CONTRACTOR:
The Provision of 911 Dispatch Relocation Consulting Services (the "Agreement") dated October 12,
2015 between the Board of County Commissioners of Pitkin County (the "County") and DiDonato
Consulting Services, Inc. (the "Contractor"), is hereby amended as follows:
Description of Change:
• County requests additional hours and travel from Contractor. Increase the budget amount for the
Contract an additional$45,000.00.
• Contractor shall apply a decrease of 10% in the Contractor's hourly rates, effective December 1,
2015. This shall appear as '10% Small Agency Labor Discount' applied to each invoice submitted
to County.
Travel costs and other expenses shall remain as defined in Contract.
Reason for Change
The initial project kick off meetings indicate that more hours will be required for 911 Dispatch Relocation
Consulting.
Original Contract Price $ 15,000.00
Net Increase/Decrease in Contract Price(this change order) $45,000.00
*Total Adjusted Contract Price(including this change order) $ 60,000.00
In all other respects the Agreement is in full force and effect and remains unchanged by this
Amendment.
