Operational Status: According to the inspectors observations the system was functioning as designed. Both tanks were in good,
water -tight condition with inlet and outlet tees in place. The tanks gravity flowed to a distribution box that was not accessible from
grade that split flows between the 1977 bed and 1992 addition of trenches. There were no observation ports to check for standing
effluent, there was no evidence of failure

Estimated capacity of the system has been listed on the permit and we recommend that you monitor and/or meter water use to prevent overuse and possible
failure.

Onsite Wastewater Treatment System (OWTS) Use Permit Inspection

As requested, CBO Inc. performed an Onsite Wastewater Treatment System (OWTS) Use Permit
inspection at 124 Holland Hills Road, Basalt, Colorado on November 8, 2016. The legal description of
the 2.1 -acre property is Lot 18, Holland Hills at Basalt.
The permits and record drawings were provided by Pitkin County Environmental Health Department
(Parcel ID #2467-174-01-014). The subject OWTS consists of two 1000 -gallon, two-compartment
concrete `VCP' septic tanks. Effluent gravity flows to a distribution box, and distributes effluent between
the original (1977) 13'x 100' pipe and gravel soil treatment area (STA) and two chamber trenches
(1993), each with 17 `Standard' Infiltrator® chambers. The original OWTS is documented by Pitkin
County Health Department Permit Number 77104, which was given final approval on March 31, 1978.
Additional STA was added under Individual Sewage Disposal System Permit 92043. The system was
sized to accommodate 6 bedrooms. The permit received final approval on April 13, 1993.
The septic tanks are located to the southwest of the residence. These septic tanks have access two
access points to the inlet side of the septic tank and one access point to the outlet side of the septic tank.
Only the inlet sides were accessible initially; however, the outlet access was dug up and brought to grade
with a sprinkler box. Inlet and outlet baffles have a PVC tee cast into the concrete. The concrete outlet
baffle on both septic tanks are showing signs of significant deterioration, but are still intact.
The first septic tank in the series was pumped by B&R Septic at the time of our inspection. The second
septic tank in the series did not have enough solids accumulated to warrant pumping.
The distribution box was not accessible and therefore, we were not able to inspect this component. We
recommend locating the distribution box and making it accessible from grade for future monitoring and
inspection. We walked the approximate STA and found greener grass over one of the trenches. No
inspection ports were visible in the STA. There were no signs of surface saturation or evidence of failure
at the time of our inspection.
This evaluation is not a guarantee of future system performance. This inspection is good for one year.
Recommendations:

arising from the inspection of the OWTS or this report. The evaluation does not ascertain the impact the
system is having on groundwater.
