Contract#bocc.con.114.2016 Rea.. 8.31.11 jea/j., Budget Line Item# 110.17.92817.82275 2/6/20121 m ` I CO U T CONTRACT FOR PROVISION OF SERVICES THIS CONTRACT is made and entered by and between the Pitkin County Board of County Commissioners ("County") and Concrete Stabilization Technologies, Inc. 8500 East Warren Avenue, Denver, CO 80231 (hereinafter"Contractor"). 1. Term. The term of this contract is from June 6,2016 to June 10,2016. 2. Contractor's Obligations. Contractor shall provide foundation and trench stabilization to the west side stairs at the Historic Pitkin County Courthouse. The job description is fully defined in Exhibit A: Detailed Scope of Work. 3. Compensation and Expenses, Invoicing, Payment and Offset. The County shall compensate Contractor for its services in accordance with the Project Budget and Schedule set out in Paragraph 2. It is expressly understood and agreed that in no event will the total compensation and reimbursement to be paid hereunder exceed the sum of twenty seven thousand five hundred fifty one dollars ($27,551.00) for services rendered. By contract or amendment, the County and Contractor may reallocate the budget among project tasks if the total budget amount remains unchanged. Contractor shall invoice for the project monthly based on hours worked,with payment expected within thirty(30) days of invoice,but any payment by the County may be offset by any amount the Contractor owes the County for any reason. 4. County's Exclusive Ownership of Work Product. Drawings, specifications, guidelines and other documents prepared by Contractor in connection with this contract shall be the property of the County. However, Contractor shall have the right to utilize such documents in the course of its marketing,professional presentations, and for other business purposes. Contractor assigns to County the copyrights to all work prepared, developed, or created pursuant to this contract, including the right to: 1) reproduce the work; 2) prepare derivative works; 3) distribute copies to the public; 4) perform the works publicly; and 5) to display the work publicly. Contractor shall have right to use materials produced in the course of this contract for marketing purposes and professional presentations,articles,speeches and other business purposes. 5. Pitkin County's Obligations. Pitkin County shall administer this contract through a County Representative. Jodi Smith, Facilities Superintendent will manage the project as the County's Representative. This proposal modify s the bid by inckiding stabization of the water pipe trench and removes repair of the mortar joints from CST's scope of work. All the quaifbation information ii the bid package stil apples. Foundations&Trench Stabilization:We belbve that The URETEK Deep Injection Method71" (UDI)s idealy suited to arrest any further settlement of the west stairway and stabilize the fil soils in the water line trench. UDI precisely places expanding structural polymer(ESP)in a uniform sequential top down pattern of injection that elininates the deferential degrees of non-uniform density within the soil mats. As the polymer expands it creates an area of densification, which compacts and densifies the adjacent soil to increase/restore the bad bearing capacity of the sols. We will have bsers sensitive to 1 mm attached to the stairway. When movement is registered by the lasers,ejection will cease as the sols are now sufficiently densified to support the imposed bad or injection will continue to lift the foundation if Pitkin County desires. The process is quick and than,with little or no disruption to the Courthouse. The only debris is fines from the drrliig operation. For this project we propose lift the stairway as directed by Pitkin County. See attached Drawings #1, &2 for the proposed pattern of injection. If we are able to perform two (2)Dynamic Cone Penetrometer(DCP)tests prior to injection to confirm our proposed pattern of njection, estimated quantities, footing configuration and modify the pattern if necessary we will warranty the treated area for two years. If no DCP tests are completed we wil warranty the treated area for one year. If any settlement occurs within the warranty period re-injection will be completed at no cost to the owner. Additional information about our DCP equipment is attached and fisted under Note#4. Job/ Project Description—Stabize the sols under the stairway foundation and trench backfll. Probes:CST drives 1/2-5/8"diameter metal tubes(probes)into the sod to the appropriate depths. It is through these probes that the polymer is injected. CST wi drive these 1'bebw the surface when the injection is complete. 2. Utilities:CST wi have al utiities marked prior to instaiatbn. Private bcates are the responsbiGty of the owner. The polymer material is in a kquid state as it is intially injected under the foundation or floor. As is expands it aggressively seeks any voids. If any drain Ones or conduits are broken it "s possible for the polymer to enter the Tine and potentially clog the Cine. We wdl do our best to make sure this does not happen, but this material can be removed with a"roto rooter"type device at the owner's expense. 3. Trench Stabilization:As shown on Drawings #1 &2, injections have been added in the trench to increase the density of the weak fill sods as identfied in sods report. The densfiication of the trench will aro encapsulate the granular fi to stop it from acting as a water condut. 4. Trench Insulation:Some photos show maid insulation around the new water pipe. Our injections wdl not crush this insulation, but encapsulate it and get into any vokls that may exist between various ploces. 5. Pipe Penetration:The water line penetrating the court house foundation wal does not appear to be Faking. However if any small holes exist the expanding polymer wi seal them during the trench stabdization process. 6. Sidewalk Underpinning:It is not shown on Drawing #1 but two injections have been included to stablhe the area where the skiewak was underpinned. 7. Monitoring/Instrumentation: Ntultiple lasers wi be used to monitor upward movement. One or more emitter units wi be stationed far enough away from the njectron zone so that they will not be ii pacted. Multiple receiver units, on tripods, will be stationed on the sidewak, stair treads and alcove to monitor any upward movement. Normally to monitor sideways movementwe would attach a receiver to the star-wal and use a vertical laser to monitor any sideways movement of the wal. However since we cannot attach a unit diectly to the wall we wi have to build some type of header unit above the stair, supported on the north and south side, to support a receiver unt, dial. indi:ator or string tine and plumb bob to montor sideways movement. Our laser receiver units are sensitive to 1mrn and our dial indicators to 0.01". We would be happy to provide a sketch if necessary. 8. DCP Testing:Our estimated scope of work and pricing was completed via a visual inspeabn of the site, YEH Sods report and experience. We propose to complete two(2)DCP test prior to injections. Our DCP is a very compact device which provides continuous blow counts from the starting point to the depth of term hation. A 1 1/2"hole is dried thorough the concrete. In the 1 1/2"hob a sacrificial cone and 1 meter rods that are incremented every 10cm are inserted. These rods are driven into the sods by a percussive head(30 kg/66 lbs.)dropped a distance of 20 cm/7.88". We record the number of drops (bbws, N value)of the percussive head needed to drive the rod 10 cm/3.94".These blow counts every 10cm provide us information about your soi strengths or weaknesses.This DCP has been calbrated so that the blow counts are convertdhle to SPT(N value) blow counts. Post injection DCP tests will be completed near the pre injection DCP tests to show the improvement i7 sol density. • The DCP is used to identity the strength or weakness of the sod so that we can verify our pattern of injection and estk hated quantity of material prior to the start of the project. CST is not a geotechnical firm and offers this testing because of the compactness of the unit; speed of testing, continuous readings, and design verification. The primary purpose of this tool is to locate the weak zones in the soils and quantify the comparative degree of ground densification improvement achieved by the process,as exhibited by the increased number of percussive blows required to penetrate the treated soil mass,when compared to a pre-injection test performed within the confines of the treated mass. A certain number of penetrometer test sets(pm-&post injection)are performed on each project at CSTIURETEK's discretion for CST![TRETEK's verification of their processes and methods. The CST/URETEK penetrometer is a metric tool designated DPM 30;the percussive head weighs 30 kilos,it is dropped a constant 20 centimeters,and the blows are measured every 10 centimeters. The hydraulically operated tool is powered by an electric motor. While this tool is typically utilized exclusively by and for CST/URETEK to verify site conditions,it is also used to correlate to geotechnical Standard Penetration Tests,(Npst). The conversion factor is easily expressed by the following formula.
