a new documentation in succeeding years to be effective.
"I'he purpose of this agreement is to summarize [he pertinen[terms of Pitkin County's arrangement with you. The County's
obligation under the Workers' Compensation S[atute requires Pitkin County and you to formalize the terms of this
engagement in a notarized writing.
It is mutually agreed that the Counry does not have an exclusive righ[ ro your services and it is contemplated that you
may provide similar work to other businesses, governments and non-profit organizations. You acknowledge that the
work performed for Pitkin County will be that of an independen[ contractor and that no employee-employer relationship
is created. You have full control over how the services are performed and it is expected that work will be conducted in
accordance with industry standards by applying professional judgment. The County will provide, if necessary,
documents and information to complete the work described in the scope of work. The County representative signing this
letter is available to you to discuss any details related to this agreement. You acknowledge and agree that you shall not
be entitled to workers' compensation benefits in connection with this project and that you shall be responsible for
the payment of all s[ate and federal income taxes.
You shall obtain, and maintain con[inuously for the term of this arrangement a[ our own expense, occurrence form
Commercial General Liability with limits of 51,000,000 Each Occurrence, 51,000,000 General Aggrega[e and
51,000,000 Products/Completed Operations Aggregate including an endorsement naming the County as an additional
insured (for ongoing operations and completed operations) and/or other insurance to indemnify Ihe Counry for the
activities and services of this of this arrangemenL You are not relieved of any liability or other obligations assumed
pursuant to this arrangement by reason of your failure to obtain or maintain insurance or by reason of your failure to
procure or maintain insurance in sufficient amounts, durations,or types.

with reasonable requests made in the course of an investigation, undertaken pursuant ro CRS §8-]7.5-102(5), by the Colorado
Department of Labor and EmploymenL If ConVactor participates in the Department program, Contractor shall deliver to the
contracting State agency, Institution of Higher Education or political subdivision a written,notarized affirmation,affirtning that
Contractor has examined the Iegal work status of such employee, and shall comply with all of the other requirements of the
Department program. If Contractor fails to comply with any requirement of this provision or CRS §8-17.5-101 et seq., the
contracting S[a[e agency, ins[itution of higher education or political subdivision may [erminate this contract for breach and, if
so terminated,Contractor shall be liable for damages.

WHEREAS, actions of the Federal Communications Commission and the Colorado Public Utilities
Commission approved use of the three digit telephone number"2-1-P' for access to community health
and human services information and referral; and
WHEREAS, Pitkin County Department of Health and Human Services wishes to provide individuals and
families in Pitkin County with referral information regarding non-emergency services such as food
assistance, shelter, financial and job assistance, low cost health care, counseling and family support .
services; and
WHEREAS, Western Colorado 2-1-1 wishes to cooperate with Pitkin Counry Department of Health and
Human Services to provide the 2-1-1 service described herein; and
WHEREAS, cooperation between Western Colorado 2-1-1 and Pitkin County Department of Health and
Human Services will be in the genera] interest and for the general welfare of the residents of Pitkin
� Counry including individuals and families meeting public assistance eligibility criteria.
NOW,THEREFORE, for and in consideration ofthe agreements set forth herein,the Pitkin County
DepaRment of Health and Human Services and Westem Colorado 2-1-1 agree as follows:
L Scope of Services:
i To establish and maintain the 2-1-1 system in Pitkin County, Westem Colorado 2-1-1 shall
provide and/or coordinate agreed upon services tha[ may include, but not be limited [o the
following:
•Develop an inventory of public agencies and non-profit programs currently providing health and
human services in Pitkin County.
• Develop and provide targeted trainings for providers on how to input agency and program data
onto the 2-1-1 website. �

• Establish and maintain a method for regularty communicating with the health and human service
agencies and programs for accurate data within the system.
• Integrate, input and update the data into the master database for Western Colorado 2-1-1.
• Provide training and orien[ation for call specialists related to knowledge of directions, landmarks
and other unique information that may be helpful to matching services in Pitkin County with
caller's needs.
• Provide coverage of the 2-1-1 Area Information Center(Call Center)with at least one AIRS—
Alliance oF Information and Referral Systems—certified professional call specialist Monday
through Friday, 8:00 am to 5:00 pm. After- hours coverage is available to callers from 5:00 pm to
8:00 am and is provided by our sister 2-1-1 in Larimer Counry.
• Submit regular reports regarding the use of Western Colorado 2-1-1 by Pitkin County residents.
• Develop and implement a strategy for outreach and promotion of Western Colorado 2-1-1 in
Pitkin County. �
➢ Develop and disseminate 2-1-1 outreach materials to promote the use of2-1-1 on an
ongoing basis, including logos and brochures. These materials will be used to emphasis a
regional approach [o information sharing/referral networks with sister counties Eagle and
Ga�eld as the three counties are working more and more closely with one another to the
benefit of Roaring Fork Valley citizens;
➢ Develop radio, newspaper and television campaigns to masimize awareness of 2-I-1 and
� its benefits to citizens.
➢ Provide a role in emergency response in Pitkin County during a disaster simation(ie. fire,
flood,winter storm,etc). 2-1-1 can also provide critical service of resource and referral
during recovery periods after a disaster as well. 2-1-1 is norrnally activated to assist in
disaster situations [o alleviate some of the stress on 9-1-1 and can feed information to the
general public with help from a PIO in that local area.
IL Time of Performance. The term of this Agreement shall begin on the 15'"day of November,
2013 and shall terminate on the 31�`day of December, 2013. This Agreement will terminate on
the date set forth above, unless renewed by a written extension amendment for another like term,
except that any terms or conditions may be reviewed and renegotiated annually.
II I. Compensation and Method of Paymeat. Payment will be in one lump sum of$5,600 to the
Western Colorado 2-1-I fiscal agent, Mesa County Department of Human Services, at the address
listed at the beginning of this Agreement.
IV. Records Reports and Information. At such times and in such forms as may be required,
Western Colorado 2-1-1 shall fumish statements, records, reports, data and information pertaining
to matters covered by this Agreement.

V. Independent Contractor. Western Colorado 2-I-1 shall perform duties hereunder as
independent contractors and not as employees. Western Colorado 2-1-1 will secure all personnel
and materials required to perform the services covered in this AgreemenL
VI. Trmination for Cause. If,through any cause, Western Colorado 2-1-1 shall fail to fulfill, in a
timely and proper manner, its obligations under this Agreement,the Department of Health and
Human Services shall therwpon have the right to terminate this Agreement for cause by giving
written notice to Wes[ern Colorado 2-1-1of its intent to terminate and at least ten(10) days
opportuniry to cure the default or show cause why termination is otherwise not appropriate.
