Septic permit approved per compliance with the engineer design and specifications dated May 15, 2006. Any
changes must be approved by this department and the design engineer prior to them being made.
Minimum horizontal distances between components of the system and physical features shall conform to the Pitkin
County ISDS regulations.
This design calls for one 1000 gallon two-compartment tank with an effluent filter on the outlet. A distribution box
with a baffle will distribute the effluent to 6 rows of 6 Quick 4 chambers.
This department does not endorse any brand of products. This permit must be kept on-site during installation.
The engineer must do a final inspection of the installation and submit an as -built letter to this department. This
department must also be called for an inspection with a minimum of 48 hours advanced notice.
The owner has the responsibility that all components are inspected and maintained on a regular basis by qualified personnel.
Revegetation over the area of the field is very important for the functioning of the system. Pitkin County has guidelines that must be
followed to assure the plants that are introduced are appropriate for the conditions of the area. The use of native plants is strongly

The fee for a ISDS application is $600 for a permit that takes 6 hours or less for the department to approve. If approval takes
longer than 6 hours, a rate of $100 per hour will be charged. The maximum fee is $1000. The basic fee of $600 is due at the
time of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit; second,
before final approval of the ISDS permit.
Application for an individual sewage disposal system is hereby submitted. I hereby certify that the above information is true and
accurate and that I have provided true and accurate information on locations of all existing and proposed wells, contour
intervals, buildings, property lines, ditches, slopes, waterlines, springs, suction or irrigation lines, drinking water cisterns, drain
tiles, irrigation ditches, lakes, water courses, streams, floodplains, dry gulches, and existing septic systems. I hereby certify that
any such features not shown on attached site map are not present. Issuance of the permit does not imply the approval of any
other permit required for construction pursuant to Pitkin County codes. No construction may be undertaken until all approvals
and permits have been obtained.

Here are the photos I took.
By our calc sheet he only needs 34 chambers, so they could remove the two closest to the property line.
Besides the tank riser, we'll need to check that he puts a handle extension on the effluent filter.
The inlet tee needs an upper extension to bring it to within 1" of the underside of the tank lid.

ALL SERVICE septic, LLC performed an installation observation on October 26th, 2006, for the
subject property.
The installation included a 1000 -gallon, two-compartment precast concrete septic tank with the
addition of an effluent filter in the second compartment, and one `Quick -4' chamber bed. The bed
had a total of 34 chambers, and 313 SF of infiltrative area. A distribution box was installed to
distribute effluent, as indicated on as -built drawings.
Our OWS design dated May 15th 2006 indicated a total of 36 chambers to be installed. It was
authorized by our office on October 25th to install 34 chambers. This was due to underground utilities
and setbacks to property lines.
The OWS was installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes
are made to the OWS.

The observation was based on information submitted and our site visit. If conditions are different
from conditions described in this letter, ALL SERVICE septic, LLC should be notified. All additional
OWS construction must be according to the county regulations. The installer should have
documented and demonstrated knowledge of the requirements and regulations of the county in which
they are working.

The plastic rings that you saw onsite were the risers. He was going to install plastic
risers.
He changed to concrete on our request. I like concrete better, but some plastic risers
are quite good, like Orenco.

Subsurface Investigation & Revised Upgrade Onsite Wastewater System Design Existing Residence

ALL SERVICE septic, LLC performed a percolation test and a revised upgrade onsite wastewater
system (OWS) design for the 2 -bedroom guest quarters above the garage. The revision is based on a
change in the drain field location. All previous OWS designs regarding this property should be
discarded to avoid confusion. The property is located in Holland Hills Subdivision, in an area where
OWS are necessary. The local water district serves the residence.
SITE CONDITIONS
There is an existing residence that is served by an individual OWS. A 2 -bedroom guest quarters
exists above the garage, and is presently using the existing OWS. This OWS design will separately
serve the guest quarters. �k Vv -k -s has y

The subsurface was investigated by excavating an 8 -foot test pit near the proposed drain field location as
shown on attached plans. Three percolation holes were dug. The test pit indicated the site has a 6 -inch
root zone, underlain by sandy clay to clayey sand to the maximum depth explored of 8 feet.
Groundwater or bedrock was not encountered. Percolation rates ranged from 14 to 20 minutes per
inch (MPI). An 18 MPI rate will be used to design the OWS.
DESIGN SPECIFICATIONS
The OWS design is based on 2 -bedrooms and an average sewage load of 300 GPD. The installation
must include a 1000 -gallon, two-compartment, precast concrete septic tank, with the addition of a
filter in the second compartment. Tank alternatives may be considered. One gravelless chamber bed
must be installed as indicated on plans provided. Six rows of 6 `Quick 4' chambers, for a total of 36
chambers, equaling 331 square feet (SF) of infiltrative area, are required. A distribution box should
be used to distribute effluent. Construction must be according to the county ISDS regulations, the
septic permit provided by Pitkin County Environmental Health Department, and this design.

OPERATION INFORMATION AND MAINTENANCE
The surface of the drain field should be seeded upon completion. Vegetation is an important factor in
drain field performance. Geo -fabrics or plastics should not be used over the drain field. Livestock
should not graze on the drain field. Plumbing fixtures should be checked to ensure that no additional
water is being discharged to OWS. For Example, a running toilet or leaky faucet can discharge
hundreds of gallons of water a day and harm a drain field.
The homeowner should pump the septic tank every two years and clean the effluent filter as needed.
Garbage disposal use should be minimized, and non -biodegradable materials should not be placed
into the OWS. Grease should not be placed in household drains. Loading from a water softener
should not be discharged into the OWS. No hazardous wastes should be directed into the OWS.
Mechanical room drains should not discharge into the OWS. The OWS is engineered for domestic
waste only.
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, air release valves and weep holes should be installed to allow pump lines
to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain
field due to the potential to compact soil. Extensions should be placed on all septic tank components
to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be
used, unless plastic or fiberglass is required. Septic tank risers should extend to grade. Access to all
tank compartments and distribution devices is optimal.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC, and the county must view the OWS during construction. The OWS
observation should be performed before backfill, after placement of chambers and distribution pipes.
Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must
also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the
installation.
LIMITS:
The design is based on information submitted. If soil conditions encountered are different from
conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction
must be according to the county regulations. Requirements not specified in this report must follow
county regulations. The installer should have documented and demonstrated knowledge of the
requirements and regulations of the county in which they are working.
