The Provision of Document Scanning and Indexing Services (the "Agreement") dated April 15, 2016
between the Board of County Commissioners of Pitkin County (the "County") and Mountain States
Imaging, LLC(the"Contractor"),is hereby amended as follows:
Description of Change:Extend contract completion date to August I,2017.
Reason for Change: OST requested additional services at the beginning of 2017. Indexing the file box
contents;delivering file boxes to File Finders and re-categorization of closing packet scanned documents
these tasks should be completed by August 1,2017.
