THIS CONTRACT,is made as of tlie date last below signed beriveen Rtkin Cotmty herein
referred to as"Comiry",aud RA.D.Educatioual Programs hereafter referred to as"Contractor".
Whereas,RA.D.Educational Programs,a non-profit agency,is promoting HN/AIDS prevention
; among yout(�by providing the first comprehensive,6fth du�ougli twelftL grade,school-based HIV
education program in the Roariug Fork Valley:
Now tl�erefore,it is liereby agreed that iu consideration of the mutual undertakings and .
, agreemenYS l�ereinafter set forth,tl�e Couuty and Contractor agree as follows: .
Subject to its other provisions,the term of the Contract shall be&om I st day of January,

� Sectiou I. County Shall:
1. County agrees to purchase and Contractor agrees to furnish the following services
' hereivafter refened to as"Purchased Seivices":
' Promotion ofHN preventiou among youth through affective HIV education about
I HIV tra�ismission,risk reduction,and decision making skills.

� Facilitation o£cowmuuity/parendand family discussions about HIV/AIDS to
reiuforce the message of HN prevention. �
2. County agrees to purchase these services for the following targeted groups:
All children attending Sftit dtrougU twelfth grades in the Roaring Fork VaIIey and
tkeir famities.

j3. Couuty agrees to pay as services are provided as follows:

! $5,570 cash towards admitustrative costs.
The total amount oftitis coutract shalt not exceed$5,570 cas6. TI►e County will pay
� contractor in quarterly paymauts on tLe last day ofMarch,June,September,and .

Rights Act of 1964,Sectiou 504 of t6e Reliabilitation Act of 1973,and Titie XX �
oftUe Social Security Act of 1975 as revised.

Section III. General Provisions: � �

terminate on the expiration of the thirty days,and the liability of tlie parties

5. Iu the event this Coutract is ternunated,fmal payment to tUe Contractor may be �:
withlteld at tUe discretion ofthe Counry m�til Suai audit. Incorrect paymeuts to the
Contractor due to omission,error,fraud,or defalcation shall be recovered from
the Contractor by deduction from subsequeut paymeuts wider dus Contract or
otUer covtracts betweett the County and tUe Contractor,or by the County as a debt
due to t2�e County. The waiver ofauy vio]ation sl�all Ziot be construed as a waiver l
of any other or subsequent violation of this Contract or appropriate statutes or
regulatious. o . '
