1. The Lontractor shall provide, through the services of two � ��
� public healtfi sanitarian{s), community sanitation services, which shall include �
� at least the sanitary regulation of food service establishments, retail markets
'� (inctuding product sampli'ng), schools, cfii'id care centers, summer camps, mobile
home parks, campgrounds, motels and hotels; responding to environmen[al health
emergencies; and� fulfilling reasona6le requests 6y the State for assistance in •
program activiti'es not specifically set forth herein.
2. Minimal contrac[ fulfillment for a year will include the following
acti'vities:. 2 inspections per food serJi'ce establishment; 1 inspection per
retai:l market; 1 sample each of fiam6urger or ground beef and sausage produced
hy each retail market;. l inspection per scfiool; 1 inspection per child care :�
center; 1 ihspecti'on per summer camp; mo6ile home parks, campgrounds, motel and -
hotel i'nspections upon complaint; and plan review as submitted. "

by the Consumer Protec[ion Division in the Requirements for County Sanitarians �' ��� �
which are a[[ached and by this reference made part hereof as Attachment N1.
If [here if any reason to question whether or not an applicant may meet these
minimum qualifications (for education or experience), it is necessary to sub-
mit Che application to the State for review and approval prior to employment.
5• The State will, in consideration of said services and repor[ing
by the Contractor, cause to be paid [o the Contractor a sum not to exceed

� State; and conditioned upon affirmation by the State Consumer �

submitted according to the terms of this contract.

� 6. The S[ate will continue payment for sanitation personnel only so �
long as the Contractor renders acceptable services, as determined by the State.

Athe acceptability of the quality and quantity of services rendered. The perform- ' t'
ance evaluation will consist of information su6mitted on daily reports, inspection

� 8. Payment pursuant to this contract will be made as earned, in whole
or in part, from availa6le State funds encum6ered in an amount not to exceed �
;� One Hundred Eigfity Eight Thousand and Fourteen Dollars for the purchase of County•
Sanitation Serv.Pces-. It is furtFer understood and agreed that the maximum �
; amount of State funds availa6le for fiscai year 1989 for the pu�chase of Sanitarian
Services is in the amount of $188,014.00. Ttie lia6ility of the State, at any time, �.,
� for sucfi payments shall be limited to the unencum6ered amount rema'ining of such funds. '
9• Changes in total reim6ur5ement amounts for the above named services �

2. AC all times during the perFormance of this contract, the Contractor shall strictly
adhere to all applicable federal aad state laas that have been or may hereafter be
escablished.
3. Contractor authorizes Health to perform audits and to make inspections for the �;�:z:;.:.;

� agreement shall thereupon cease, but the parties shall not be relieved of the duty to

5. This agreement is in[ended as the complete integration of all understandings
� 6etween the parties. No pzior or contemporaneous addition, deletion, or other amendment � � :
i hereto shall have any force or e#fect whatsoever, unless embodied herein in writing_ No �
� subsequent novation, renewal, addition, deletion, or other amendment hereto shall have �'• �

� 6. If this concract involves the erpenditure o£ federal funds, this contract is
t � contingent upon continued availability of federal Funds Eor payment pursuant to the
? terms of this agreement. Contractor a2so agrees to fulfill the requirenents of Office
' !� of Management and Hudget Circulars A-87 and A-102 or A-110, whichever is applzcable.
� 7_ To be considered for payment, billings £or reimbursement pursuant to this contract
, must be received Wichin 60 days after the period for which reimbursement is being ,
� requested and final billings on the contract must be received by the State Heal[h
Department within 60 days after the end of the contract term.

� 9_ If Contractor receives $25,000.00 or more per year in Federal funds in the
aggregate from Health, Lontractor agrees Co have an annual audit, by an independenr �
j certified puhlic accountant, �ahich meets the requirements of Office of Management and
' Budget Circular A-128. If Contractor is required to submit an annual indirect cost
proposal to Healeh Por reviev and approval, Contractor's audi[or will audit the proposal
in accordance vith the requirements o£ OMB Circular A-87. Concractor agrees co furnish
one copy of the audit reports to the Health Department Accounting Office wi[hin 30 days
of cheir issuance, but not later than nine months after the end of Concractor's fiscal
year. Contractor agrees to take appropriate corrective action r+iehin six monehs of the
;� report's issuance in instances of noncompliance with Federal laws and regulations.
Contractor agrees to permit Health or its agen[s to have access ro its records and .;
£inancial statements as necessary, and further agrees to retain such records and � ,1
financial statements for a period of three years after the date of issuance oE the audit
