The Pitkin County Department of Social Services (PCDSS) , hereby t' :
enters into an agreement with the Eagle County Department of � =' '
Social Services (ECDSS) for that agency to isaue food stamps to ?-:
! eligible recipients of Pitkin County for the 1994 calendar year. �`
. � 1
i Servicea provided by Eagle Couaty Department of Social Services: i
I � :..;
1. Mail issuance of food stamps to eligible households
residing in Pitkin County. The main issuance will be on the
first working day of each month. Supplemental issuance will be �
throughout the month on the date that authorization is received. �
� 2. Month end, quarterly and annual reports of food stamp �
issuance, returns and destruction will be completed by ECDSS and
forwarded to the State Department of Social Services.
3. Monthly computer reports regarding Pitkin County food
- stamp households will be printed by ECDSS and furnishe� to PCDSS.
� 4. When food stamps are retumed by mail to ECDSS, a .
representative from ECDSS will contact the PCDSS technician for
specific instruction about reissuance or return of food stamps to �
inventory. Federal regulation provides that reissuance or return
; to inventory must be made within 48 hours of return.

.�w:� Respoasib3lity of Pitkin County Department of Social Services:
� 1 1. The Pitkin County Department of Social Services will
�- receive all applications for food stamps from Pitkin County -
residents and determine eligibility for food stamps. Computer
_. entry of food stamp authorization will be made by PCDSS.
, 2. When food stamps are returned to PCDSS and reissued
and/or destroyed by the PCDSS, that Departmerit will complete form
'-- OMB 0584-0052 and forward that form to ECDSS. �
, - In consideration of services provided by ECDSS to PCDSS under �
this agreement, the Pitkin County Department of Social Services ��,
, agrees to pay the Eagle County Department the sum of $1500 in „s
quarterly payments of $ 375, beginning March 31, 1994.
This agreement shall become effective January 1, 1994 and shall ��
� continue in effect for one year and is renewable upon mutual
� agreement for additional 12 month periods. ,��f

This agreement may be terminated for any reason by either part 4 -
I upon the giving of 60 days prior written notice o£ such 1�,
� termination to the other party. Notices shall be deemed to have
been.given upon the •mailing of said notice d1s b'y United States
imail, certified, first-class, postage prepaid, and addressed to ''1' '
� the respective part at the respective address as appears herein. ;:,'
