T'he Law Walks Replacement Project 90,2013 (the "AgreemeaY') dated August 14, 2013 between the
Board �f County Commissioners of Pitkin Couaty (the "County'� and All Velley Maintenance and
Mechanical Iac. (the "Contractor");is herehy amend�ed as follows:
Descrioriori of Chan�e�. This change order was submitted after the work was complete�; verbal
authorization by Jack Wheeler as described below with exceprion of items # 5.
Courthouse:
1. Extra piping tow0.rds the garage 200 fr and the stairs for snow melt aad run the conduit from oae
side of tbe building to the other side for future; not in quote or scope of work.
2. Flush the old snowmelt out and instell it with new glycol and purging and put in a bigger pump.
Raboard it from one aide of the building to the other side of the buildin,g; not in quote or scope
of work
3. Fix all the landscaping, the front side and clean up all the debris from the Fire Sprinkler Project
as direcked by Jeck Wheeler,which included new seed, fertilizer and new dirt from Lowes; not �n
quote or scope of work.
4. Exh�a demo fot concrete for the stairs,towards the gazage and the pan w0s 6" concreta it took

Humen Services:
6. Human services, the front entry failed beeause it was only poured 2" we correeted it by pauring
4" and extra pad for the disabled/ADA Access, includes demo end disposal; unforeseen situation.
