This OWTS Construction Permit is approved on the condition of compliance with the engineer design as submitted with the application and the specifications
cited above. Changes must be approved by this Department and the engineer prior to construction.
This is a permit amendment to permit 0018.2012.powt due to the discovery of groundwater 2' below grade. Permit
0018.2012.powt is void. This system is designed for 7 bedrooms and will consist of 1500 single compartment tank followed
by a 2000 gallon 2 -compartment tank with a 1.5 FAST treatment system in the second chamber. The treated effluent will then
flow to a 1000 gallon concrete pump vault. The soil treatment area shall be a 75X25 foot mounded bed. A minimum of 2 feet of
native material and 6" of gravel will be used to build the mound. 1.5" distribution pipe in (5) 70 foot laterals with 1/8" holes
every 4' will distribute the effluent to the soil treatment area. The piping shall then be covered with 2" of gravel and 1 -foot of
cover with a final minimum slope of 3:1.
This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this
Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for
the final Inspection with a minimum of 48 hours notice.
Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS
Regulation.
This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, In
accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction
Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail.
THIS PERMIT IS EXPRESSLY CONDITIONED UPON COMPLIANCE WITH ALL REQUIREMENTS OF THE PITKIN
COUNTY OWTS REGULATION, INCLUDING BUT NOT LIMITED TO THOSE CONDITIONS SPECIFIED ABOVE.
Plans and specifications for the proposed OWTS have been reviewed and are considered satisfactory. Permission is hereby
given to the property owner(s) to perform the work indicated in accordance with the Pitkin County OWTS Regulation. This
Permit will expire 1 year from the date of issuance unless construction on the system has commenced. An "as -built" drawing
must be submitted and approved by EH before final approval of the system will be issued.

ALL SERVICE septic, LLC observed the installation of the onsite wastewater treatment system
(OWTS) on February 28, April 13, and August 1, 2013 and May 22, 2014 for the subject property.
Hughes Excavation installed the system. Start up of the pump and MicroFAST® system was
conducted by Valley Precast in May of 2013. Valley Precast personnel also performed a squirt test
on the pressurized field at the time of pump start up.
The installation included a 1500 -gallon, single -compartment concrete septic tank, followed by a
2000 -gallon treatment tank with a 1.5 MicroFAST® unit, followed by a 1000 -gallon, two-
compartment tank with a Biotube® Pump Vault and duplexing pump system. The 1000 -gallon tank
was anchored due to high ground water levels in the area of the tank installation.
Effluent is pumped to a 75' x 25' pressurized mound system, for a total of 1875 square feet of
infiltrative area. Groundwater was discovered at approximately 2 -feet below grade. The mound
consists of 8 -inches of gravel with 1.5 -inch perforated pressure pipe, underlain by 2 -feet of native
filter material. Proper cover over the mound system was established. The mound has greater than
3:1 slope.
An annual service contract for the MicroFASTO is required per manufacturer specifications and
Pitkin County OWTS Regulation. A copy of the executed service contract must be submitted to
Pitkin County Environmental Health Department. This service contract is currently maintained by
Valley Precast.
The OWTS was generally installed according to specifications. This observation is not a guarantee
of workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if
changes are made to the OWTS in the future. Any additional OWTS construction must be according
to the county regulations.

must have documented and demonstrated knowledge of the requirements and regulations of the
county in which they are working. The quality of the installation is dependent of the expertise of the

We have reviewed the soils report prepared by HP Geotech dated July 17's 2013, Project No 113-
064K.
The soils are reported as a very silty sandy loam. This is indicative of a 30 MPI percolation rate.
Ninety percent of the soil tested is granular material, which exhibits adequate percolation, and 10% of
the soil tested was clay.
The design loading for the subject property, of 30 MPI is acceptable. We understand a pressurized
system with secondary treatment has been submitted, and we agree with the technology. Sizing
based on 30 MPI with dosing and secondary treatment requires a minimum square footage of 1840
square feet. This requirement is consistent with the sizing criteria outlined in the design revisions
dated July 22, 2013.

ALL SERVICE septic, LLC was notified of high groundwater levels in the area of the field
installation. A site visit was conducted on July 18, 2013 and groundwater was observed in a pit near
the absorption area at approximately 2 -feet below grade. Groundwater will be estimated at 2 -feet
for the purposes of this design.
The OWTS design dated August 21, 2012 has already been installed. The design is based on 7 -
bedrooms for the main residence with an average daily flow of 1400 GPD. The installation includes a
1500 -gallon, single -compartment tank followed by a 1.5 MicroFAST® system in a 2000 -gallon tank,
followed by a 1000 -gallon, two-compartment concrete tank with a Biotube® pump vault in the
second compartment. An the automatic distributing valve (ADV) was installed to distribute effluent
to ten gravelless chamber trenches, each with ten `Quick 4' Infiltrator® chambers in each row.
The previously installed tanks and secondary treatment system will remain; however, and the ADV
and all Infiltratorg chambers will be removed. Trench voids must be filled with on-site soils and
prepared for installation of the mound.

On-site soils were sampled for feasibility of use in the mound system. A gradation analysis and
hydrometer analysis was run by HP Geotech and results are detailed in a report dated July 17, 2013, Job
No 113 064K.
The gradation performed by HP Geotech shows the soil consists of 6% gravel, 39% sand, 45% silt, and
10% clay. The sample tested has a USDA Soil Classification of loam. A long term acceptance rate
(LTAR) of 0.90 gal/sq. ft will be used to design the mound. The basal area under the mound has a

perc rate of approximately 27 mpi, as described by previous soil investigation by ALL SERVICE septic,
LLC dated November 4, 2008.
DESIGN SPECIFICATIONS
The mound system will be 75-feet by 25-feet and consist of a minimum of 2-feet of on-site material,
6-inches of gravel conforming to specifications detailed in Table 6.28-5 of Pitkin County Onsite
Wastewater Treatment System Regulations, 1.5" distribution piping with a level manifold and 70-
foot laterals with 5-feet of separation between the laterals. Distribution piping should be 2.5-feet
from the edge of the mound. 1/8-inch holes should be placed every 4', facing down, in the
distribution laterals with the last hole facing up. 2-inches of gravel must be placed over the
distribution piping, covered by native soil for a minimum of ]-foot of cover over the distribution
laterals. The mound must have a minimum of a 3:1 slope.
The existing Orenco® PF3005 Duplex pumping system will pressurize the mound system. A squirt
test should be conducted prior to covering the distribution piping.
This design must be submitted to Pitkin County Environmental Health Department with an
application for a Request for an Amendment to an OWTS Permit.

Construction must be according to the Pitkin County Environmental Health Department OWTS
regulations, the OWTS Construction Permit provided by Pitkin County Environmental Health
Department, and this design.
OPERATION INFORMATION AND MAINTENANCE
The surface of the drain field should be seeded upon completion. Vegetation is an important factor in
drain field performance. Erosion control should be practiced prior to and during re-vegetation. Geo-
fabrics or plastics should not be used over the drain field. No heavy equipment, machinery, or
materials should be placed on backfilled drain field. Livestock should not graze on the drain field.
Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS.
For example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and
harm a drain field.
The homeowner should pump the septic tank every two years, or as needed gauged by accumulation
of scum and sludge in the tank. The effluent filter should be cleaned as needed. Garbage disposal use
should be minimized, and non-biodegradable materials should not be placed into the OWTS. Grease

should not be placed in household drains. Loading from a water softener should not be discharged
into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains
should not discharge into the OWS. The OWTS is engineered for domestic waste only.
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, air release valves and weep holes should be installed to allow pump lines
to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain
field due to the potential to compact soil. Extensions should be placed on all septic tank components
to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be
used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices
is optimal.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC, and the county must view the OWTS during construction. The OWTS
observation should be performed before backfill, after placement of chambers and distribution pipes.
Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must
also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the
installation.
LIMITS:
The design is based on information submitted. If soil conditions encountered are different from
conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction
must be according to the county regulations. Requirements not specified in this report must follow
county regulations. The installer should have documented and demonstrated knowledge of the
requirements and regulations of the county in which they are working.

This system is designed for 7 bedrooms and I consist of 1isingle compartment tank followed by a 2000 gallon 2
compartment tank with a 1.5 FAST treatment stem in the snd chamber. This flows into a pump tank which will dose
effluent to an ADV (automatic distributing valv which will diuted effluent to 5 zones. The ADV must be placed at a high
point to allow drain back. Each zone will consi of 2 "quick avelless chamber trenches for a total of 10 trenches with 10
quick4 units, each separated by at least'6,feet o undisturbedl. Trenches can be a maximum of 3 feet deep.
This Permit must be kept on-site during installation. The gineer musTdo a final inspection of the installation and submit an "as -built" letter to this
Department within 30 days of the final inspection, unless alRnger period of time has been agreed upon, in writing. This Department must also be called for
the final inspection with a minimum of 48 hours notice.
Minimum horizontal distances between components of the syst and physical features shall conform to the requirements of the Pitkin County OWTS

ALL SERVICE septic, LLC performed a subsurface investigation and an onsite wastewater treatment
system (OWTS) design for the subject residence. The property is located outside of Snowmass,
Colorado in an area where OTWS and wells are necessary.
SITE CONDITIONS
A 5 -bedroom single-family residence (one of the rooms is a bunkroom), having greater than 6000 SF
of living area is proposed at the subject property. The design will be based on 7 -bedrooms for future
build out consideration. The property is located along Snowmass Creek. The proposed drain Feld
area is relatively flat, and vegetation consists of a moderate cover of grasses. Water is provided by a
well, located 188+ feet from proposed absorption area.
SUBSURFACE
The subsurface was investigated by digging one profile pit and three percolation holes, as indicated. The
materials encountered in the profile pit consisted of 1 -foot of a clayey root zone, underlain by sandy clay
with scattered boulders to the maximum depth explored of 7 feet. Groundwater or bedrock was not
encountered. Three percolation holes were dug, in the area of the proposed drain field, as indicated.
Percolation rates ranged from 20 to 40 minutes per inch (MPI). A 20 MPI rate will be used to design
the OWTS.
DESIGN SPECIFICATIONS
The OWTS design is based on 7 -bedrooms for the main residence with an average daily flow of 1400
GPD. The installation must include a 1500 -gallon, single -compartment tank followed by a 1.5 FAST
system in a 2000 -gallon tank, followed by a 1000 -gallon, two-compartment concrete tank with a
Biotube pump vault in the second compartment. Tank alternatives may be considered. Ten gravelless

chamber trenches, using `Quick4' InfiltratorTM chambers, must be installed, as indicated. Each
trench must have 10 chambers each for a total of 100 chambers, equaling 987 SF of infiltrative area.
An automatic distributing valve must be used to distribute effluent to the trenches.
Construction must be according to the Pitkin County Environmental Health Department OWTS
regulations, the OWTS Construction Permit provided by Pitkin County Environmental Health
Department, and this design.
OPERATION INFORMATION AND MAINTENANCE
The surface of the drain field should be seeded upon completion. Vegetation is an important factor in
drain field performance. Erosion control should be practiced prior to and during re -vegetation. Geo -
fabrics or plastics should not be used over the drain field. No heavy equipment, machinery, or
materials should he placed on backfilled drain field. Livestock should not graze on the drain field.
Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS.
For example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and
harm a drain field.
The homeowner should pump the septic tank every two years, or as needed gauged by accumulation
of scum and sludge in the tank. The effluent filter should he cleaned as needed. Garbage disposal use
should be minimized, and non -biodegradable materials should not be placed into the OWTS. Grease
should not be placed in household drains. Loading from a water softener should not be discharged
into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains
should not discharge into the OWS. The OWTS is engineered for domestic waste only.
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, air release valves and weep holes should be installed to allow pump lines
to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain
field due to the potential to compact soil. Extensions should be placed on all septic tank components
to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be
used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices
is optimal.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC, and the county must view the OWTS during construction. The OWTS
observation should be performed before backfill, after placement of chambers and distribution pipes.
Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must
also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the
installation.

The design is based on information submitted. If soil conditions encountered are different from
conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction

Hi Doug,
I know things are still getting sorted out with this property, but I am also sending the
signed permit to Tim Petz. We will both have this on record for future reference, so don't
feel like you have to keep this in your back pocket. I looked in the Assessor's files and the
new owner is not listed there as of today.
I did a site visit today and wanted to get this permit issued so we can sign off on the
building permit when they are ready to go. As a side note, this address did not make much
sense to me, given other addresses nearby and the side of the road the property is located
on. I have emailed Community Development to let them know.
If you have any questions, feel free to call, but the permit will be on file and ready to go
when the new owner is ready to either put in the system or pull a building permit.

ALL SERVICE septic, LLC performed a subsurface investigation and an onsite wastewater treatment
system (OWTS) design for the subject residence. The property is located outside of Snowmass
Colorado, in an area where OTWS and wells are necessary.
SITE CONDITIONS
A "gym single-family residence, having greater than 6000 SFIof living area, and ®
46MOVe proposed at the subject property. The ADU will have approximately 1000 square feet (S
The property is located along Snowmass Creek. The proposed drain field area is relatively flat, and
vegetation consists of a moderate cover of grasses. Water is provided by a well, located 188+ feet
from proposed drain field.
SUBSURFACE /� o
The subsurface was investigated by digging one profile pit and three percolation holes, as indicated. The / Y
materials encountered in the profile pit consisted of**bot of a clayey root zone, underlain by sandy clay"
with scattered boulders to the maximum depth explored of 7 feet. Grouqdigpter or bedrock was note
incountered. P3ree percolation holes were dug, in the area of the proposed drain field, as indicated.
Percolation rates ranged from 20 to 40 minutes per inch (MPI). A 27 MPI rate will be used to design
the OWTS.
DESIGN SPECIFICATIONS
The OWTS design is based on 7 -bedrooms for the main residence and 1 bedroom for the ADU and
an average sewage load of 1550 GPD. The installation must include a 2000 -gallon, single -
compartment, precast concrete septic tank, to be followed by a 1000 -gallon, two-compartment

concrete tank with a Biotube pump vault in the second compartment for the main residence, and a
1000 -gallon, two-compartment concrete septic tank with a Biotube pump vault in the second
compartment for the ADU. Tank alternatives may be considered. Sixteen gravelless chamber
Vpnches, using `Quick -4' Infiltratofiv chambers, must be installed as indicated. Each trench must
have 11 chambers each for a total of 176 chambers, equaling 1738 SF of infiltrative area. A flow
splitter basin and serial distribution must be used to distribute effluent.
Construction must be according to the Pitkin County Environmental Health and Natural Resources
Department OWTS regulations, the OWTS Construction Permit provided by Pitkin County
Environmental Health and Natural Resources Department, and this design.
OPERATION INFORMATION AND MAINTENANCE
The surface of the drain field should be seeded upon completion. Vegetation is an important factor in
drain field performance. Erosion control should be practiced prior to and during re -vegetation. Geo -
fabrics or plastics should not be used over the drain field. No heavy equipment, machinery, or
materials should be placed on backfilled drain field. Livestock should not graze on the drain field.
Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS.
For Example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and
harm a drain field.
The homeowner should pump the septic tank every two years and clean the effluent filter as needed.
Garbage disposal use should be minimized, and non -biodegradable materials should not be placed
into the OWTS. Grease should not be placed in household drains. Loading from a water softener
should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS.
Mechanical room drains should not discharge into the OWS. The OWTS is engineered for domestic
waste only.
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, air release valves and weep holes should be installed to allow pump lines
to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain
field due to the potential to compact soil. Extensions should be placed on all septic tank components
to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be
used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices
is optimal.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC, and the county must view the OWTS during construction. The OWTS
observation should be performed before backfill, after placement of chambers and distribution pipes.
Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must
also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the
installation.

The design is based on information submitted. If soil conditions encountered are different from
conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction
must be according to the county regulations. Requirements not specified in this report must follow
county regulations. The installer should have documented and demonstrated knowledge of the
requirements and regulations of the county in which they are working.
