Naw therefoi-e, it is hereby agreed that in consideration of the r
mutual undertakings and agreements hereinafter set forth, the ��

Subject to its other provisions, the term of ihe Contract � i•�
shall be froia Jul� 1, 1888 through December 31, 198$. P -,
Section I. County Shall:
1. County agrees to purchase and Contractor agrees to
,� furnish the following services hereinafter referred to
� as "Purchased Services". �
:� A. Provides counselin� services to .veterans and to �
� address a wide variety of issues, problems, and +`
concerns. Cooperates with other County human �:
d service agencies to arrange for and coordinate '
� appropriatie services to veterans.
B. Assists veterans and others with questions
regarding insurance, pensions, loans, hospitaliza-
tion, disability and other benefits.
C. Prepares and suUmits applications for adjusted
compensation, insurance, pensions, loans,
disability compensation, hospitalization, -
vocational training, and other benefits_
D. Prepares and submits correspondence, statistics :�
Iand reports to state, federal and local agencies � ,
� and of£icials. ''<
� . E. Maintains files and records. " �
_ '• F. Other functions as needed relating to the office �'
t of Veteran's Service Officer.
G. Maintain office in Pitkin County Community Center .� ;
8 hours per week including travel time. l ;

following targeted groups:

County agrees to pay $13.86 per hour for 8 hours ,
of service per week. The hourly rate indicated is
described in Attachment A. Office space will be
provided by the County at no cost to contractor.
The total amount of this Contract shall not exceed �2,883. `�

4. County will pay contractor in quarterly payments in _
February, May, Aug, and Nov of each year during term of this �

Section II. Contractor shall: ��

. 1. Strictly adhere to all applicable federal, state, and �
local laws or regulations (including County Procurement t,
Code) that have been or maY hereafter be established. ��
2. Subcontracts utilized by the Contractor shall be �
subject to the requirements of the Contract as listed
� here, and the Contractor is responsible for the -
i performance of any subcontractor.
3. Abide by all applicable provisions of the Title VI and
� VII of the Federal Civil Rights Act of 1964, Section
504 of the Rehabilitation Act of 1973, and Title XX of
the Social Security Act of 1975 as revised. 1
4. Maintain procedures for safeguarding client information
and comply with the same. �.L_�

� documentation and other records relevant to this �

; be subject at all reasonable times to inspections,
review or audit by Federal, State or County personnel, �3 '�• '

7. Provide an annual evaluation of services.
Section III. General Provisions: � .
1. The Parties to this Contract intend that the relation- �-•.;;
ship between them contemplated by the Contract is that

servant of Contractor shall be deemed to be an employ- I�.�:.�'` '
ee of County. Contractor will be responsible for its ���� ;=

acts and the act of its' a�ents, employees, servants
and sub-contractors during the performance of the
Cuntract. "
2. Payment pursuant to this Contract, iS in County, State,
or Federal funds, wi�ether in whole or in part, is
� subject to and contin�ent upon the continuing availabi- d:-'_...
�� lity and appropriation of County funds for the purpose S
; , thereof. Nutice shall be given in accordar,ce with 1'�
number 4 below. � ,. ,
3. This contract constitutes the full and complete ; 1'
� agreement of the parties and supercedes or incorporates
any prior written and oral agreemeni:s of the parties.
In addition, contractor understands that no County
official or employee, other than the Board of County
_ Commissioners acting as a body at a Board meeting, hes
authority to enter into a contract or to modify the
terms of this contract on behalf of the County. Any
_ such contract or modification to this contract must be
in writing and be ea:ecuted bq the parties hereto. ��
4. Either party shall have the right to terminate this ;,�
Contract by giving the other party thirty days notice `
by registered mail, return receipt requested at the ��
addresses below iisted, effective upon mailing. If
notice is so given, this Contract shall terminate on
the expiration of the thirty days, and the liability of
the parties hereunder for further performance of the �
terms of this Contract shall thereupon cease, but
perform their obligations up to the date of termina-
ition_ The County's contact person for this contract
� , shall be the Human Services Director. The Contractor's
contact person for this contract shall be the Garfield
County Social Services Director. �,..
5. Tn the event this Contract is terminated, final payment `#
to the Contractor may be withheld at the discretion of
_ the County until final audit which, audit, if required •' i
by the County, shall be completed within six months of
termination. Incorrect payments to the Contractor due ,f.'
� to omission, error, fraud, or de£alcation shall he r,i
. recovered from the Contractor by deduction from ;� ?
