County ISDS Regulation in effect at the time of issue. This permit becomes invalid 6 months from the date that the permit was
issued unless system construction has commenced or an extension has been approved in writing by the Department.

The fee for a ISDS application is $330 for a permit that takes 6 hours or less for the department to approve. If approval lakes
longer than 6 hours, a rate of $55 per hour will be charged. The maximum fee is $1000. The basic fee of $330 is due at the time
of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit: second, before
final approval of the ISDS permit.
Application for an individual sewage disposal system is hereby submitted. I hereby certify that the above information is true and

Intervals, buildings, property lines, ditches, slopes, waterlines, springs, suction or irrigation lines, drinking water cistems, drain

. Issuance of the permit does not imply the approval of
that any such features not shown on attached site map are not present
any other permit required for construction pursuant to Pitkin County codes. No construction may be undertaken until all

The Aspen/Pitkin Environmental Health Department, Pitkin County and employees of these agencies will be held harmless
should the individual sewage disposal system fail or malfunction, The permit to construct is issued on information submitted by
the applicant or his/her representatives. The owner assumes full responsibility, in case of failure of the system.

Dear Nancy:
Pursuant to County Regulations, this letter and attached drawing provides documentation that the new
ISDS system recently installed is in general compliance with the permitted design. Sopris Engineering has
performed two site visits to measure, inspect, and document the as built conditions of the constructed
system. We have coordinated our efforts with the contractor that built the system. The as built size and
relative location of the new ISDS system is delineated on the attached drawing.
A 2000 gallon dual compartment septic tank was installed with 4" tee baffles. Four inspection
port/monitoring wells were installed. The absorption field was constructed at the proposed relative
elevations delineated on the site plan. The location of the field was moved approximatley 40 feet to the
west. The 18' x 86' bed was installed level utilizing specified materials installed according to design. The
minimum setback distances from the well and river high water line has been maintained. The dual grinder
pump assemblies and dosing tank has been installed according to the engineered design plans.
The distribution/treatment media, distribution pipes, and installation are in accordance with Pitkin County
Regulations, the design presented in the Sopris Engineering Report and the design drawings, both dated
September 28, 1998. Attached is an as -built drawing dated 1/31/01.

Sony I didn't get back to you sooner in response to your letter dated January 6, 2000. I was called to
review the system at the very beginning of construction. The contractor had not completed several items.
The contractor was to call when he was ready for a final inspection and preparation of as -built drawings. I
have never heard from the contractor and I don't know the status of the ISDS or if the house has been built.
If the contractor or owner contacts me I would be glad to review and document the ISDS.

i spoke with marty schlumberger this afternoon, and researched the lulu
subdivision approvals a bit. their recorded covenants (which were reviewed
by the County) allow septic systems outside of the building envelopes, and
resolution no. pZ-90-39 does not specifically require the septic to be
within the building envelope, so i would say that they are OK with the
septic outside of the envelope. please let marty know if everything is ok
now. thanks

Dear Mr. Schlumberger:
As requested by you, a representative of Hepworth-Pawlak Geotechnical, Inc. performed
percolation testing at the subject site. The work was done in accordance with our
agreement for Professional Services to you dated July 29, 1998. We previously conducted
a subsoil study for foundation design at the subject site and presented our findings in a
report dated March 31, 1998, Job No. 198 178-1
A profile pit and three percolation test holes were excavated on August 4, 1998 at the
locations shown on Fig. 1. The profile pit was logged, hand dug holes excavated in the
test pits and the percolation test holes were pre-soaked. The subsoil encountered in the
Profile Pit consisted of about %r foot of topsoil overlying silty sand and gravel with
scattered cobbles to a depth of 9 feet. No groundwater was observed in the Profile Pit and
the subsoils were slightly moist to moist.
Percolation testing was performed on August S, 1998. The percolation test results are
presented on Table I. The percolation rates were based on the last two readings of the test.

The subject site is located off Lower River Road on Lot 2, Lulu Subdivision in Pitkin County.
The house will have four bedrooms and a living area of 4,489 square feet. The lot is
approximately 13.24 acres in size. Water is supplied by a well on the east side of the
property about 80 feet south of the edge of pavement of Lower River Road. The existing
ground surface in the area for the proposed leach field slopes to the southwest at a 7%
grade.
Subsurface Conditions:
Attached herewith are the results of the subsurface investigation and percolation test
conducted by Hepworth-Pawlak Geotechnical, Inc. dated August 5, 1998. The subsoil
encountered consists of about six (6) inches of topsoil above silty sand and gravel with
scattered cobbles to a depth of nine (9) feet. No water table was encountered above
the nine feet depth of the profile pit.
Three test holes were used with varying absorption rates for each of the test holes with
the fastest measured rate of 15 minutes per inch and the slowest measured at 30
minutes per inch. The design of the proposed system was based on the longer, more
conservative absorption time of 30 minutes per inch.
Final System Design:
A site plan is provided to detail the various components of the proposed system. Please
note that the location of the absorption bed (leach field) will require a lift pump as the
bottom of the bed will be two to three feet above the finished floor elevation at the
middle section of the new structure. A dosing tank is suggested and recommended to
assist the absorption field.

Pitkin County Reauirements:
Under the Pitkin County Individual Sewaae Disposal System Regulation (Effective January
1, 1998), the system criteria is based on a peak daily sewage flow to the absorption area
of 1400 gallons per day. This flow will require a total septic tank volume of 1750 gallons.
A minimum of 1534 square feet of conventional gravel absorption bed is required by
county regulation. As stated above, it is highly recommended two grinder pumps be
installed to feed the leach field from the dosing tank.

1) All materials and installation practices shall conform to the Pitkin County Individual
Sewage Disposal Regulation.
2) All sewer lines in the system are to be Schedule 40 or SDR -35 PVC.
3) The contractor shall take whatever measures are necessary to ensure that the
concrete septic tank and sewer lines are completely watertight.
4) Add a two - way cleanout to the service line from the house.
