GENERAL PROVISIONS OF AN
INDIVIDUAL SEWAGE DISPOSM. PE2hTT
:hi- Permit must be retained and made available for inspeotiin at the above location upon
:-egnes t .
This Permit shall not be transferred without apprc:a' cf the issuing agency.
This Prrmit shall expire 120 days after its issuance if constrc.:^ion has not commenced and
a valid building permit has not been obtailled. Any change :Ln plass or specifications after
th` permit has been issued inv�lldates t:a ?ermit, unless appr6val is secured from the
Health Officer for such changes.
This Permit is temporary until final approval is giante' by th- issuing agency in writing.
Final approval car -not be granted until construction, ropair andlor operation have occurred
in tz)nfor.ity with the conditions of this permit. This remit is valid only for the indi.-
:iiduai sev;age disposal system and/or equipment specif.icelly identified hereln.
Before issuing final approval of this Permit, the issuing agency reserves the ri;ht to
impose conditional terms and conditions required to meet Its applicable regulations on a
continuing basis. Monitoring and testing reauiremints may also he imposed. The issuing
officer shall he no*ified rot less than 24 houra prio: to backfillinb or closing up the
work which would prevent inspection of components installed In places otherwise inaccessible
following such back -filling or closure.
This Permit n.ry be .revolved cr suspended by the is=u_:.g agency for reasons set forth in the
regulations of said agency or of the State Board of ile ltF., as applicabl.., including failure
to•maet aay terse or condition twposed thereon during temporary approval or upon final approval.,
Each and every cor.ditior< of this Permit is a material parr. hereof a%: is not severable. Any
challenge to, nr ,apk;eal Of, a condition hermof -hall constitute a rejection of the entire
permit and upon au,+ ^ccurrence this 7er,mit shall be deemed denied ab iairic.
This Permit is issued and acccp=ed with the understanding that a fee in the amount of

be charged for each of no more than two annual effluent sanpie.s routinely collected and

it compliance with e;npiicable state and local regulations adopted pursuant to Article 10
of Tif L•: 25, C.R.S. .19.73, as amended, except for the purposes of establishing final approval
of an instali.ed system for issuanca of a local occupancy permit pursuant to C.R.S. 1973,
