Hallam St. Aspen, CO 8 161 1 Phone: (970) 920-5224 lindsey.utter t6pitkincounty.com CONTRACTOR: Mike Fowler SGM Inc. 118 West 6`s Street, Suite 200 Glenwood Springs, CO 81601 Phone: (970) 384-9075 mikef@sgm-inc.com PROJECT NAME: Lazy Glen OS Bridge—Design Phase The Master Service Agreement for Provision of On -Call Engineering Services (the "Agreement") dated January 1, 2017 between the Board of County Commissioners of Pitkin County (fl c "County") and SGM Inc.; 118 West 6" St., Suite 200; Glenwood Springs, CO 81601 (the "Contractor"), shall include the following services. I. Contractor's Obligations. Contractor shall perform the following scope of work, time estimates and estimated costs per project phase. The scope of work will advance the Lazy Glen OS bridge project from the conceptual design stage through final bridge design and bidding. Including tasks to complete the 404 permit process with the US Army Corps of Engineers. Task order completion date is September 1, 2017. Phase I Final Design: • Complete the design for the preferred bridge type (prefabricated steel truss bridge) • Complete the design for the trail that will tie into the bridge from both the Lazy Glen side and the Rio Grande Trail • Prepare bid -ready plans and specifications for the complete project (bridge and trail) • Prepare a final Engineer's Opinion of Probable Cast • Design support through the bid phase Additional information: With the choice of a prefabricated steel truss bridge, the superstructure will be designed by the fabricator. The remaining engineering effort of designing the substructures (abutments) will be performed by SGM with foundation recommendations from the geotechnical engineer. Contract #: 051.2017 C-1 One day of field surveying will be required to complete the topographic survey. A base map of the proposed trail corridor will be prepared with the additional field survey information. This base map will be used to develop the trail alignment and profile. Earthwork quantities will be calculated and site drainage features will be designed. Trail design criteria will be established with input from Pitkin County Open Space. Also included in this phase is the Hydrology and Hydraulics analysis. It is anticipated that the proposed bridge structure may encroach into the 100 -year floodplain. To determine the extent of the impact, a HEC -RAS analysis will be performed to model proposed changes in the river channel. The results of the hydraulics analysis will be used to meet Pitkin County Floodplain Development requirements. Time to Complete: 2 to 3 months Completion date: April 28, 2017 Fee Estimate: $ 44,600 Phase 2 — Permitting: • Obtain authorization from the US Army Corps of Engineers (USACE) to construct the bridge • Meet Pitkin County Floodplain development requirements Additional information: Much of the background work has been completed for the 404 Pre -Construction Notification (PCN). The PCN can be completed when the final impact limits are determined by the design team. in order to facilitate the review process, we have included additional consultation time with the various review agencies for meetings onsite or at their offices. Time to Complete: 3 to 4 months (with USACE review time) Completion date: May 31, 2017 Fee Estimate: $ 8,800 Other Items which may be required, but are excluded from this scope of work are: • FEMA CLOMR or LOMR • Plat revisions • Easement agreements and exhibits (e.g. CDOT drainage easement) • Construction management and inspection Phase 3 — Bid Phase • SGM will provide plans and specifications to the County for inclusion into the bid documents. • It is anticipated that a Pre -Bid meeting will be held and there will be a nominal effort during bidding to respond to questions and evaluate bid proposals. Time to Complete: 1 month Completion date: Sept. I, 2017 Fee Estimate: $ 3,500 Total Fee Estimate for this Task Order: $56,900 Contract #: 051.2017 C-1 2. Compensation and Expenses. Invoicing. Payment and Offset. The County shall compensate Contractor for its services in accordance with the Project Budget and Schedule set out in Paragraph 1 of this Task Order. It is expressly understood and agreed that in no event will the total compensation and reimbursement to be paid hereunder exceed the sum of $56,900 for all services rendered. By Task Order m Task Order Amendment, the County and Contractor may reallocate the budget among project tasks if the total budget amount remains unchanged Contractor shall invoice for the project monthly based on hours worked, with payment expected within thirty (30) days of invoice, but any payment by the County may be offset by any amount the Contractor owes the County for any reason. Any invoices not sent in the following manner may have payment delayed. All invoices for this task order shall reference Task Order 051.2017 C-1 and Lazy Glen OS Bridge Design Phase. Invoices shall be sent electronically in PDF format to lindsey.utter@piddneounty.com 3. Pitkin County's Obligations. Pitkin County shall administer this contract through a County Representative. County Representative will manage the project as the County's Representative. The services provided andproducts delivered by the Contractor under this contract will be subject to review by the County's Representatives, or a designee, for compliance with Contractor's obligations prior to final payment. 4. Formation of Task Order. This Task Order is issued in accordance with the provisions of the Agreement. Contractor agrees to provide services subject to the terms of this Task Order and for the avoidance of doubt this Task Order consists of the terms set out in the Agreement.
