Preventive Health Services. Provide health consultation, � education and services to promote optimal health of persons in �, all age groups in the community. a) Parent and Child Health. Provide consultation, 1 educational and health services {nursing, nutritional, medical and social) for children, their parents and the coaQnunity to promote the optimal growth and development of children. i) Newborn, Infant and Preschool Child Health Program. ';� Provide a system of health services designed for ' the prevention, early detection, treatment and follow-up of childhood diseases, including = I metabolic disorders, handicapping conditions and . birth defects; counsel and teach basic nutrition, 1 fluoride supplementation; provide appropriate � . � follow-up for children with� health problems. . �� � Programs included are Child Protection Team, SIDS ,- - (Sudden Infant Death Syndrome), EPSDT (Early Periodic Screening, Diagnosis and Treatment), ' . : Handicapped Childrens Program, WIC (Women, Infants . _ and Children) adolescent health projects, preschool and private school screenings. , :( , b. Strictly adhere to all applicable federal., state, and local laws or regulations (including County Procurement Code) that � � have been or may hereafter be eatablished. � ��`} - 2. Hold and maintain for the term hereof the necessary licenses � and certifications as specified under State law which permit � the performance of the services to be purchased herein. � 3. Maintain a sliding-scale fee schedule for its services in ! order to aceomodate low income individuals. ' 4. Utilize subcontractors who shall be subject to the S. %f" requirements of the Contraat as listed here, and be . responsible for the performance of any subcontractor. 5. Abide by all applicable provisions of the Title VI and VII of �-� � the Federal Civil Rights Act of 1964, Section 504 of the �� .� Rehabilitation Act of 1973, and Title XX of the Social Security Act o£ 1975 as revised. �+ �• 6. Maintain written requirements for safeguarding client information and comply with said requirements. ; � . :� , 7. Indemnify County and the State of Colorado against any -" - liability and loss against all claims and actions based upon s or arisin out of dama e or in'u ' 9 g 7 ry, including death, to persons or property caused or sustained in connection with the �' performance of this eontract or by conditions created thereby, `j , _ or based upon any violations of any statute, ordinance, or regulation and the defense of any such claims or actions; � maintain insurance in such types and amounts as are necessary „• ;;', . to fund this indemnity; provide proof of such insurance to the � county upon demand therefor.
