�'� THIS A6REEMENT is entered into as of January 1, 1978 ' �

� between the Board of County Commissioners of Pitkin County and the Upper
Colorado River Community Center Board.
WHEREAS, Upper �olo�ado Rivc;- Commun�ty �enter Board is
responsible for providing services to Developmentally Delayed persons
of all ages, and
WHEREAS, each of the above parties desire jointly to have
these services available for qualifying citizens of Pitkin County, arid
WHEREAS, it is appropriate for tlie Board of County Commis-
sioners of Pitkin County to allocate local funding for local match of
State and Federal monies for the purposes of purchasing services from
the Community Center Board.
NOW, THEREFORE, BE IT RESOLVED by the Board of County
Commissioners of the County of Pitkin, Colorado:
1. This contract shall be effective for the calendar year
of 1978 and any extension or renewal thereof shall be
re-negotiated at budget time, that is prior to September
30, 1979.
2. UCRCCB will provide educational services to all eligible �
handicapped and delayed individuals residing in Pitkin
County (who are not serviceable by the appropriate school �
district) for a maximum of $1,800 in local match from }�

Pitkin County during CY 1978. j

3. The unit of service for billing purposes shall be defined �
as the enrolled day as defined by the Department of �
�
Institutions. �
4. The fee to Pitkin County for this unit of service shall �
be $1.54/client/day. �
5. UCRCCB snall bill Pitkin County on a monthly basis,
identifying clients, number of enrolled days per client, `

and programs attended. �

6. Pitkin County agrees to pay UCRCCB following review and # ,

acceptance of billing by the Human Resources Coordinator 1

7. It is likewise agreed that services for alI eligible �
clients will continue throught the CY 1978 but payment
will not eacceed $1,800.
- 8. This unit cost will be re-negotiated be£ore contract is
established for CY 1979.
9. This agreement is contingent upon continuing State and ' °
Federal funding after July 1, 1978.
10. This contract may be cancelled and terminated by either
party, but that the party desiring to terminate or cancel
' must give written notice thirty (30) days prior to the
date of cancellation.

. 11. By subscribing to this Agreement, the parties represent
to each other that they have heretofore acquired the
� necessary appsoval and authorization to enter into this
l Agreement.
IN WITNESS WHEREpE, the parties hereto have executed this
