Fee Proposal The scope of work, and therefore our fee,is based on our understanding of the project as outlined above. If you have different expectations or desires please let us know and we will be glad to establish a scope and fee that meets your needs. 1. We will perform the duties as an independent contractor per the attached fee proposal. These rates include all home office expenses, business insurance, health and retirement benefits, cell phones, travel expenses, etc. Monthly reports would be provided indicating the work accomplished. Invoices would be submitted monthly. Payment would be expected within ten(10)days of receipt of the invoice. The only reimbursable expenses would be items like plan reproductions, postage, etc. These would be billed at cost plus 15%. 2. We have based the fee on the following: a. Dave will continue to provide the same role as he has done in previous months for the Sheriff and Administration Facility. In 2015 we have averaged$8,750 per month in salary for Dave. Our proposal takes this to a lump sum of approximately S9,200 per month which helps cover our liability insurance and overhead as well as the additional workload of the upcoming phases. In addition we have added the scope of the Ambulance Facility and the oversight of the HHS Programming. b. We have added Bruce Flynn to the staff to oversee the design and coordination of the temporary office locations. Bruce would average about 60 hours per month from Sept to June across the various projects. c. Our proposal also include Bruce completing the building standards process. d. We have added Jim Raff to oversee the day to day move operations. Jim would ramp up to being essentially '/s time during the actual moves to help be on the ground during the actual moves. e. Since the county has elected not to provide project accounting,we will provide that service through a web based system that allows the county to access real time cost data and any point in the project f. We have added Celeste Callahan for project accounting and admin support at our home office. 12 Contract#bocc.con.104.2016 Rev.8.31.11 jaa/js Budget Line Item# 110.18.92818.82726—Temporary Office Relocations 2/6/2012 lm Exhibit B: Monthly Fees 13 O.
