The Stein Bridge Foundation and Trail Rehabilitation (the "Agreement") dated May 13, 2015between
the Board of County Commissioners of Pitkin County (the "County") and Mueller Construction
Services, Inc. (the "Contractor"), is hereby amended as follows:
Description of Change:
Work and materials associated with adjustment of the Holy Cross manhole at the project site. The work
plans only specify"Adjust Manhole"which is significantly less work than was required.After coordination
with Holy Cross it was determined the manhole was a 6'x8' concrete vault which required being raised
three feet. The work included removal of several large boulders and excavation around the vault to install a
three foot high and 6' x 8' concrete collar and seal supplied by Holy Cross. In addition the contractor
installed 20 linear feet of 12" cmp pipe between the raised vault and roadway to drain the channel in order
to retain a 10'width of roadway.
Reason for Change:
Outside original scope of work.
Original Contract Price plus change orders 01 -03 $ 304,517.00
Net Increase in Contract Price(this change order 04) $ 2,185.00
*Total Adjusted Contract Price(including this change order) $306,702.00
In all other respects the Agreement is in full force and effect and remains unchanged by this
Amendment.
