The Facilities Condition Audit (the "Agreement ") dated March 30, 2011 between the Board of County
Commissioners of Pitkin County (the "County ") and Facility Engineering Associates, PC 445 Union
Boulevard, Suite 120, Lakewood, Colorado 80228 (303) 984 -7300, ext 101 (the "Contractor "), is hereby
amended as follows:
Description of Change: EXHIBIT A, Dated 11/17/2011
Earlier this year, FEA was tasked with providing Facility Condition Assessments for multiple buildings
within the Pitkin County portfolio. As part of this work, FEA reviewed the structural, building envelope,
mechanical, electrical, plumbing, roofing, ADA, and fire /life safety systems. In addition, FEA
subcontracted to have infrared scans of the building electrical systems be done, as well as load monitors
to determine how close to capacity selected building electrical systems are.
FEA has been requested to perform a Facility Condition Assessment for the Ambulance Quarters. As part
of this scope of service, FEA proposes to review the building in accordance with the same scope of work
done earlier this year with the exception of providing electrical load readings and infrared scans of the
electrical systems at the building. Infrared scans will not be included in this study.
Reason for Change
Pitkin county has determined through and Inner Governmental Agreement that the Ambulance Quarters is a
county maintained asset. The facilities is more than 20 years old and needs to be assessed to determine a
10 year capital replacement schedule for the next budget year.
Original Contract Price $ 71,529.00
Net Increase/Decrease in Contract Price (this change order) $ 6,900.00 (base bid only)
*Total Adjusted Contract Price (including this change order) $ 78,429.00
This change order extends the time to complete work by February 28, 2012.

SUBJECT: Proposal for Facility Condition Assessment — Ambulance Quarters

Facility Engineering Associates, P.C. (FEA) is pleased to submit our proposal to provide a Facility
Condition Audit to Pitkin County Facilities Management. Our proposal is provided in response to a
request from Jodi Smith of Pitkin County for a Facility Condition Assessment for the Ambulance
Quarters.
1.0 PROJECT BACKGROUND
The Ambulance Quarters is a two floor, 3,800 SF building that was constructed in October of 1991. The
building reportedly has space for three ambulances and has quarters for personnel located above these
bays. A temporary storage and parking shelter has also been attached to the side of the building to
provide additional storage space.
2.0 SCOPE OF WORK

3.0 PROPOSED FEE
Based on the Scope of Work described in this proposal, we will complete the work for the following lump
sum fees:
Base Facility Condition Assessment $6,900
Optional Electrical Infrared Scans $1,900*
*FEA understands that Pitkin County has an ongoing relationship with Durgen Electric
and may prefer to use Durgen Electric for the infrared scanning. However, Durgen
indicated that they did not own their own infrared camera and must rent it at an
approximate rental cost of $1,150. Due to the level of rental cost, FEA obtained a bid
from another electrical contractor (ID Electric). If the other contractor is used, FEA
anticipates the cost to be approximately $800 (not -to- exceed) for the infrared scan of up
to 5 electrical panel boards.
The above fees include reimbursable costs including travel costs, equipment rental, and subcontractor
costs associated with completing the scope of work..
SCHEDULE
Based on our current workload, FEA anticipates being able to perform the site visit for the project within
two weeks of receipt of written authorization. The report for the building will be ready within two
weeks of the field site visit. We understand the draft report needs to be Pitkin County by December 20
for their review.
CONTRACT FORM
FEA proposes to perform this work as an additional service to our existing contract.
AUTHORIZATION
If the scope of work and proposed fee are acceptable, please provide a change order to our original
contract. Please note that this proposal is valid for 60 days or as mutually agreed upon by both parties.
