CONTRACTOR: ADK Executive Search
The "Agreement" dated November 15 2011 between the Board of County Commissioners of Pitkin
County (the "County") and ADK Executive Search. (the "Contractor"), is hereby amended as follows:
Description of Change: ADK Executive Search shall conduct the ' initial telephone interviews on the
County's behalf for the Assistant Director of Aviation (Administration) position. Upon completion of the
initial telephone interview process, a summary report for up to (11) eleven candidates shall be provided to
the County.
Reason for Change: It was best sought to utilize the Contractor for the conduction of the initial telephone
interview process.
Original Contract Price $ $16.500
Net Increase/Decrease in Contract Price (this change order) $ 5.000
*Total Adjusted Contract Price (including this change order) $ 21,500
in all other respects the Agreement is in full ' force and effect and remains unchanged by this
Amendment.
