtae of rank or failure of another patty's work, i.e. electrician, plumber, excavator, etc. The goal of VPI is to observe the operation and perform maintenance for the Onsite Wastewater System to allow for proper long-term function of the system and compliance with the conditions of the installation w it and mcommendatiotu of the manufacwter. VPI will perform O&M services as outlined for 5265 per visit, with two visits per year ($530 per year). payment in full forthe fust ( 3 ) years is required prior to initiation of the agreement. This agreement period will be for the ( 3 )-year period subsequent to placing the system into operation. The percolation rates were taken on April 4", 2008 at the site of the OWTS on the 3W lot. gl:L Thompson has requested that a second round of percolation tests be done to re -confirm the percolation rates later in the spring, which Ranch Lake II, Inc has agreed to. These rates will be compared to the current rates and reevaluated if necessary. CTL Thompson has a high level of confidence in the current information, but deems the second round of information as a good backup to the current information. This will also be a good check to see if wintertime rates are 100% reliable and provide good information to the county on their code changes. The OWTS being designed by High Country Engineering, Inc (HCE) is a modified sand filter system based off of a house bedroom count of 5 rooms. The interior of the main residences has not been finalized at this time. The 5 bedroom count is the maximum number of bedrooms that can effectively fit within the floor space available. But the final bedroom count will also be a point of review later in the spring/summer once the final floor plan has been designed. Thunder Construction and Ranch Lake II, Inc will be re -reviewing the design and percolation rates prior to construction of the OWTS and resubmitting information to Pitkin County Environmental if any design parameters have changed. The center of the planned ISDS location was staked by others. Our field representative directed excavation of the percolation pits on April 3, 2008. The locations of the percolation pits (P-1 through P-3) profile pit (Profile) are shown on Figure 1. Graphic logs of the soil encountered in the pits are shown on Figure 2. Our field representative prepared the pits for percolation testing on April 3, 2008 and our field representative performed the tests the following day. Test results are presented on Figures 3 and 4. The percolation rate in P-1 was about 20 min/inch. However, we believe the rate from P-2 and P-3 is more indicative of the rate that will be observed for the ISDS.
