TH2S AGREEMENT is entered into as of the first day of Januazy, 1977 t
by and between the Board of County Commissioners of Pitkin County and i
Basalt Rescue, Inc.

WF�REAS, Basalt Rescue, �c. provides ambulance service to outlyi.ng �.
Pitkin County areas; and �

WHEREAS, it is deemed desirable to have these services available for �
citizens of Pitkin County; and �

WHEREAS, each party is authorized by statute or otherwise to contfact ;,�

with one another. �;

1. This contract shall be effective starting January 1, 1977 and �'f

for calender year I977 and any extension or senewal thereof shall be re- °

negotiated at budget time, that is priar to September 30, 1977, �
2. Emergency ambulance service will be provided to the outlying �
areas of Pitkin County by Basalt Rescue, Inc. �

3. Basalt Rescue, Inc, agrees to maintain a minimum of one {1) �'

f fully equipped and staffed ambulance which at all times shall be on call �'
� �
? twenty-four (24) hours a day, seven (7) days per week. In addition, �
Basalt Rescue, inc. will provide one (1) driver and one (1) emergency �
R medical technician at all times, Basalt Rescue, inc. will cause the �
; �,

ambulance to respond promptly to all calls for assistance whether fxom '�
private or governmental sources. �

4. Pitkin County agrees to pay Basalt Resaue, Inc. sixty dollars

toward the wst of each call made in Pitkin Countp, the total for calender.: ;

year-1977 not to exceed $1500,00 (fifteen hundred dollazs), t

5. The billing and payment procedure shall be as follows: �

� ' A. Basalt Rescue, Inc. will submit, prior to the tenth of each �
month, an accounting of services delivered in Pitkin County �
during the previous month. This will be submitted to the 3

H. The accounting of services delivered wi11 include: �

7. Hasalt Rescue, Inc, sha11 permit Pitkin County to review or '��,

conduct an audit of Basalt Rescue, Inc, accounting systems, books and '

records, which are xelevant to this Aqreement, �

� 8. This contract may he cancelled and terminated by e3ther party, # ,

�i but the party desiring to terminate or cancei must give written notice to �
� the other party at least thirty (30) days prior *o the date of cancellation. �
