The Contract for Provision of Services (the "Agreement") dated between the Board of County
Commissioners of Pitkin County (the "County") and Dr. Kimberly S. Levin (the "Contractor"), is
hereby amended as follows:
Description of Change:
1. TERM. The term of this amendment begins January 1, 2014 and expires with the
original contract expiration December 1, 2014.
2. SCOPE OF SERVICES. Exhibit B to existing contract is amended as shown.
3. COMPENSATION AND EXPENSES, INVOICING- COMPENSATION-.
Increase to $1,000 for 2014.
Reason for Change:
We are increasing Dr. Levin's compensation to $1,000 to cover the City of Aspen portion of$250,
which they have discontinued.
Original Contract Price: $ 8,700 in 2013; $12,000 in 2014
Net Increase/Decrease in Contract Price(this change order) $3,300
*Total Adjusted Contract Price(including this change order) approximately as follows:
$ 3,300 increase from 2013-2014
In all other respects the Agreement is in full force and effect and remains unchanged by this
