This Permit must be retained and made nvailable for inspe.^.ti.on at the above location upon
request.
This Permit shall not be transferred without approval of the issuing agency.
\This Permit shall expire 120 days after its issuance if construction has not conmtenced and
b valid building permit has not keen obtained. Any mange in plans or specifications after
the permit has been issued invai.idates the permit, unless appi.-e al is secured from the
Realth Officer for such changes.
This Permit is temporary until final approval is granted by tho issuing agency in writin'.•.
Final approval cannot be granted until conKtruction, repair and/or operation have occurred
in conformity with the conditions of this. permit. This Permit is valid only for the indi-
vidual sewage disposal system and/or equ_pment spcci`ically identified herein.
Before issuing final approval of this Permit, the issuing agency reserves the right to
impose conditional terms and conditions required to meet its applicable regulations on a
Wntiuuing basis. Monitoring and testing requirements may also be imposed. The issuing
of4'icer shall be notified not less than 24 hours prior to backfilling or closing up the
wer'.t which would prevent inspection of components installed in places otherwise inaccessible
following such backfilling or closure.
This Permit may be revoked or suspended by the i_ssuino agancy '.or reasons set forth in the
regulations of said agency or of the State Board of Health, as applicable, including failure
to meet any term or condition imposed thereon during temporary approval or upon final approval..
Each acrd every condition of this Permit is a materia: part hereof and is not severable. Any
chailenge to, or appeal of, a condition hereof shall constitute a rejection of the entire
uesmit and upon such occurrence this Permit shall. he di:emed dented ab initio.
This Permit is issued and accepted with the understanding that a fee in the amount of
plus cents for each mile traveled by a designated inspector from
the principal office of the issuing agency to the location of the system and return, may
be charged for each of no more than two snuual effluent. samples routinely collected and
tested from subject system as deemed necessary t; the issuing agency to rnonirur said
system for compliance with the Provisions of Article 10, Title 25, C.R.S. 1973, as amended,
and the rules and regulations which implement said hrt'_cle; and that a like fee may be
charged for each such sampling taken in conjunction with the determination of compliance
of subject system with a cease and desist order issued pursuant to C.R.S. 1973, Section
25-10-107(1)(k).
The issuance of this Permit does riot imply compli.ance.,with other state or local regulatory
or building requirements, nor shall it act to certify that the subject system will operate
In compliance with applicable state and local regulations adopted purcant to Article 10
of Title 25, C.R.S. 1973, as amended, except for the purposes of establishi.ng final approval
of an installed system for issuance of a local uccupm,,y per;.iL pursuant to C.R.S. 1973,
25x10-111(2).
