requests by submitting documented proof of proper �
� expenditure of State funds thus far received to a
contract monitor designated by the State. �
b. The Contractor shall request the final payment which
' is the amount withheld by the State until the project
is complete, for the Project by suhmitting to the

� that all funds received by the Contractor have been [

E-� 8. Personnel. The Contractor represents that he has, or will .
� secure at his own expense, unless otherwise stated in Exhibit A,
all personnel, as employees of the Contractor, necessary to per£orm
the work and services required to be performed by the Contractor under
this Contract. Such personnel may not be employees of or have any_
� contractual relationship with the State. All of the services re- ��
e� quired hereunder will be performed by the Contractor or under his �
� supervision, and all personnel engaged in the work shall be fully
�� qualified and shall be authorized under State and local law to

perform such services.
�� 9. Termination of Contract for Cause. If, through any cause,
i� the Contractor shall fail to fulfill in a timely and proper maivier
' his obligations under this Contract, flr if the Contractor shall

11. Chan es. The State may, £rom time to time, requiYe changes
in the scope o services of the Contractor to be performed hereunder.
�;. However, this Contract is intended as the complete integration of — +
�: all understandings between the parties, at this time, and no prior �
c . or contemporaneous addition, deletion, or other unendment hereto %
shall have any force or effect, whatsoever, unless embodied in a �
written contract amendment incorporating such changes, including s �

the Contractor, executed and approved pursuant to the State's Fiscal �

: 12. Reports. At Ieast two copies of all reports prepared as a . i

and Mineral Iiapact in the Departsnent of Local Affairs within two

13. Indemnification. Contractor, in consideration for State's (

harmless and defend State, and all of its employees and agents,
acting officially or otherwise, from any and all liability, eZaims, • '�

property of and injuries to, or death of all persons whatsoever, �
� which may occur, or is sustained in connection with the performance �

� 1. Scope of Services

� The objective of this Project is to provide an adequate co�nunication system via
a public safety radio system for emergency law enforcement, fire and rescue use �
�? in the Crystal River Valley area. The Project consists of the following elements:
,fy a) the purchase and installation of two (2) 110-watt mobile relays at the Elephant �
,�, Mountain radio site, b) the purchase and installation of two (2) fixed base sta- ,
���. tions located at the Redstone Inn, Redstone, Colorado, and at the Town of Carbondale i
police station to access the co�nunication system, c) the purchase and installation i
of a tone module and control for the Pitkin County Sheriff's Office mobile radio � -
�" '9 units, d) the purchase and installation of three (3) mobile units (8-channel with
auto-scan) in ambulances serving the area covered, e) the purchase of fifteen (15) i
pager and charger units, f) the purchase of seven (7) hand-held radio units (8- � .
channel), and g) the purchase and installation of four (4) mobile scanning and I
� radio units (S-channel) in the fire and rescue vehicles serving the area covered. .
Impact Assistance Funds will be used to finance all coimnunication equipment pur- �
!-� chases, assembly work and installation. Pitkin County, the Contractor, has com-
� pleted all required design and engineering work. The Contractor will accomplish �
, s all necessary site development work, construct access roads and incur all appli- �
cable administration costs. Although related to the Project, design and engineer-
ing work, site development, access roads construction and administration work are .
� not a part of this Project. �

�;; meets all applicable standards, regulations and specifications. All purchases of ?
�F� communication equiproent, assembly and installation will be performed by the �

' nication equipment for qualified suppliers and the Colorado Department of Admini- �
�'•,�} stration, Division of CoTmnunications. Upon execution of this Contract, the

y:; Contra.�tor will purchase each equipment item from the supplier, including the !
# �" Division of Communications, submitting the lowest responsible bid. The Department �
}�;; of Local Affairs, Division of Impact Assistance, will be sent the name of the
�: supplier who the Contractor selects as the lowest responsible bidder for each '
equipment item and the purchase price of the equipment item and installation costs
if applicable. The Contractor will comply with all licensing requirements of the
Federal Coimnunications Commission in the assembly and installation of all co�u-
nication equipment purchased.

5. Contract Monitoring
Contract monitoring will be provided by the Colorado Department of Local AfFairs
Division of Impact Assistance and the Contractor on an as—needed basis. Upon •
completion of the Project, the Contractor shall notify the Division of Impact
Assistance that the Project was completed in accordance with all applicable
standards, regulations and snECifications.. Said notification shall be fotwarded '
to the Division of Impact Assistance at the time a request for final payment is s
made.
6. Reporting Schedule •
Financial and narrative status reports will be submitted in accordance with the
payment schedule at the time payments are requested Two (2) copies of all • ,
final financial and narrative status reports shall be submitted to the Depart—
ment of Local Affairs Division of Impsct Assistance at the time a request for i�
