Septic permit approved per compliance with the engineer design and specifications dated December 4, 2006. Any
changes must be approved by this department and the design engineer prior to them being made.
Minimum horizontal distances between components of the system and physical features shall conform to the Pitkin
County ISDS regulations. The engineer has stated that the setback from the field to the ditch below can be reduced
to 60 feet. A geologic report accompanies his statement. There must be at least a 10 foot separation from the field
to the any property lines.
This system will consist of the existing 1250 gallon 2 compartment tank with an effluent filter on the outlet followed
by one 1250 gallon 2 compartment tank with a pump in the 2ntl compartment. Effluent will be pumped to a field
consisting of 3 trenches with 18 Quick 4 chambers and 2 trenches with 12 chambers, for a total of 78 chambers.
Serial distribution will be used. There must be at least 6 feet of undisturbed soil between each trench. The existing
drywell will be abandoned. An 8 foot profile hole on the downhill portion of the proposed field must be dug to
verify soil conditions prior to installation.
This department does not endorse any brand of products. This permit must be kept on-site during installation.
The engineer must do a final inspection of the installation and submit an as -built letter to this department. This
department must also be called for an inspection with a minimum of 48 hours advanced notice.
The owner has the responsibility that all components are inspected and maintained on a regular basis by qualified personnel.
Revegetation over the area of the field is very important for the functioning of the system. Pitkin County has guidelines that must be
followed to assure the plants that are introduced are appropriate for the conditions of the area. The use of native plants is strongly

The fee for a ISDS application is $600 for a permit that takes 6 hours or less for the department to approve. If approval takes
longer than 6 hours, a rate of $100 per hour will be charged. The maximum fee is $1000. The basic fee of $600 is due at the
time of application. The remainder, if any, will be due in two stages: first, at the time of issuance of the ISDS permit; second,
before final approval of the ISDS permit.
Application for an individual sewage disposal system is hereby submitted. I hereby certify that the above information is true and
accurate and that I have provided true and accurate information on locations of all existing and proposed wells, contour
intervals, buildings, property lines, ditches, slopes, waterlines, springs, suction or irrigation lines, drinking water cisterns, drain

any such features not shown on attached site map are not present. Issuance of the permit does not imply the approval of any

ALL SERVICE septic, LLC performed an installation observation on January 4, 2007, for the subject
property.
The installation included the existing 1250 -gallon septic tank, followed by a new 1250 -gallon, two-
compartment precast concrete septic tank with a BiotubeTM pump vault. Five gravelless chamber
trenches using `Quick -4' chambers were installed for a total of 770 SF. Three trenches had 18
chambers, and two trenches had 12 chambers, for a total of 78 'Quick -4' chambers. Serial
distribution was used to distribute effluent.
The OWS was installed according to specifications. This observation is not a guarantee of
workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes
are made to the OWS.
LIMITS:
The observation was based on information submitted and our site visit. If conditions are different
from conditions described in this letter, ALL SERVICE septic, LLC should be notified. All additional
OWS construction must be according to the county regulations. The installer should have
documented and demonstrated knowledge of the requirements and regulations of the county in which
they are working.

This report presents the geologic investigation for the subject site. The basis of our geologic investigation
is the setback between onsite wastewater system (OWS) drain field and irrigation ditches per Pitkin
County Environmental Health Department (PCEHD) regulations. The geologic investigation addresses
the separation distances between water features and the proposed drain fields."elite onsite wastewater
design (OWS) has been presented under separate cover, prepared by All Service Septic, LLC.
SITE CONDITIONS
The site is located in a residential area where OWS are required. A full cover of native grasses exist at the
site. The slope of the proposed drain field location is 5% to the south-southwest An irrigation ditch flows to
the south of the property.
GEOLOGIC SETTING
GEOLOGIC MAP OF THE CARBONDALE QUADRANGLE. C -97-A and GEOLOGIC MAP OF THE
WOODY CREEK QUADRANGLE. GO -%7 were used in the investigation.
A test pit will be excavated in the area of the proposed drain field prior to construction to confirm the
upper soils condition. Based on our experience in the area, and the digging of the percolation holes, the
upper soils consist of silty, clayey sand. Ground water is not anticipated within 8 feet of the current grade.
The geological maps indicate Terrace and Pediment Gravel (Holocene, Qg) deposits, underlain by the
Maroon Formation (PIPm). The Matron Formation consists of clayey siltstone and silty claystone with
lenticular beds of sandstone. The general fracture plane of the Maroon Formation indicated on GQ -967 is
11 degrees to the west. The general fracture plane of the local bedrock will not direct effluent
directly towards the subject irrigation ditch. The upper soils in the subject area are residual soils of
the Maroon Formation and are suitable for proper treatment of effluent. The onsite soils will promote
proper treatment of effluent with 4 feet of soil and effluent interface.

CONCLUSIONS
' Based on geological conditions and evaluation of risk of contamination of surface water and ground
water, it is not necessary to add 8 feet additional distance for each 100 gallons of wastewater flow over
I' 1000 gallons per day as specified in the table to prevent contamination. A 60 -foot separation between the
absorption area and the irrigation ditch is appropriate to prevent contamination. All other setback
distances must be followed.

' If conditions considerably different from those described in this report are encountered, we should be
notified to observe the conditions. If proposed construction is changed, we should be notified to evaluate
the effect of the changes on the wastewater system. If there are questions, please call.

ALL SERVICE septic, LLC performed a percolation test and a repair onsite wastewater system
(OWS) design for the subject residence. The property is located outside of Basalt Colorado, in an area
where OWS are necessary. A geological report has been prepared by our firm and is presented under
separate cover.
SITE CONDITIONS
A 5 -bedroom single-family residence, having less than 6000 SF exists on the property. The local
water district serves the subject property. There is an existing drywell that will be abandoned,
although the existing septic tank will be incorporated into the proposed OWS. The proposed drain
field area slopes 5 percent to the southwest, and vegetation consists of a full cover of native grasses.
An irrigation ditch flows south of the proposed drain field.

The subsurface was evaluated by performing a percolation test. An 8 -foot test pit must be dug to confirm
soil conditions prior to construction. Three percolation holes were dug, in the area of the proposed drain
field, as indicated. Percolation rates ranged firom 20 to 16 minutes per inch (MPI). A 19 MPI rate will
be used to design the OWS. The percolation holes were hand dug, presoaked, and prepared
appropriately prior to performing percolation test.
DESIGN SPECIFICATIONS
The OWS design is based on 5 -bedrooms and an average sewage load of 1000 GPD. The installation
must include the existing 1250 -gallon, two-compartment, precast concrete septic tank, with an
effluent filter added to the outlet, to be followed by a 12,0 -gallon, two-compartWAW concrete septic

OPERATION INFORMATION AND MAINTENANCE
The surface of the drain field should be seeded upon completion. Vegetation is an important factor in
drain field performance. Erosion control should be practiced prior to and during re -vegetation. Geo -
fabrics or plastics should not be used over the drain field. Livestock should not graze on the drain
field. Plumbing fixtures should be checked to ensure that no additional water is being discharged to
OWS. For Example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day
and harm a drain field.
The homeowner should pump the septic tank every two years and clean the effluent filter as needed.
Garbage disposal use should be minimized, and non -biodegradable materials should not be placed
into the OWS. Grease should not be placed in household drains. Loading from a water softener
should not be discharged into the OWS. No hazardous wastes should be directed into the OWS.
Mechanical room drains should not discharge into the OWS. The OWS is engineered for domestic
waste only.
ADDITIONAL CONSTRUCTION NOTES
If design includes a pump, air release valves and weep holes should be installed to allow pump lines
to drain to minimize risk of freezing. Excavation equipment must not drive in excavation of drain
field due to the potential to compact soil. Extensions should be placed on all septic tank components
to allow access to them from existing grade. Precast concrete tanks and distribution boxes should be
used, unless plastic or fiberglass is required. Access to all tank compartments and distribution devices
is optimal.
INSTALLATION OBSERVATIONS
ALL SERVICE septic, LLC, and the county must view the OWS during construction. The OWS
observation should be performed before backfill, after placement of chambers and distribution pipes.
Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must
also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the
installation.
LIMITS:
The design is based on information submitted. If soil conditions encountered are different from
conditions described in report, ALL SERVICE septic, LLC should be notified. All OWS construction
must be according to the county regulations. Requirements not specified in this report must follow
county regulations. The installer should have documented and demonstrated knowledge of the
requirements and regulations of the county in which they are working.
