COLORADO WEST REGIONAL MENTAL HEALTH CENTER 1996 �
THIS CONTRACT,made as of t1�e date last below sig�ed betweeu Pitkni County herein referred to as
� "Couuty", and Colorado West Regional Meutal Health Center wirich operates Colorado West
Recovery Center hereafter refeaed to as"Coutraaor". Provisions of this contract are pursuaut to the
audiority granted by CRS 1973, 25-1-310 and is licensed and monitored by State of Colorado,
Department of Healdi,Alcohoi and Drug Abuse Division.

L County agrees to purchase and Contractor agrees to furnish the following services
hereinafter refened to as"Purchased Se�vices":
iA. Detoxificarion seroices for 150 cli�ts annual average iu Piticm Counry at
I�olding facitity nn Aspen inctuding UA testing and avtabuse monitoriog and 24
•' hour staffing in Glenwood Springs.
B. Halfway House admissions for 11 ctients average annually.

� A. Clients curreutly mtoxicated or in withdrawal.
� B. Clieuts released from intensive treatment in need of transitional living serviees.

' Se�tion II.Contractor sliall:
1. Strictty adhere to all applicabie federal,state,and local]aws or regularions(iaclud'wg
� County Procuremeut Code)tliat have beeu or may hereafter be established.
2. Ho]d and maintain for the term hereof the necessary license(s)as specified uuder State
law wUicli permits the performance ofthe secvica(s)to be purcl�ased I�erein.
_ 3. Shall waintaiu a sliding-scale fee schedule for its se�vices in order to accommodate low
income iudividuais.
4. Subcontracts utilized by the Contractor shall be subject to the reqairements of the
Contract as listed here,and the Covtractor is responsible for the perFormance of any
subcontractor.
S. Abide by all applicable provisious of d►e Title VI and VII of the Federal Civil Riglus
_ Acf of 1964,Section 504 ofihe Rehabilitation Act of 1973,and Title XX ofthe Social
Security Act of 1975 as revised.
6. Mainta'ni writteu requirements for safeguarding client information and comply with said
requirements.
i 7. Indemnify County and die State of Colorado against any liability and loss against all
� claims a��d acpons based upon or acising out of damage or injury,including dead�,to
persons or property caused or sustained'm connection witli the performance of this
, Contract or by conditions created thereby,or based upon any violarions of any statute,
ordina�ice, or regularion and tUe defense of any such claims or actions; u�aintain
insurauce iu such types and amouuts as are vecessary to find tivs indemuity;provide
proof of such u►surance to die county upon demaud tLerefore.
8. Mamtain secvice grogram records, fisca(records, documeutation and other records
relevant to this Contract for a period of five years. The above st�all be subject at all
reasonabte times to iuspections,review or audit by Federal,State or County personnel,
a��d other persons authorized'm writing by tUe Couuty.

4. Either party sGall have tlie rigi�t to termivate d�is Contract by giving the other parry
ninety days norice by registered mail,rehun receipt requested at tUe addresses below
- fisted,effective upon maiGug. Ifnotice is so given,this Contract shall termivate on d�e
expirarion of the tivrty days, and the liability of the parties hereunder for fwtUer
perfom�ance of tlie temu of tlus Contract shall thereupon cease,but perform tUeir •;
obligatious up to the date of termivarion. The Count}�s contact person for this contract
sUa]1 be the Hutnan Services Director. The Coutractor's contact person for this {
contract shall be the Colorado West Recovery Ceuter Director.
5. In the event tUis Contract is temvnated, Suiai payment to the Coutractor may be
' widilield at the discretiou of d�e County until final audit. Incorrect payments to the
Contractor due to omission,error, fraad,or defalcation sUal1 be recovered from the
, Contractor by deduction &om subsequent payme�its under this Contract or other y
cantracts between tUe Couuty aud the Contractor,or by the County as a debt due to
d�e County. 17ie waiver of any violation siiall not be construed as a waiver of any n .
other or subsequent violation of tliis Contract or appropriate statutes or regulations. �
