advanced treatment system did not have a maintenance agreement at the time of inspection but is now in place. The

Septic permit approved' per compliance with the engineer design and specifications stamped and revised
09196104. Any changes must be approved by this department and the design engineerrp for to them being made.
Minimum horizontal distances between components of the system and physical features shall conform to the Pitkin
County ISDS regulations.
'Because of the slow percolating soils, the design is for a drip dispersal system based on an LTAR rate of 0.2.
There is one 2,500 gallon tank followed by a 1,000 gallon tank with two Advantex AX -20 filter modules. Both tanks
are single compartment, concrete tanks. The field is six zones of Geoflow driplines for a total of 5,700 lineal ft, for
an effective area of 11,875 sq ft. There is a two foot separation between each dripline. Driplines are kept shallow,
but must be a minimums of 12 inches below the surface of the ground.
This department does riot endorse any brand of products. This permit must be kept on-site during installation.
The engineer must do a final inspection of the installation and submit an as -built letter to this department. This
department must also be called for an inspection with a minimum of 48 hours advanced notice.
'Revegetation over the area of the field is very important for the functioning of the system. Pitkin County has guidelines that must be
followed to assure the plants that are introduced are appropriate for the conditions of the area. The use of native plants is strongly
encouraged.

Plans and specifications of the oposed individual sewage disposal system have been reviewed and are considered
satisfactory. Permission is hereby granted to the owner or the agent to perform the work indicated in accordance with the Pitkin
County ISDS Regulation in effect at the time of issue. This permit becomes invalid 6 months from the date that the permit was
issued unless system construction has commenced or an extension has been approved in writing by the Department.

As requested, �BO Inc. performed an Onsite Wastewater Treatment System (OWTS) Use Permit
inspection at 1000 Lazy O Road, Snowmass, Colorado on March 20, 2017. The legal description of the
7.83 -acre property is Lot 7, Lazy O Ranch Subdivision.
The permit andidrawing of the proposed system' was provided by Pitkin County Environmental Health
Department (Parcel ID #2645-034-01-007). The subject OWTS consists of one 2500-gallon2 concrete
septic tank followed by 1000 -gallon concrete septic tank that serves as a re -circulating tank with the
recirculating splitter valve (RSV) and recirculating pump. Effluent gravity flows to pump vault located
downhill and west of the residence, east of Lazy O Road. The Geoflow@ headworks is located near the
pump vault. Effluent is pumped to an automatic distributing valve (ADV) located just south of the
driveway. The Geoflow® drip line or soil treatment area (STA) is located in the open field south of the
driveway and east of Lazy O Road. The STA consists of six zones, each with 5 drip lines, 190 -feet long
for a total of 950 feet of drip line in each section. Individual Sewage Disposal System Permit 04072
documents thisi system. The system was sized to accommodate 5 bedrooms. The permit received
final approval on July 27, 2006.
This system has advanced treatment technology and requires routine maintenance. We understand
there is no pass or current maintenance contract on this system. A maintenance contract must be
secured for cgntinued operation and maintenance of this system. We recommend Valley Precast
out of Buena Vista for this work.
A clean out is located to the north of the residence. A manhole access, which has 10 -feet of risers,
provides access to where the sewer line from the main house and sewer line from the caretaker dwelling
unit (CDU) come together. This manhole access appears to have an accumulation of some debris, but
flow through the access appeared normal. The septic tanks and two AdvanTex® AX 20 pods are located
west of the residence, just below the driveway. We believe the first septic tank in the series is 3000 -
gallons. This septic tank has both inlet and outlet tees. The septic tank was pumped by Altitude Septic
at the time of toe inspection. There was a significant amount of debris flowing over the inlet tee prior to
pumping.
Half of the volume of the septic tank housing the RSV and recirculating pump was pumped at the time of
the inspection. The pump truck was full and can return for completion of pumping, if necessary. We did
leave some effluent in the septic tank in order to run the pumping system safely during maintenance.
The effluent filter was extremely clogged and was cleaned at the time of pumping. The RSV was also

As requested, �BO Inc. performed an Onsite Wastewater Treatment System (OWTS) Use Permit
inspection at 1000 Lazy O Road, Snowmass, Colorado on March 20, 2017. The legal description of the

dirty and was cleaned at the time of the inspection. We opened the valves on the laterals in the AX pods
to flush them and black sludge came out of the laterals. The last lateral in both pods did not have flow
and we believe these laterals are blocked. Additional flushing will be necessary to properly clean the
laterals and pods.
We observed the discharge pump, which is located down the hill below the Geoflow(@ system. The filter
appears to be clogged and in need of cleaning. We did not have water available at that location to clean
the filter. The headworks for the Geoflow® system is located near the discharge pump tank. There
appears to be bjroken piping in the headworks that will require repair work.
The control panI is located on the north side of the residence.
We observed the ADV which is located south of the driveway, uphill of the STA. It is accessible in a riser.
The STA was observed and it appears there are air release valves at the end of each Geoflow® drip line
section which are accessible from grade. We walked the STA and did not observe any evidence of
surface saturation or failure at the time of our inspection.
Repairs and continued routine maintenance is necessary prior to obtaining the required OWTS Use
Permit for thisWTS.
Required Repa rs:
• Hinges and supports on the second AX 20 pod must be repaired.
• Handle extensions must be installed on the effluent filter and float tree in the recirculating tank.
• The lid on the manway covering the headworks is very deteriorated (too deteriorated to put back
on) and must be replaced.
• The discharge pump tank should be pumped (with access from Lazy O Road) and the discharge
filter must be cleaned.
• Handle extensions must be installed on the effluent filter and float tree in the discharge tank.
• The AdvanTex® system and Geoflow® system must be flushed by a qualified maintenance
provider.
• A contract must be secured by a qualified maintenance provider to assure proper long-term
maintenance.
This report, documentation of repairs and routine maintenance by a qualified maintenance
provider, and a completed OWTS Use Permit Application must be submitted to the Pitkin County
Environmental Health Department. Applications may be submitted to Pitkin County Environmental Health

as well as the inability of the Inspector to supervise or monitor the use or maintenance of this OWTS, this

particular prospective buyer, and the Inspector disclaims any warranty, either expressed or implied,
arising from the inspection of the OWTS or this report. The evaluation does not ascertain the impact the
system is having on groundwater.

Alarms: Alarms will Oe responded to via VeriComm remotely at no charge. If an onsite visit is needed VPI will respond twice at
no charge before servio call pricing begins. *Any materials not under warranty will be billed to owner.
Service Calls: Servicc calls will be billed at a rate of $80.00 per hour and $50.00 per hour for travel time. A visit to the site other
than the routine visits I rovided by this agreement or a system warranty issue will be deemed a service call. Examples would
include (but are not firr. 'ted to) damage caused by acts of vandalism, failure of tank or failure of another party's work, i.e.
electrician, plumber, excavator, etc. If a safety concern is present owner will be contacted immediately to resolve issue.
The goal of VPI is too serve the operation and perform maintenance for the Onsite Wastewater System to allow for proper long-
term function of the sy tem and compliance with the conditions of the installation permit and recommendations of the
manufacturer.
VPI will perform O& services as outlined for $285 per visit, with two visits per year ($570 per year). Payment in full for the
(1, 2, 3, 4, 5) years is r -,quired prior to initiation of the agreement. This agreement period will be for the (1, 2, 3, 4, 5) year period
subsequent to placing t ie system into operation. If you renew for 5 years, you will receive a 10% discount.
