As requested, CBO Inc. performed an Onsite Wastewater Treatment System (OWTS) Use Permit
inspection at 360 W. Sopris Creek Road, Basalt, Colorado on September 21, 2016. The legal description
of the 2.65 -acre property is Lots 25 & 26, Nystrom Tracts.
The permit and record drawing was provided by Pitkin County Environmental Health Department (Parcel
ID #2465-242-01-011). The subject OWTS consists of one 1000 -gallon, two-compartment concrete
septic tank. Effluent gravity flows to a 22' x 52' pipe and gravel soil treatment area (STA). Individual
Sewage Disposal System Permit 84032 documents this system. The system was sized to
accommodate 2 bedrooms. There are currently 3 bedrooms in the residence.
The septic tank is located south of the residence. No clean out was visible between the house and the
septic tank. Both manhole lids were located approximately 2 -feet below grade and had to be dug up.
The plumbing entering and exiting the septic tank was an angled fitting. These angled fittings will be
replaced with a proper open "tees". Risers must be added so the tank is accessible from grade. New
lids will be required as the current lids will not fit the new risers properly.
The septic tank was pumped by All Valley Resource as part of the inspection.
We walked the approximate STA and found a metal plate that we were not able to remove. Upon close
inspection, there appears to be a PVC pipe under the metal plate. The port was dry. There was no
evidence of surface saturation and no evidence of failure in the STA at the time of the inspection.
This evaluation is not a guarantee of future system performance. This inspection is good for one year.
Required Repairs:
• Add risers to bring septic tank manholes lids to grade.
• Replace septic tank lids for a proper fit on new risers.
• Replace plumbing in and out of septic tank with proper inlet and outlet tees.
Recommendations:
• Install effluent filter on the outlet tee of the septic tank.
This report and a receipt confirming the required repairs must be submitted to the Pitkin County

based upon what was observed and the Licensed Inspector's expertise in onsite wastewater technology.
The Inspector has not been retained to warrant or guarantee the proper functioning of the system for any
period of time in the future. Because of numerous factors which may affect the operation of an OWTS,
as well as the inability of the Inspector to supervise or monitor the use or maintenance of this OWTS, this
report shall not be construed as a warranty by the Inspector that the system will function properly for any
particular prospective buyer, and the Inspector disclaims any warranty, either expressed or implied,
arising from the inspection of the OWTS or this report. The evaluation does not ascertain the impact the
system is having on groundwater.

Not sure if we got a bunch of pics, I think Ray took one of the inlet side and we can forward to Carla for the report
Narrative:
1. Removed and disposed of the original lids
2. Cut and removed old inlet pipe and installed new 4" inlet T
3. Cut and removed old outlet pipe and installed new 4" inlet T— extended outlet pipe by 4 or 5 inches in order to provide room if
the owners want to install an outlet filter
4. Installed a single riser and lid on the inlet side
5. Installed a single riser and lid on the out let side
6. Backfilled the entire area, each lid is covered with 3" of soil

and have the tank pumped, when necessary, generally every three to five years.
Don't:
• Pour cooking grease or oil down the
sink or toilet.
• Rinse coffee grounds into the sink.
• Pour household chemicals down the
sink or flush them.

• Flush non -degradable products or
chemicals, such as feminine hygiene
products, condoms, dental floss,
diapers, cigarette butts, cat litter, paper
towels, pharmaceuticals.
Don't:
• Park or drive on your drainfield. The
weight can damage the drain lines.
• Plant trees or shrubs too close to your
drainfield, roots can grow into your
system and clog it.

• Concentrate your water use by using
your dishwasher, shower, washing
machine, and toilet at the same time.
All that extra water can really strain
your septic system.
