AMENDMENT TO PURCHASE OF SERVICES
CONTRACT AND LEASE AGREEMENT
THE PITKIN COUNTY HEALTH AND HUMAN SERVICES PURCHASE OF SERVICES
CONTRACT AND LEASE AGREEMENT ASPEN COUNSELING CENTER OF COLORADO
WEST REGIONAL MENTAL HEALTH CENTER(the "Agreement and Lease") dated March 5,
2001 between Pitkin County (the "County") and Colorado Regional Mental Health Center, which
operates the Aspen Counseling Center of Colorado West Mental Health Center(the"Contractor"),is
hereby amended as follows:
3. Pitkin County's Obligations.
3. County agrees to pay as services are provided as follows:
1. $76,747.00 in cash towards administrative costs. The County has
paid the Contractor $28,373.50 as of March 31, 2001. The County
will pay the Contractor $28,373.50 by September 30, 2001. The
remaining $20,000.00 will be paid to Contractor by September 1,
2001.
This Amendment is executed pursuant to the direction of the Board of County
Commissioners for Pitkin County,Colorado at its May 22,2001 Work Session. In all other respects
the Agreement and Lease is in full force and effect and remains unchanged by this Amendment.
