pregnancy screenings in an effort ta prevent unwanted pregnancies which fur[her �
poverry in Pitkin County. Communiry Health Services targets low-income individuals '��
and families for these services by community service announcemenu,Spanish and •.
• � English information cards on the Counry buses, schoo!educational seminars, ;
. ' brochures translated in English and Spanish that aze disuibuted in tfie churches and to �`
the community. �}

� � 3. Services offered by Communiry Health Services, Inc. aze available for referral from
the community and ue often linked by referrals from other Human Service agencies
in Pitkin County who identify service gaps for[he low-income population.
Community Healih Services, Inc. meets and works closely with other health care and �.
�� Human Service providers in the community so that[he medical communiry and other
' Human Services agencies have easy access to CHS, Inc. for referrals of the low-
• �� income population.

;� Foltow-up referrals for health, natrition, clothing and financial needs are made by
