The Provision of 911 Dispatch Relocation Consulting Services (the "Agreement") dated October 12,
2015 between the Board of County Commissioners of Pitkin County (the "County") and DiDonato
Consulting Services, Inc. (the "Contractor"), is hereby amended as follows:
Description of Change:
• County requests forty(40) additional hours of work from Contractor. The scope is for one (1)more
trip for acceptance testing. Increase the budget amount for the Contract an additional$8,500.00.
Original Contract Price $ 90,000.00
Net Increase/Decrease in Contract Price(this change order) $ 8,500.00
*Total Adjusted Contract Price(including this change order) $98,500.00
In all other respects the Agreement is in full force and effect and remains unchanged by this
Amendment.
