Changes must be approved by this Department and the engineer prior to construction. This permit is to replace a failed OWTS. The existing OWTS must be properly abandoned and evidence of proper abandonment submitted to this office as part of the final approval of this permit. Sewage will flow via gravity from the residence to a new 1500 -gallon concrete tank with a traffic rated lid and a pump installed in the second compartment of the tank. Effluent will be pressure dosed to a new 12' X 50' mounded sand filter STA (2 feet of sand above grade, 3.5 feet of total material over groundwater level; 3 feet above groundwater level required for TL1 STA that is pressure dosed). The sewer and effluent lines must be encased within 50 -feet of all surface water. The tank and STA must be a minimum of 50 feet to surface water. The STA must also be a minimum of 25 feet to any water line and 10 feet to the property line. Due to repair work being delayed by the sale of the property, this permit will expire in 45 days instead of the usual 30 days. This Permit must be kept on-site during installation. The engineer must do a final inspection of the installation and submit an "as -built" letter to this Department within 30 days of the final inspection, unless a longer period of time has been agreed upon, in writing. This Department must also be called for the final inspection with a minimum of 48 hours notice. Minimum horizontal distances between components of the system and physical features shall conform to the requirements of the Pitkin County OWTS Regulation. This Permit is conditioned upon the property owner(s) providing for regular inspection, cleaning, and maintenance of the system by qualified personnel, in accordance with the manufacturer's recommendations and the requirements of the Pitkin County OWTS Regulation. Issuance of this OWTS Construction Permit does not constitute a guarantee, warranty, or representation by the Department that the system will operate properly or will not fail. Altitude Septic installed the system. The existing septic tanks were pumped and properly abandoned by crushing the bottom, sides and top and filling the void with fill material. The existing mounded STA was abandoned in place. The sewer pipe from the residence to the septic tank was encased for approximately 78 -feet in order to maintain the minimum required setback from the irrigation ditch. The clean out was installed just before the septic tank rather than near the residence to maintain the integrity of the encasement pipe. The location of the mound system was rotated and moved slightly to accommodate the required 25 -foot setback to the Little Elk Creek Subdivision water line that runs through the property. The location of the water line was described and estimated on site by Dan Harris, representative of the Little Elk Creek Homeowner's Association. The installation included a new 1500 -gallon, two-compartment concrete septic tank with Orenco® ProPak with a PF3005 pump. The floats were set to deliver approximately 90 gallons per dose. There is an effluent filter in the second compartment of this tank. The filter must be cleaned annually, or as needed. Pump start-up was conducted by Valley Precast personnel on November 1, 2016. The control panel is located on the northeast corner of the house, within line of sight of the septic tank. Groundwater was not encountered within 2 -feet from the top of the tank, and therefore no anchoring was required. The tank was filled with water to assure stability of the tank during backfill. We recommend future pumping of the septic tank only occur during drier months, and not at a time of high groundwater. Effluent is pressure dosed through a 1.5 -inch diameter pump line. The 1.5 -inch diameter pump line has a minimum 1% grade for proper drain back into the septic tank after each pump cycle. The 12' by 50' sand filter mound consists of 3 -feet of sand filter material, with 2 -feet extending above native grade to maintain the required separation between the infiltrative area and high seasonal groundwater. Laterals used to disperse the effluent are surrounded by washed coarse screened gravel or crushed stone. A 1.5 -inch diameter level manifold connects three 1.5 -inch diameter laterals, each with 5/32" diameter orifices facing down, 3 -feet on center. Each 1.5 -inch diameter lateral ends in a sweeping ell facing up with a ball valve for flushing. A soil separation fabric was placed over the gravel layer followed by approximately 1 -foot of topsoil. The OWTS was generally installed according to specifications. This observation is not a guarantee of workmanship and/or parts and materials. ALL SERVICE septic, LLC should be notified if changes are made to the OWTS in the future. Any additional OWTS construction must be according to the county regulations. LIMITS: Observations are limited to components that are visible at the time of the inspection. The installer must have documented and demonstrated knowledge of the requirements and regulations of the county in which they are working. The quality of the installation is dependent of the expertise of the installer, soil type, and weather conditions. Please call with questions. The property is located in Old Snowmass, in an area where OWTSs are necessary. The future owners of the property are contemplating a bedroom addition. Revisions have been made to the original OWTS design documents dated April 11, 2016 and May 15, 2016. These documents should be discarded and replaced with documents dated June 14, 2016. The legal description of the 1.92 -acre property is Lot 1, Block 2, Filing 2, Little Elk Creek Village. Parcel ID #2645-042-02-005 SITE CONDITIONS A 3 -bedroom, single-family residence presently exists and is utilizing an existing OWTS that is experiencing problems. The existing OWTS is documented by Aspen/Pitkin Environmental Health Department Permit 80033. The OWTS consists of one 1000 -gallon, two-compartment concrete septic tank followed by a 5 -foot diameter concrete pumping chamber with a pump. Effluent is pumped to a mounded soil treatment area (STA). The residence is provided potable water by the community water system that serves Little Elk Creek Subdivision. We have estimated the location of the water line on the property. This water line must be located and must maintain a minimum 25 -foot setback to any OWTS component. If different than depicted on our drawings, this office must be contacted. An irrigation ditch and pond are present on the property. This design has located the OWTS components a minimum of 50 -feet from the water courses. If there is a desire to locate the components within the 50 - foot setback, the ditch must be lined or placed in a culvert where it comes within 50 -feet of the proposed OWTS components. The sewer line from the house to the septic tank must be properly encased within 50 -feet of each side of the ditch (or 10 -feet if the ditch is lined or placed in a culvert). See encasement detail 3/W4.0. The proposed soil treatment area (STA) is relatively flat and located in an area prone to high groundwater conditions. The site plan provided to this office was not complete with topography. r Page 2 The subject property is located within the 100 -year flood plain. A mini -excavator was used to excavate the Test Pits. A visual and tactile soil analysis was completed by Carla Ostberg at the time of excavation.' The materials encountered in Test Pit #1 consisted of dark brown, moist topsoil with red and gold mottling observed at 2 -feet below grade, underlain by medium brown, moist sandy gravel to a maximum depth explored of 4.5 -feet. No bedrock or groundwater was encountered; however, we estimate seasonal groundwater to be present at 2 -feet below grade in this area. The materials encountered in Test Pit #2 consisted of dark brown, moist topsoil to a depth of 1 -foot, underlain by medium brown, moist sandy gravel with red and gold mottling observed at 1.5 -feet below grade. The pit was explored to a maximum depth of 4.5 -feet. No bedrock or groundwater was encountered; however, we estimate seasonal groundwater to be present at 2 -feet below grade in this area. A soil sample was taken from Test Pit #1 at approximately 1 -foot below grade. The sample formed a ball but did not form a ribbon. The texture of the soil was gritty. The soil had granular to blocky structure shape, moderate structure grade, and loose to friable consistence. A sample was taken from 4 -feet below grade. This sample did not form a ball. Sizing of the basal area of the STA will be based on Loamy Sand to Sand, Soil Type 1. The OWTS design is based on 4 - bedrooms. A design flow of 600 GPD will be used. The existing mounded STA may be abandoned in place or removed. The installation must include a new 1500 -gallon, two-compartment concrete septic tank with OrencoO ProPak with a PF3005 pump. If groundwater is found within 2 -feet of where the top of the tank will be set in the location of the septic tank, this office must be notified and a H-20 rated lid must be installed. The floats should be set to dose approximately 90 gallons each pump cycle. The control panel for the pump must be located within line of sight of the septic tank. Effluent will be pressure dosed through a 1.5 -inch diameter pump line. The 1.5 -inch diameter pump line must have a minimum 1% grad for proper drain back into the septic tank after each pump cycle. A sand 12' by 50' sand filter mound will be onstructed. Topsoil and vegetation must be scraped from the surface and replaced with sand filter material. Sand filter material must be clean, coarse sand, all passing a screen having four meshes to the inch. The sand must have an effective size between 0.25 and 0.60 mm. The uniformity coefficient must be 4.0 or less. Material meeting ASTM 33, for concrete sand, with one percent or less fines passing 200 mesh sieve may be used. Laterals used to disperse the effluent must be surrounded by washed coarse screened gravel or crushed stone. All of the gravel or stone must pass a 2'/z -inch screen and must be retained on a 3/4 -inch screen. A 1.5 -inch diameter level manifold will connect three 1.5 -inch diameter laterals, each with 5/32" diameter orifices facing down, 3 -feet on center. Each 1.5 -inch diameter lateral must end in a sweeping ell facing up with a ball valve for flushing. El Page 5 The mound must have a minimum 3:1 slope (horizontal: vertical); therefore, the basal area will extend 9 -feet beyond the outside perimeter of the sand. Soils in the sloped perimeter areas must be well draining and able to support vegetative growth. This soil must be approved by this office prior to installation. The mound must be crowned in a manner to promote drainage off of the mounded area. The component manufacturers are typical of applications used by contractors and engineers in this area. Alternatives may be considered or recommended by contacting our office. Construction must be according to Pitkin County Onsite Wastewater Treatment System Regulations, the OWTS Construction Permit provided by Pitkin County Environmental Health Department, and this design. REVEGETATION REQUIREMENTS An adequate layer of good quality topsoil capable of supporting revegetation shall be placed over the entire disturbed area of the OWTS installation. A mixture of native grass seed that has good soil stabilizing characteristics (but without taproots), provides a maximum transpiration rate, and competes well with successional species. No trees or shrubs, or any vegetation requiring regular irritation shall be placed over the STA. Until vegetation is reestablished, erosion and sediment control measures shall be implemented and maintained on site. The owner of the OWTS shall be responsible for maintaining proper vegetation cover. OPERATION INFORMATION AND MAINTENANCE The property owner shall be responsible for the operation and maintenance of each OWTS servicing the property. The property owner is responsible for maintaining service contracts for manufactured units, alternating STAs, and any other components needing maintenance. Geo -fabrics or plastics should not be used over the STA. No heavy equipment, machinery, or materials should be placed on backfilled STA. Livestock should not graze on the STA. Plumbing fixtures should be checked to ensure that no additional water is being discharged to OWTS. For example, a running toilet or leaky faucet can discharge hundreds of gallons of water a day and harm a STA. If an effluent filter or screen has been installed in the OWTS, we recommend this filter or screen be cleaned annually, or as needed. If the OWTS consists of a pressurized pump system, we recommend the laterals be flushed annually, or as needed. The homeowner should pump the septic tank every two years, or as needed gauged by measurement of solids in the tank. Garbage disposal use should be minimized, and non -biodegradable materials should not be placed into the OWTS. Grease should not be placed in household drains. Loading from a water softener should not be discharged into the OWTS. No hazardous wastes should be directed into the OWTS. Mechanical room drains should not discharge into the OWTS. The OWTS is engineered for domestic waste only. ADDITIONAL CONSTRUCTION NOTES If design includes a pump, weep holes must be installed to allow pump lines to drain to minimize risk of freezing. The pump shall have an audible and visual alarm notification in the event of excessively high water conditions and shall be connected to a control breaker separate from the high water alarm breaker and from any other control system circuits. The pump system shall have a switch so the pump can be manually operated. 0 Q Page 6 Excavation equipment must not drive in excavation of the STA due to the potential to compact soil. Extensions should be placed on all septic tank components to allow access to them from existing grade. Backfill over the STA must be uniform and granular with no material greater than minus 3 -inch. INSTALLATION OBSERVATIONS ALL SERVICE septic, LLC must view the OWTS during construction. The OWTS observation should be performed before backfill, after placement of OWTS components. Septic tanks, distribution devices, pumps, dosing siphons, and other plumbing, as applicable, must also be observed. ALL SERVICE septic, LLC should be notified 48 hours in advance to observe the installation. LIMITS: The design is based on information submitted. If soil conditions encountered are different from conditions described in report, ALL SERVICE septic, LLC should be notified. All OWTS construction must be according to the county regulations. Requirements not specified in this report must follow applicable county regulations.
