The contract for The Pitkin County Senior Strategic Plan (the "Agreement") dated 5/6/2013 between the
Board of County Commissioners of Pitkin County (the "County") and Executive Service Corp. (the
"Contractor"), is hereby amended as follows:
Description of Change:
Extend our agreement with Executive Service Corps by 4 months. There will be $600 remaining in the
original.;`not to exceed" agreement after we pay for additional services provided in December. An
increase.of_$4;200 will .cover 4 months (April-July) at $1,200/month. A.grant application-has been
submitted to the Area Agency on Aging at NWCCOG for two months of this extension expense.
Reason for Change:
We estimated conservatively, and some aspects of the process took longer than we anticipated. For
example, two of the four work groups required extra meetings to accomplish their goals, and there were
timing challenges assembling busy participants. The project itself was more complex than expected.
We are told by professionals in aging and planning that the results to date are exceptional, and in line
with results of other community planning initiatives on aging. Our ESC consultant/facilitators are a large
part of the success of the project and plan, and we want and need their expertise to finalize a high quality
product, and to see us through the June Senior Summit community event and action plan going forward..
Original Contract Price $N/A
Net Increase/Decrease in Contract Price (this change order) $4,200.00
*Total Adjusted Contract Price (including this change order) $ 19,200.00
This change order extends the time to complete work by 9/6/14.
In all other, respects the Agreement is in full force and effect and remains unchanged by this
Amendment.
