Changes must be approved by this Department and the engineer prior to construction. This is a permit to add a septic tank to an existing OWTS. The new tank would serve a new residence and utilize the existing soil treatment area. Per thje engineer, the existing soil treatment area is adequate to serve 4 bedrooms total. A new 1000 - gallon, two-compartment tank will be installed with a pump installed in the second compartment. Effluent will be pumped to the existing soil treatment area. The existing tank will remain to serve the existing 3 -bedroom residence. The new tank must be a minimum of 50 feet to any, well. This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this Department within 30 days of tho final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection with a minim m of 48 -hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation. This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail. Native Ex installed the system. A new 1000-gc'llon, two-compartment concrete septic tank was installed with an Orenco® Biotube Pump Vault and PF3005 pump in the second compartment. The control panel for the pump is located in the garage. Valle Precast personnel performed start up on the pumping system on January 26, 2017. Handle extensions for the effluent filter and float tree will be installed during Valley Precast's next visit to the property. A 2.0 -inch diaoeter transport pipe was installed and runs from the new septic tank to a distribution box at the high point.' A 4 -inch diameter pipe exits the distribution box and gravity flows to the septic tank serving the main residence, entering the side knock -out. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are' working. The property is located outside of Basalt, in an area where OWTSs and wells are necessary. The legal description of the property ("Site") is Lot 14, West Sopris Ranch. SITE CONDITIONS An existing residence is served by an existing OWTS. Two Individual Sewage Disposal System (ISDS) Permits documjent this system. ISDS Permit #03028 documents the original system installation. ISDS Permit #04084' documents the septic tank replacement. An as -built drawing provided by Church and Associates, LL� dated 9/29/05 describes the OWTS as consisting of one 2000 -gallon, two-compartment concrete septiq tank followed by four gravelless chamber trenches, each with 14 `Standard' Infiltrator® chambers in each row for a total of 56 `Standard' Infiltrator® chambers and 1050 square feet of infiltrative area. A private well oh site is located greater than 50 -feet from the proposed septic tank. The Site is covered vegetated with sage and other native grasses with a slight slope to the north. DESIGN SPECIFICATIONS Initial design and soils documents have been reviewed to determine if the existing OWTS can accommodate 4 -bedrooms. Soil documentation from both Church and Associates, LLC's May 8, 2003 design document and soil information provided in HP Geotech's report dated April 28, 2003, Job 103199 classify soils that are consistent with Soil Type 2A under current Pitkin County OWTS Regulation. Minimum required OWTS capacity for a three-bedroom main house with less than 6000 square feet of living area and a one -bedroom ADU under current Pitkin County OWTS Regulation requires 1250 -gallons of tank capacity and 840 square feet of infiltrative area using 0.50 gallons/square foot long term acceptance rate (LTAR), gravity trench application, and chambers for the distribution media. The existing OWTS consists of 2000 -gallons of septic tank capacity and 1050 square feet of infiltrative area (using 10.75 square feet per `Standard' Infiltrator® chamber); therefore, the existing OWTS is sufficient to accommodate 4 total bedrooms on the property; however, an additional septic tank and Page 2 pump must be installed near the ADU to transport effluent to the existing OWTS due to grade. Topographic lines were only provided on a portion of the Site pertaining to construction of the ADU. Elevation difference between the outlet of the proposed septic tank serving the ADU and the outlet side of the existing septic tank must be verified to assure proper pump sizing. A new 1000 -gallon, two-compartment septic tank will be installed with an Orenco® Biotube Pump Vault and pump in the second compartment. The control panel for the pump must be located within line of sight of the septic tank. Plastic tanks such as Roth® or Infiltrator® may be considered if access for the concrete tank is difficult. Elevation and distance must be confirmed to verify proper pump sizing. A 1.5 -inch diameter transport pipe will be installed and run to the outlet, side knock -out of the existing septic tank. This can transition to a 4 -inch diameter SDR 35 pipe just before entering the septic tank. We recommend the existing distribution box be uncovered to check for level and equal distribution to the trenches, and risers added for accessibility from grade for future monitoring. Flow equalizers should be installed on the outlet lines to assure equal distribution to the trenches. The existing septic tank has one manhole lid was in disrepair, which has been covered with another style lid that does not fit properly. We recommend replacing the original lid with one from the original tank manufacturer, Copeland Concrete out of Rifle, Colorado. The component manufacturers are typical of applications used by contractors and engineers in this area. Alternatives may be considered or recommended by contacting our office. Construction must be according to Pitkin County Onsite Wastewater Treatment System Regulations, the OWTS Construction Permit provided by Pitkin County Environmental Health Department, and this design. REVEGETATION REQUIREMENTS An adequate layer of good quality topsoil capable of supporting revegetation shall be placed over the entire disturbed area of the OWTS installation. A mixture of native grass seed that has good soil stabilizing characteristics (but without taproots), provides a maximum transpiration rate, and competes well with successional species. No trees or shrubs, or any vegetation requiring regular irritation shall be placed over the STA. Until vegetation is reestablished, erosion and sediment control measures shall be implemented and maintained on site. The owner of the OWTS shall be responsible for maintaining proper vegetation cover. OPERATION INFORMATION AND MAINTENANCE The property owner shall be responsible for the operation and maintenance of each OWTS servicing the property. The property owner is responsible for maintaining service contracts for manufactured units, alternating STAs, and any other components needing maintenance. Geo -fabrics or plastics should not be used over the STA. No heavy equipment, machinery, or materials should be placed on backfilled STA. Livestock should not graze on the STA. Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS. For example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a STA. The homeowner should pump the septic tank every two years, or as needed gauged by measurement of solids in the tank. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste only. Page 3 ADDITIONAL CONSTRUCTION NOTES If design includes a pump, air release valves and weep holes must be installed to allow pump lines to drain to minimize risk of freezing. The pump shall have an audible and visual alarm notification in the event of excessively high water conditions and shall be connected to a control breaker separate from the high water alarm breaker and from any other control system circuits. The pump system shall have a switch so the plump can be manually operated. Excavation equipment must not drive in excavation of the STA due to the potential to compact soil. Extensions shojuld be placed on all septic tank components to allow access to them from existing grade. Backfill over the STA must be uniform and granular with no material greater than minus 3 -inch. INSTALLATION OBSERVATIONS ALL SERVICE peptic, LLC must view the OWTS during construction. The OWTS observation should be performed before backfill, after placement of OWTS components. Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the installation. LIMITS The design is based on information submitted. If soil conditions encountered are different from conditions described in report, ALL SERVICE septic, LLC should be notified. All OWTS construction must be according to the county regulations. The permits and record drawings were provided by Pitkin County Environmental Health Department (Parcel ID #2465-153-00-006). Two Individual Sewage Disposal System (ISDS) Permits document this system. The septic tank lids were accessible from grade. One manhole lid was in disrepair, but had been covered with another style lid that does not fit properly. We recommend replacing the original lid with one from the original tank manufacturer, Copeland Concrete out of Rifle, Colorado. The septic tank] appeared to be in good condition, with both inlet and outlet tees present. An effluent filter was present and was cleaned as part of the inspection. Scum and sludge levels in the septic tank do not warrant pumping at this time. The distribution box was not accessible from grade. There are four inspection ports present at the end of each row of chambers. There was a good cover of native vegetation over the soil treatment area, with no signs of surface saturation or failure at the time of the inspection. This evaluation is not a guarantee of future system performance. This inspection is good for one year. Recommendations: • Clean effluent filter annually, or as needed. • Replace inlet manhole lid with one from the same manufacturer as the tank for proper fit. The system was designed under our Job No. 15705W, dated May 8- 2003, revised December 29 200, The site waspermitted for a two-bedroom residence with a living area under 2000 ft ,based on the May 8, 2003 design. The revised design was not submitted to the county for review by our original client, howevor the installation was based on the revised design. The system includes the installation of a 1000 -gallon, two-compartment, concrete septic tank with an effluent filter on the outlet, and four gravelless chamber -trenches. Each trench has one row of 14 chambers for a total of 54SbWber, and 868 ft2 of infiltrative area. An as -built drawing is included as Figure 1. As installed, t e system will serve a two-bedroom residence with a living area between 2000 ft2 and 6000 ft2. To serve a three-bedroom residence with a living area between 2000 ft2 and 6000 ft2, a 1000 -gallon, single -compartment, concrete septic tank must be added in series prior to the existing septic tank. To serve a four-bedroom residence with a living area between 2000 ft2 and 6000 ft2, a septic tank must added as described above and one gravelless chamber must be added to each trench to bring the total to 60 chambers and P30 ft2 of infiltrative area. Ifthe OWS is to be upgraded in the fixture, the upgrade must be designed by a professional engineer and submitted to the county for a new septic permit by the client. The observed components of the OWS were installed in general conformance with our design. Final electrical connections must be made by a licensed electrician.
