Changes must be approved by this Department and the engineer prior to construction. This system is designed to serve the new caretaker unit. It will consist of a 1000 gallon, 2 -compartment tank, with an effluent filter in the outlet tee. This will gravity flow to a distribution box which must be accessible from grade. The D -box will evenly distribute effluent to 3 gravelless chamber trenches, each consisting of 17 "quick -4" chambers. Observation ports must be installed at both ends of each trench, each trench must be separated with at least 6 feet of undisturbed soil. Tank risers must be accessible from grade. This Permit must be kept on-site during installation. This Department must also be called for the final inspection with a minimum of 48 hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation, This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturers recommendations and the requirements of the Pitkin County OWTS Regulation. Western Slope Excavating installed the system. The installation included a new 1000 -gallon, two-compartment Copeland Concrete septic tank with an effluent filter in the second compartment of the septic tank. This filter must be cleaned annually, or as needed. Effluent gravity flows to a distribution box, which is accessible from grade. Effluent is distributed to three gravelless chamber trenches, each with 14 'ARC ADS' chambers for a total of 630 square feet of infiltrative area. There were least 6 -feet of undisturbed soil between each trench. Inspection ports were installed at the beginning and end of each trench. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. The property is located in Woody Creek, an area where OWTSs and wells are necessary. SITE CONDITIONS The property is currently developed with a main house and CDU, both served by an existing OWTS. The main house will remain and continue to be served by the existing OWTS, documented by Individual Sewage Disposal System (ISDS) Permit 94070. An OWTS Use Permit Inspection was conducted on May 29, 2013 and OWTS Use Permit 0023.2013.powu was issued June 3, 2013. The existing CDU will be removed and a new, 2 -bedroom CDU is proposed. The proposed CDU will be served by a new OWTS. The proposed soil treatment area (STA) is in an irrigated yard area. The area slopes slightly to the west toward the Roaring Fork River. The proposed CDU will be served by a private well. There are two wells on the property, both greater than 100 -feet from the proposed STA. 0 0 Page SUBSURFACE The subsurface was investigated on April 7, 2015 by digging three soil profile test pit excavations (Test Pits). A visual and tactile soil analysis was completed by Carla Ostberg at the time of excavation.' The materials encountered in the Test Pit #1 consisted of medium brown, moist, clay loam to 6 -feet, underlain by medium to reddish brown, moist, sandy clay loam with pit run material consisting of 2 to 4 - inch diameter cobbles, with some cobbles up to 1 -foot in diameter to a maximum depth explored of 8.0 - feet. No bedrock or groundwater was encountered. The materials encountered in Test Pit #2 consisted of medium brown, moist, clay loam to 3 -feet, underlain by medium to reddish brown, moist, sandy clay loam with pit run material consisting of 2 to 4 - inch diameter cobbles to a maximum depth explored of 8.0 -feet. No bedrock or groundwater was encountered. The materials encountered in the Test Pit #3 were consistent with those encountered in Test Pit #2. No bedrock or groundwater was encountered. A tactile analysis of the soils was taken from Test Pit #1 at 3 -feet. The soil formed a ball and a ribbon 1 to 2 -inches in length before breaking. The texture was both gritty and smooth. Soil structure shape was blocky, soil structure grade was moderate to strong, and consistence was friable. Given the varying depth of sandy clay loam with pit run material, we are sizing the STA on the more conservative soil type, Soil Type 3, Clay Loam with blocky structure and medium to strong structure grade. A long term acceptance rate (LTAR) of 0.35 gallons per square foot will be used to design the OWTS, in accordance with Pitkin County OWTS Regulation, Table 6.28-7. A design flow of 300 GPD will be used. Effluent will Flow to three gravelless chamber trenches, each with 17 'Quick 4' Infiltrator® chambers in each trench. There must be at least 6 -feet of undisturbed soil between each trench. Inspection ports must be installed at the beginning and end of each trench and may be cut to grade and covered with a sprinkler box. Page 4 The component manufacturers are typical of applications used by contractors and engineers in this area. Alternatives may be considered or recommended by contacting our office. Construction must be according to Pitkin County Onsite Wastewater Treatment System Regulations, the OWTS Construction Permit provided by Pitkin County Environmental Health Department, and this design. REVEGETATION REQUIREMENTS An adequate layer of good quality topsoil capable of supporting revegetation shall be placed over the entire disturbed area of the OWTS installation. A mixture of native grass seed that has good soil stabilizing characteristics (but without taproots), provides a maximum transpiration rate, and competes well with successional species. No trees or shrubs, or any vegetation requiring regular irritation shall be placed over the area. Until vegetation is reestablished, erosion and sediment control measures shall be implemented and maintained on site. The owner of the OWTS shall be responsible for maintaining proper vegetation cover. OPERATION INFORMATION AND MAINTENANCE The property owner shall be responsible for the operation and maintenance of each OWTS servicing the property. The property owner is responsible for maintaining service contracts for manufactured units, alternating STAs, and any other components needing maintenance. Geo -fabrics or plastics should not be used over the STA. No heavy equipment, machinery, or materials should be placed on backfilled STA. Livestock should not graze on the STA. Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS. For example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a STA. The homeowner should pump the septic tank every two years, or as needed gauged by measurement of solids in the tank. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste only. ADDITIONAL CONSTRUCTION NOTES If design includes a pump, air release valves and weep holes must be installed to allow pump lines to drain to minimize risk of freezing. The pump shall have an audible and visual alarm notification in the event of excessively high water conditions and shall be connected to a control breaker separate from the high water alarm breaker and from any other control system circuits. The pump system shall have a switch so the pump can be manually operated. Excavation equipment must not drive in excavation of the STA due to the potential to compact soil. Extensions should be placed on all septic tank components to allow access to them from existing grade. Backfill over the STA must be uniform and granular with no material greater than minus 3 -inch. INSTALLATION OBSERVATIONS ALL SERVICE septic, LLC must view the OWTS during construction. The OWTS observation should be performed before backfill, after placement of OWTS components. Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must also be observed.
