Date: 1(-2• Zoi Neill Taylor ntavlor()white-bark.com Onsite Wastewater Treatment System (OWTS) Use Permit Inspection 30 Bear Creek Lane Pitkin County, Colorado Neill, As requested, CBO Inc. performed an Onsite Wastewater Treatment System (OWTS) Use Permit inspection at 30 Bear Creek Lane, Redstone, Colorado on October 18, 2016. The legal description of the 0.525 -acre property is Lot H, Crystal River Park Subdivision. The permit and record drawing was provided by Pitkin County Environmental Health Department (Parcel ID #2729-292-01-008). The subject OWTS consists of one 1500 -gallon, two-compartment concrete septic tank. Effluent gravity flows to a drywell, described as 22' x 8' x 10' deep below the inlet. Individual Sewage Disposal System Permit 99001 documents this system. The system was sized to accommodate 3 bedrooms. The permit received final approval on August 5, 1999. The septic tank is located to the southeast of the residence. A clean out is located to the east of the front door. The manhole lids on the septic tank are located on either side of a landscaped area. The lids were less than 8 -inches below grade; however, there were rocks and other landscape features that made access difficult. We recommend leaving the area of the manhole lids accessible for future system maintenance. We have provided measurements to each septic tank lids from the corners of the residence for reference. The septic tank was pumped by B&R Septic at the time of our inspection. The septic tank is buried approximately 4.5 to 5 -feet below grade. The liquid levels in the septic tank appeared normal prior to pumping. It is difficult to inspect a septic tank at this depth. Both inlet and outlet tees were present. The engineer design and permit call for an effluent filter on the outlet tee of the septic tank; however, the filter was not present. This filter must be Installed. Upon completion of installation, please provide the invoice and a photo. Effluent from the septic tank gravity flows to a 22' x 8'x 10' deep below the inlet drywell, or seepage pit. There is one inspection port in the drywell. We measured 14'7" to the bottom of the inspection port, and 12" of effluent has ponded at the bottom of the drywell, meaning 1/10'h of the capacity of the drywell is saturated. We recommend continuing to monitor depth of effluent in the drywell. We walked the area above and down gradient of the drywell and found no evidence of surface saturation or failure at the time of our inspection. This evaluation is not a guarantee of future system performance. This inspection is good for one year. The area of drywell is 776 square feet (SF). The OWS was installed as indicated on Figure 1. The observed components of the OWS were installed in conformance with the plans and specifications.
