Changes must be approved by this Department and the engineer prior to construction. THIS SYSTEM WAS ALREADY INSTALLED PRIOR TO PERMITTING. This design calls for one 3000 gallon single compartment tank followed by one 1500 gallon two compartment tank with a biotube pump vault. Effluent will be pumped to a concrete distribution box. A flow regulator should be placed on the line going to the southernmost trench to receive only 50% flow. There will be 9 trenches installed. The first 2 will have 25 Quick 4 chambers, the next 6 will have 24 chambers, and the last will have 11 chambers for a total of 205 chambers. There must be at least 6 feet of undisturbed soil between each trench. This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as-buiit" letter to this Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection with a minimum of 48 hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation. This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturers recommendations and the requirements of the Pitkin County OWrS Regulation. Effluent is distributed with a concrete distribution box with a baffle. The box has been brought to grade with a PVC sprinkler box to provide access. That license requires you to ensure that: Construction, installation, alteration, repair, inspection, or cleaning of any OWTS shall be incompliance with [the] Regulation and with the conditions set forth in the permit. OWTS Regulation Sec. 6.28.150(C)(1)(a). On August 22, 2008, Nancy MacKenzie, the Environmental Health Specialist with this Department, conducted a final inspection of an OWTS installation for the Dupre residence at 2673 West Sopris Creek Road. The system installed was not in compliance with the design that was permitted by this Department (ISDS Permit 07065B issued on 6/16/07 for Project #1268, dated 8/8/07). Please be advised that your license may be revoked for failure to comply with the OWTS Regulation. Revocation of your license will preclude you from installing OWTSs in Pitkin County for at least one year. Thus, we hope that you take this matter very seriously and that your future OWTS installations conform to Pitkin County's regulations. It seems like if it would have been done properly, we would have required a new application packet and reviewed it under the new regs. That means new fee . and everything on the checklist (although I'm sure we have many of the required items in our file already). I will review it and issue a new permit. The wrong one was put in. So ... Nancy and I crunched numbers and saw that they had done enough to comply, based on new regs. So I changed the design. The latest design has been installed (cause it was already in), and that is the one thats needs to be permitted. Upon new permitting, I will do the as -built letter. Hope this make sense. The property is located outside of Snowmass Colorado, in an area where OWS and wells are necessary. SITE CONDITIONS A 6 -bedroom single-family residence, and a 2 -bedroom caretaker unit having a total approximate area of 12,000 SF are proposed as indicated on the plans. The lot is presently vacant. The proposed drain field area slopes 10 to 15 percent to the south, and vegetation consists of a full cover of sagebrush and grasses. The property will be served by an onsite well located 212 feet from the proposed drain field. SUBSURFACE The subsurface was investigated by digging one profile pit and three percolation holes, as indicated. The materials encountered in the profile pit consisted of 6 -inches of a clayey root zone, underlain by very sandy clay to 3 feet, underlain by sandy clay to the maximum depth explored of 8 feet. Groundwater or bedrock was not encountered. Percolation rates ranged from 27 to 40 minutes per inch (MPI), with an average rate of 36 MPI. A 36 MPI rate will be used to design the OWS. DESIGN SPECIFICATIONS The OWS design is based on 8 -bedrooms and an average sewage load of 1600 GPD. The installation must include a 3000 -gallon, single -compartment, precast concrete septic tank, followed by a 1500- ) 1p • Subsurface InTestigation and Revised OWS Design Project No. 1268 8/25/08 Page 2 gallon, two-compartment concrete septic tank with a Biotube pump vault. Tank alternatives may be considered. Nine gravelless chamber trenches, using `Quick -4' Infiltratof"M chambers, must be installed as indicated. Trenches must have a total of 205 chambers, equaling 2024 square feet (SF) of infiltrative area. One trench may have 11 chambers, although a flow -regulating device must be placed in the distribution box to regulate effluent at 50% to that trench. A concrete distribution box, with access brought to grade must be installed. Construction must be according to the county ISDS regulations, the septic permit provided by Pitkin County Environmental Health Department, and this design. OPERATION INFORMATION AND MAINTENANCE The surface of the drain field should be seeded upon completion. Vegetation is an important factor in drain field performance. Erosion control should be practiced prior to and during re -vegetation. Geo - fabrics or plastics should not be used over the drain field. Livestock should not graze on the drain field. Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWS. For Example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a drain field. The homeowner should pump the septic tank every two years and clean the effluent filter as needed. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed into the OWS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWS. No hazardous wastes should be directed into the OWS. Mechanical room drains should not discharge into the OWS. The OWS is engineered for domestic waste only. ADDITIONAL CONSTRUCTION NOTES If design includes a pump, air release valves and weep holes should be installed to allow pump lines to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain field due to the potential to compact soil. Extensions should be placed on all septic tank components to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices is optimal. INSTALLATION OBSERVATIONS ALL SERVICE septic, LLC, and the county must view the OWS during construction. The OWS observation should be performed before backfill, after placement of chambers and distribution pipes. Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the installation. If soil conditions encountered are different from conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction must be according to the county regulations. Requirements not specified in this report must follow county regulations.
