Whereas, Gazfield County is providing '+; '� services to its Veterans through the Garfield County Veterans Service - office and whereas, Pitkin County wishes to contract for provision of Veterans services for its residents; ,,;-. Provides counseling services to veterans and to address _ a wide variety of issues, problems, and concerns. 7 Cooperates with other County human service agencies to 's arrange for and coordinate appropriate services to veterans. - { B. Prepares and submits applications for adjusted compensation, insurance, pensions, loans, disability `� � compensation, hospitalization, vocational training, and ` other benefits. , � D. County agrees to pay as services are provided as follows: ; ;:.�'3• � :. County agrees to pay $3,285 to furnish serviaes outlined �� '`; in Section I. A-F. These services will be provided from +x::��.` the Contractors office in Glenwood Springs. However, space will be available on an "as needed basis" in the : Schultz Health and Human Services Building in Pitkin �i�° - . County at no cost to the contractor. � `` The totaZ amount of this Contract shall not exceed $3,285 cash. � The rates paid above include all of contractor overhead and ' �',: administrative costs in complying with terms of this contract. `�� 4. The County wi11 pay contractor in quarterly payments on the last day of March, June, September, and December. The • contractor will provide County with a first quarter report identifying the criteria that will be used to establish ,;;� performance standards. These performance criteria will be evaluated against actual service numbers in a written report by the aontractor to be submitted to the County before the -., second and fourth quarter payments. Strictly adhere to all applicable federal, state, and local laws or regulations (including Article V11 of the Pitkin County Procurement Code) that have been or may hereafter be " established. 2. Subcontracts utilized by the Contractor shall be subject to �� i the requirements of the Contract as listed here, and the _.. Contractor is responsible for the performance of any ' subcontractor. ( 3. in relation to his performance under this contract abide by 1 all applicable provisions of the Title VI and VII of the `'� Federal Civil Rights Act of 1964, Section 504 of the � i Rehabilitation Act of 1973, and Title XX of the Social � Security Act of 1975 as revised. 4. Maintain procedures for safeguarding client in£ormation and comply with the same. �� 5. Maintain service program records, fiscal records, documentation and other records relevant to this Contract for a period of five years. The above shall be subject at all sl;, 7�=� reasonable times to inspections, review or audit by Federal, ,.{, �,-:.' State or County personnel, and other persons authorized in f� 1 - ' writing by the County. �:, � i 6. In addition, contractor understands that no County official or employee, other than the Board of County Commissioners acting as a body at a Board meeting, has authority to enter into a contract or to modify ' the terms of this contract on behalf of the County. Either party shall have the right to terminate this Contract by giving the other party ninety days notice by registered mail, return receipt requested at the addresses below listed, effective upon mailing. If notice is so given, this Contract '�' shall terminate on the expiration of the thirty days, and the Ziability of the parties hereunder for further performance of k the terms of this Contract sha11 thereupon cease, but --; perform their obligations up to the date of termination.
