The Provision of Annual Pest Control Service(the"Agreement')dated March 12,2014 between the Board
of County Commissioners of Pitkin County(the "County') and Mountain Pest Control (the "Contractor").
is hereby amended as follows:
Description of Change: Additional seasonal exterior pest control service for Basalt offices located at 123
Emma Rd.. Basalt. CO 81621. Service to include treatment for the control of crawling insects and rodents.
Contractor to also control as many flying insects as possible. Service to be provided seasonally from
March through October. at a price of one hundred fifteen dollars per month (SI 15.00/month).
Reason for Change: Pest control for the Basalt offices
